Its annual incidence was 10 million cases in 2019. We report the case of a 40 years old immunocompetent patient presenting with two large subcutaneous masses in his back. The diagnosis work-up will reveal multifocal tuberculosis with pulmmonary, vertebral, muscular and lymph node lesions. This case is unusual due to its presentation in an immunocompetent patient. Several laboratories have conducted experiments to isolate characteristics of the host that would allow the infection to spread despite the absence of an immunosuppressive medical condition. We also analyze the role of the PET scanner in the initial assessment and its interest in the monitoring of extra-pulmonary disease under anti-tuberculosis treatment. Multifocal tuberculosis cases are no longer the preserve of the immunocompromised and can be found in our industrialized countries. We must enonciate this diagnosis in front of unusual presentations. The mechanisms of injury and the main modifiable and non-modifiable risk factors are now well documented and should allow the implementation of effective preventive strategies. In the treatment of the injured player, the physician will have to rely on a close collaboration with a quality sports physiotherapist and implement a series of key elements allowing an optimal return to the soccer field, which means at the same level of performance compared to the pre-injury period and with a minimal risk of recurrence. It relates to the dynamic interaction of synchronous or sequential diseases (whether communicable or not, also including mental diseases), with social and environmental factors, resulting at the end in a worse global outcome. A first article in the same Journal (1) was devoted to infectious diseases, especially COVID-19 and HIV infections. In this second article, we highlight the fact that the concept is also applicable on diseases which are not transmitted by infectious pathogens. DGF is frequent and mostly derived from the ischemia/reperfusion cascade to which the graft is subjected throughout the transplantation process. A graft biopsy is recommended after 7 days of DGF to exclude an episode of acute rejection. Note that DGF per se is associated with an increased risk of acute graft rejection, as well as with a shorter long-term graft survival. Several strategies are being studied to mitigate the ischaemic damage, thereby improving graft quality. Among these, cellular therapy using mesenchymal stromal cells (MSC) is promising, in particular via the administration of MSC in the machine perfusion during the preservation of the graft. We will discuss here the different definitions of DGF and the main predictive factors of DGF, as well as the impact on the graft outcomes. {"title":"[Integrating new migraine treatments into clinical practice. While traditional (and older) oral preventive treatments have helped numerous patients to date, their therapeutic efficacy is often low, and adverse event profiles are troublesome. However, the coupled progress of biochemistry and molecular biology as well as the application of advanced drug design methods have led to a therapeutic renewal with, in particular, the advent of monoclonal antibodies blocking CGRP («Calcitonin Gene- Related-Peptide») transmission. We provide an overview of the preventive pharmacologic options with both older oral treatments and new ones such as botulinum toxin and the newly marketed CGRP monoclonal antibodies. The latter seem particularly interesting because they have an effectiveness at least equivalent to most oral treatments with much better tolerance and compliance. Unfortunately, their very high cost confines them to a fourth line of therapy in Belgium, a disappointment for both specialists in migraine therapy and patients who suffer from frequent migraine crisis. Our results highlighted great interprofessional similarities in terms of negative and positive experiences (e.g., feeling of powerlessness, support among colleagues), as well as professional specificities (e.g., the painful feeling of being «alone at the front» among nurses). Although the stress, fatigue and/or lassitude factors cited by the participants were more numerous than the protective factors, several participants experienced an opportunity for personal and professional development. {"title":"[Dapagliflozin in DELIVER : confirmation of the benefit with SGLT2 inhibitors in heart failure with preserved ejection fraction].","authors":"André Scheen, Patrizio Lancellotti","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Gliflozins (sodium-glucose cotransporter type 2 inhibitors or SGLT2is) first demonstrated a reduction in hospitalisation for heart failure (hHF) in patients with type 2 diabetes (T2DM) at high cardiovascular risk. Then, a reduction in hHF (also combined with cardiovascular mortality) was reported in patients with heart failure, independently of the presence of T2DM. These placebo-controlled trials first concerned patients with heart failure and reduced left ventricular ejection fraction (LVEF) in DAPA-HF with dapagliflozin and EMPEROR-Reduced with empagliflozin. Afterwards, the benefit was observed in patients with preserved LVEF in EMPEROR-Preserved with empagliflozin, yet some doubt persisted in patients with LVEF > 60 %. The DELIVER study recently confirmed a significant reduction in the composite outcome hHF plus cardiovascular mortality (- 27 %) and in hHF (- 21 %) in patients (with or without T2DM) with heart failure and mildly reduced or preserved LVEF treated with dapagliflozin compared with placebo. The protection was noticed whatever the level of LVEF. These results reinforce the place of SGLT2is in international guidelines for the prevention or treatment of heart failure independently of the level of LVEF.
