However, there is a lack of systematic knowledge about the opinions of radiological staff on the most relevant aspects of sustainability. For this reason, we conducted a comprehensive survey for radiology employees on sentiment and experiences regarding sustainability in radiology.In collaboration with the Sustainability Network of the German Roentgen Society (DRG), we developed a questionnaire on various dimensions of sustainability in radiology. We conducted a nationwide online survey of radiology employees between July 1<sup>st</sup>, 2023 and November 30<sup>th</sup>, 2023. The absolute and percentage distributions were then determined.From 109 participants, mainly doctors (67/109; 62%) from university hospitals (48/109; 44.0%), 81 out of 109 rated sustainability in professional environment (74.3%) as important or very important. However, only 38 out of 109 (38%) of the respondents were able to name specific sustainable procedures in their institute. The most important topics for a sustainable radiology were waste management (26/109, 22.6%), energy reduction (19/109, 16.5%), conscious behaviour (15/109, 13%) and reduction of obsolete examinations (14/109, 12.2%). In addition, a lack of qualifications (16%), finances (21%) and compliance (21%) were named as challenges for the implementation of sustainable actions in radiology. The perceived importance of specific, sustainable measures in radiology is generally higher than the amount of already established actions.Radiology has significant, yet untapped, potential for sustainable optimization. There is a need for qualified and sensitized health care workers in radiology who are committed to sustainability in everyday clinical practice. Among other things, in this study the respondents demand a more critical indication for diagnostic workup, including avoiding redundant examinations, and a technological progress towards energy-efficient devices, which requires a dynamic exchange between radiology, industry and health care facilities. · Of 109 respondents from radiology departments, 74.3% consider sustainability to be important or very important in a professional context.. · Waste management (22.6%), energy reduction (16.5%), conscious behaviour (13%) and reduction of obsolete or redundant examinations (12.2%) are, according to those surveyed, most important for a more sustainable radiology.. · Sustainability initiatives have been institutionally established among 38% of participants. After assessment and evaluation of the reports, the competent authority submits the relevant information via a web-based reporting system (BeVoMed) to the Federal Office for Radiation Protection (BfS), which publishes the results derived therefrom. The present paper evaluates significant incidents related to X-ray exposures on humans.All reports on incidents in X-ray diagnostics and interventional radiology between 01/2019 and 10/2023, which were completed with detailed information until the reporting day (31/10/2023), were included. The following aspects were statistically evaluated: classification as an incident (section 1 subsection (22) StrlSchV), significance (section 108 StrSchV), classification to Annex 14 StrlSchV, assignment to the forms of care in the German healthcare system, and development of the reporting frequency over time. Furthermore, the content of the reports was systematically evaluated with regard to conspicuous clusters and typical problems.Until the reporting day, 383 reports (355 completed) were received. 252 reports (228 in X-ray diagnostics, 24 in interventional radiology) referred to significant incidents and were included in the detailed evaluation. Reporting frequency increased in X-ray diagnostics, whereas there was no trend in interventional radiology. Most of the significant incidents concerned examinations on an individual person (category I, criterion 2a or category II, criteria 2a and 3a) in the in-patient sector - especially in maximum care hospitals. Frequent errors concerned the inappropriate choice of parameters/protocols or were related to the administration of a contrast agent. The adjunctive use of intravascular ultrasound (IVUS) significantly improves endovascular therapy. However, data on IVUS and IC is scarce. Therefore, the aim of this investigation was to determine the safety and efficacy of IVUS in patients with IC and to evaluate discrepancies compared to angiography and potential consequences for treatment.This was a single-center prospective cohort study. Twenty patients with IC and femoropopliteal disease eligible for endovascular therapy were enrolled. Procedural data and discrepancies between IVUS and angiography were recorded.In total, 30 lesions were treated. IVUS-based measurements yielded substantially higher reference vessel diameters (RVD) and lesion lengths compared to DSA alone (RVD: 5.37 ± 0.71 mm vs. 4.74 ± 0.63 mm, p<.001, lesion length: 62.4 ± 41.4 mm vs. 42.18 ± 30.2 mm, p<.001). In 24 of 30 lesions (80%), a significant discrepancy in RVD (defined as difference >0.5 mm) and lesion length (defined as >20 mm) was determined between IVUS and standard DSA. Subsequently, IVUS assessment led to upsizing in 14 of 30 lesions (47%) and downsizing in 3 of 30 lesions (10%). On average, IVUS led to the selection of considerably larger balloons (5.25 ± 0.62 vs. 4.74 ± 0.63, p<.004) and device length (78.97 ± 44.19 mm vs. 42.18 ± 30.2, p<.001). Serious adverse events did not occur. Technical success was achieved in all cases.IVUS is safe and provides advantages regarding the evaluation of IC by depicting RVD and lesion length more reliably than standard DSA.
