Phantomless estimation of bone mineral density on computed tomography: a scoping review.

Osteoporosis is characterized by reduced bone mineral density and structural weakening of bone tissue that leads to a higher risk of fracture. Dual-energy X-ray absorptiometry is the current standard for diagnosing osteoporosis. Computed tomography provides an alternative for diagnosis, but traditional QCT involves the use of phantoms, which does not allow retrospective or opportunistic assessments of BMD. This study aims to provide an overview of the evidence and feasibility for emerging phantomless techniques for the estimation of BMD on CT.A scoping review was conducted to evaluate the feasibility and effectiveness of phantomless BMD estimation using CT. A comprehensive search of Scopus and PubMed databases focused on literature published between 2010 and 2024. Search terms included combinations of \"phantomless\", \"BMD estimation\", and \"CT\". Studies emphasizing accuracy, reliability, and clinical feasibility were included. The review identified 26 relevant studies examining methods of phantomless BMD estimation. The majority of the studies used internal anatomical references to calibrate BMD measurements. These methods demonstrated accuracy comparable to traditional phantom-based techniques. Limitations of the technique included variability in scanner types and inconsistencies caused by patient-specific factors like body composition and contrast agents.Phantomless BMD estimation methods are a feasible approach to detecting osteoporosis. The possibility to be integrated into routine CT workflows make them an attractive option for opportunistic screening. Impact of Next-Generation Computed Tomography Scanners on Image Quality and Radiation Dose in Cardiac CT Outpatient Imaging.

{"title":"Impact of Next-Generation Computed Tomography Scanners on Image Quality and Radiation Dose in Cardiac CT Outpatient Imaging.","authors":"Henrik Jakob Michaely, Melanie Lueck, Miriam van Rickeln","doi":"10.1055/a-2536-3534","DOIUrl":"https://doi.org/10.1055/a-2536-3534","url":null,"abstract":"<p><p>Retrospective analysis of 245 consecutive cardiac CTA cases before and after transition from a 128-slice single-source CT scanner (SSCT, Definition AS+, temporal resolution 150ms, detector width 38.4mm) to a 128-slice dual-source CT scanner (DSCT, Pro.Pulse, 86ms, 38.4mm).A total of 113 patients (33f/80m, mean age 66.0 years) were examined using the SSCT scanner, while 132 patients (43f/89m, 64.7 years) were examined using the DSCT scanner. Protocol selection (sequential/spiral) was performed manually on the SSCT scanner. On the DSCT scanner, protocol selection was automated using built-in AI. Heart rate, kV for CTA, and DLP (mGy·cm) for CTA were recorded. Image quality was independently evaluated for the RCA, LM, LAD, and LCX by two experienced readers on a three-point ordinal scale: 3 - excellent, 2 - minor artifacts but diagnostic, 1 - artifacts rendering vessel evaluation incomplete.The mean heart rate of the patients was not significantly different between the SSCT scanner (61.1 bpm) and DSCT scanner (61.6 bpm). The mean kV for CTA was 101.4 on the SSCT scanner and 74.2 on the DSCT scanner (p<0.0001). The mean radiation dose for cardiac CTA, measured as DLP, was 134.2 on the SSCT scanner, 53% higher than the 87.7 on the DSCT scanner (p=0.001). For the SSCT scanner, 32% and 37% of RCAs were non-diagnostic according to readers 1 and 2, compared to 3% and 2% for the DSCT scanner. For the LM, LAD, and LCX, the rate of non-diagnostic cases was 2-7% with the SSCT scanner and 0-1% with the DSCT scanner.Under otherwise unchanged external conditions, the switch from SSCT to DSCT scanner for cardiac CT resulted in a significant reduction in radiation dose by approximately 34%, along with a significant decrease in non-diagnostic examinations. · Switching from an SSCT to a DSCT scanner in an outpatient setting results in a significant dose reduction of approximately 34%.. · The number of artifact-related non-diagnostic examinations is significantly reduced when using a DSCT scanner with higher temporal resolution.. · Outpatient cardiac CTA can be performed with an average radiation dose well below 2 mSv using both SSCT and DSCT systems.. · Michaely HJ, Lueck M, van Rickeln M. These advantages must be integrated into medical didactics as part of a hybrid teaching concept, and the digital potential must be used to improve preparation and follow-up opportunities, increase the effectiveness of face-to-face courses, and optimize learning success. For this purpose, the online learning platform Raditorium.com was developed. The aim of this thesis is to demonstrate the learning success of Raditorium.com as the basis of a hybrid learning concept in the field of medical didactics and to authorize it on the basis of student evaluations.As the core of hybrid learning, the online learning platform Raditorium.com was analyzed in terms of its concept and impact. The website focuses on course preparation and follow-up and combines cursory and associative learning with the motivational strategy of gamification. Raditorium.com embodies a teaching concept for the digital enhancement of face-to-face courses in order to maximize students' learning success.The results of the student evaluations show a significant improvement in overall satisfaction since the introduction of Raditorium.com as a learning platform. In the semesters before the introduction of Raditorium.com, the average ranking of radiology among clinical subjects was 24 and subsequently increased to 1.6. The average score of the evaluation issues (1-5; 1: strongly agree, 5: disagree) over a 6-semester period improved from 3.14 to 1.2 and the final grade of the entire course from 3.5 to 1.2. Students' open comments also emphasized better exams, adjustment to individual needs, and greater learning enjoyment.Thanks to their flexibility and individual availability, online platforms make it easier for students to learn radiological concepts and provide an intrinsically motivating learning environment through the possibility of innovative learning strategies that can be ideally synchronized with the teaching concept of preparation and follow-up. · Just-in-time teaching optimizes learning motivation by providing instructional materials at the moment of need.. · Structured digital content enhances effectiveness.. · Gamification increases student motivation and participation.. · The introduction of the online platform Raditorium.com improves student satisfaction..
