This study aims to evaluate the diagnostic efficacy of various radiological parameters in PI-RADS 3 lesions to improve the decision-making process for prostate biopsies.This retrospective study included 76 patients with PI-RADS 3 lesions who underwent mpMRI and transrectal prostate biopsy at a tertiary university hospital between 2015 and 2022. Radiological parameters such as signal intensity, lesion size, border definition, morphological features, lesion location, and prostate volume were analyzed. Apparent diffusion coefficient (ADC) values and the patients' clinical data including age, prostate-specific antigen (PSA), and histopathological findings were also evaluated. Results: Among the 76 patients meeting the inclusion criteria, prostate cancer was detected in 17, with only one case being clinically significant (csPCa). Factors increasing malignancy risk in PI-RADS 3 lesions included poorly defined lesion borders, ADC values below 1180 μm²/sec, and prostate volume below 50.5 cc. The study highlighted the need for additional radiological and clinical parameters in the risk classification of PI-RADS 3 cases.This retrospective study included 76 patients with PI-RADS 3 lesions who underwent mpMRI and transrectal prostate biopsy at a tertiary university hospital between 2015 and 2022. This approach could minimize unnecessary biopsies and ensure that significant malignancies are not overlooked. Future multicenter, large-scale studies are recommended to establish more definitive risk stratification criteria. · The study emphasizes the complexity of diagnosing prostate cancer in PI-RADS 3 lesions and the importance of detailed radiological assessment.. · It highlights the significance of specific radiological parameters, including lesion border definition and ADC values, in predicting","PeriodicalId":21490,"journal":{"name":"Rofo-fortschritte Auf Dem Gebiet Der Rontgenstrahlen Und Der Bildgebenden Verfahren","volume":" ","pages":""},"PeriodicalIF":1.3,"publicationDate":"2024-09-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142140945","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

To investigate age-related differences in skeletal development, the recommendations of the Study Group on Forensic Age Diagnostics (AGFAD) rely on imaging techniques using ionizing radiation (including orthopantomograms and radiographs of the hand). Vieth et al. and Ottow et al. have proposed MRI classifications for epi-/diaphyseal fusion of the knee joint to determine different age limits. The aim of the present study was to verify whether these two classifications could also be applied to MRI of the ankle.MRI images of the ankle from 333 patients (160 female, 173 male) ranging in age from 10 to 28 years were retrospectively analyzed. T1-weighted turbo spin-echo (TSE) sequences and T2-weighted fat-suppressed sequences were analyzed for the two classifications. The different ossification stages of the two classifications were determined and the corresponding chronological ages were assigned. In addition, gender-specific differences were analyzed. Intra- and inter-observer variability was determined using Cohen's kappa.With the classification of Ottow et al., the completion of the 14th year of life could be determined in both sexes. With the classification of Vieth et al, the completion of the 14th year of life could be determined in both sexes and the 18th year of life in male patients. Intra-observer and inter-observer variability was very good and good, respectively (κ > 0.87 and κ > 0.72).In the present study, it was also possible to use both classifications for MRI of the ankle joint. The method offers the potential of an alternative or at least supplementary radiation-free assessment criterion in forensic age estimation. · MRI scans of the ankle can be used for forensic age determination.. · Classifications developed for the knee joint can also be used on the ankle.. · The applied classifications based on Vieth et al. and Ottow et al. can be used as an alternative or, at the least, an additional method when determining legally relevant age limits.. · Wernsing MF, Malokaj V, Kunz SN et al. They are caused by venous insufficiency of the pelvic or ovarian veins, leading to the development of mainly periuterine and periovarian varices. It is a progressive disease if left untreated and can cause swelling, dyspareunia, dysmenorrhea, and other symptoms, some non-specific, that affect the patient's quality of life. Interventional therapies are a central component of the treatment of PeVD, with a variety of techniques available for both diagnosis and treatment.</p><p><strong>Method: </strong> This review provides an overview of the pathophysiologic background, diagnosis, and, most importantly, interventional treatment options for PeVD.</p><p><strong>Results: </strong> There is a lack of standardized nomenclature and internationally accepted diagnostic criteria for PeVD as well as randomized controlled trials demonstrating clinical success. However, in clinical trials, endovascular therapy for PeVD has been shown to be safe and effective. This review presents the various interventional techniques for the treatment of PeVD, including embolization, stenting, and sclerotherapy.</p><p><strong>Conclusion: </strong> The importance of PeVD is receiving growing recognition. Recent advances, such as the development of the Symptoms-Varices-Pathophysiology (SVP) classification, provide an impetus to standardize nomenclature and are the first step toward systematizing disease management. Interventional therapies provide a safe and tailored minimally invasive treatment option for patients with PeVD.</p><p><strong>Key points: </strong>  · Pelvic venous disorders are an underdiagnosed condition with frequently delayed diagnosis and debilitating symptoms.. · Until now, the PeVD nomenclature has been imprecise, with terms like May-Thurner syndrome/Nutcracker syndrome.. · Interventional approaches are effective and play a central role in PeVD treatment..</p><p><strong>Citation format: </strong>· Dewald CL, Becker LS, Meyer BC.
