{"title":"[Dapagliflozin in DELIVER : confirmation of the benefit with SGLT2 inhibitors in heart failure with preserved ejection fraction].","authors":"André Scheen, Patrizio Lancellotti","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Gliflozins (sodium-glucose cotransporter type 2 inhibitors or SGLT2is) first demonstrated a reduction in hospitalisation for heart failure (hHF) in patients with type 2 diabetes (T2DM) at high cardiovascular risk. Then, a reduction in hHF (also combined with cardiovascular mortality) was reported in patients with heart failure, independently of the presence of T2DM. These placebo-controlled trials first concerned patients with heart failure and reduced left ventricular ejection fraction (LVEF) in DAPA-HF with dapagliflozin and EMPEROR-Reduced with empagliflozin. Afterwards, the benefit was observed in patients with preserved LVEF in EMPEROR-Preserved with empagliflozin, yet some doubt persisted in patients with LVEF > 60 %. The DELIVER study recently confirmed a significant reduction in the composite outcome hHF plus cardiovascular mortality (- 27 %) and in hHF (- 21 %) in patients (with or without T2DM) with heart failure and mildly reduced or preserved LVEF treated with dapagliflozin compared with placebo. The protection was noticed whatever the level of LVEF. These results reinforce the place of SGLT2is in international guidelines for the prevention or treatment of heart failure independently of the level of LVEF. Abdominal compartment syndrome (ACS) can be one of the serious postoperative complications of rEVAR. ACS is usually associated with progressive development of organ dysfunctions and poor outcomes. {"title":"[A new era for glucagon-like peptide-1 receptor agonists].","authors":"M F Neuville, N Paquot, A J Scheen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Glucagon-like peptide-1 (GLP-1) receptor agonists have a privileged place in the management of type 2 diabetes (T2D). They not only improve glucose control without inducing hypoglycaemia and trigger weight loss, but also protect against atherosclerotic cardiovascular disease. Increasing the dose of three of them (liraglutide, semaglutide, dulaglutide) allows better glycaemic results and of potential interest a greater weight reduction. Liraglutide at a daily dose of 3.0 mg and semaglutide at a weekly dose of 2.4 mg received the indication for the therapy of obesity. A recent innovation consists in the development of dual unimolecular agonists that target GLP-1 and GIP («glucose-dependent insulinotropic polypeptide») receptors (tirzepatide) or GLP-1 and glucagon receptors (cotadutide). Tirzepatide, in the SURPASS programme, showed impressive reductions in glycated haemoglobin level and body weight, greater than those observed with dulaglutide or semaglutide. Tirzepatide received the indication of the treatment of T2D and is currently tested in obesity (SURMOUNT programme). It is a reliable operation with 95 % good results at 10 years after surgery. Aseptic loosening is the primary cause of TKR failure. It results from a periprosthetic osteolytic reaction secondary to the host's reaction to the wear particle of the prosthesis and responsible for an imbalance in osteoformation and osteolysis. Polyethylene (PE) is responsible for the majority of periprosthetic reactions but the release of metal particles may play an underestimated role with the risk of systemic manifestations. The first clinical signs appear on average 7 years after prosthesis placement. Radio-clinical assessment may be normal in the early stages. Left without any real new therapeutic option to offer patients since sumatriptan was introduced on the Belgian market 30 years ago, neurologists have recently seen a change in the therapeutic landscape with the advent of new specific acute treatments for migraine: gepants and ditans. Being the only ones currently available in Belgium, gepants (including the newly marketed rimegepant) bring added value to traditional treatments such as non-steroidal anti-inflammatory drugs and triptans. It relates to the dynamic interaction of synchronous or sequential diseases (whether communicable or not, also including mental diseases), with social and environmental factors, resulting at the end in a worse global outcome. The rise of publications in peer reviewed journals is exponential, especially in the last ten years. Born after another - forgotten - epidemy, the one related to HIV (AIDS), this concept is more than ever cited within the frame of the still ongoing COVID-19 pandemics. We will highlight this concept through a couple of selected examples related to infectious diseases.
