Only interventional studies aiming to assess the effectiveness of HUBER® device (any versions, any protocol of intervention used) on physical rehabilitation outcomes of adults were included. Quality of studies was assessed using Cochrane RoB tools.</p><p><strong>Results: </strong>Out of 142 references, six interventional studies were included in this systematic review. The number of participants included per study varied from 12 to 53, length of intervention varied from 4 to 8 weeks and number of sessions per week varied from 2 to 4. Different profiles of participants were included. Globally, training sessions using the HUBER® device showed moderate beneficial effects on the following outcomes: morphological characteristics, strength, balance and muscle power. Even if the metabolic risk appears lower with antidepressants than with certain atypical antipsychotics, it should not be neglected in clinical practice, especially with antidepressants that are associated with weight gain. Metabolic disorders may occur with both selective serotonin reuptake inhibitors and tricyclics, but with some between-molecule differences within each pharmacological family. Besides potential deleterious effects of medications, the psychiatric population is also exposed to bad lifestyle habits (unhealthy diet and sedentary life) and poor medication compliance, which all together could also contribute to enhance the risk of metabolic disorders and cardiovascular complications. {"title":"[Posterior iliac crest low back pain].","authors":"Arnaud Janssen,&nbsp;Marco Tomasella","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In the context of low back pain of the posterior iliac crest, various etiologies can be evoked, in particular Maigne's syndrome in D12/L1, ilio-lumbar ligament syndrome or L5/S1 facet syndrome. Maigne's syndrome is a painful segmental vertebral dysfunction leading to a cellulo-periosto-myalgic syndrome. Propedeutic is essential to the diagnosis and is based on a rigorous segmental spinal examination and on the remote search for palpable changes in the texture of the skin, muscle and teno-periosteal tissues. The iliolumbar ligament is a powerful fibrous stabilising element of the lumbosacral hinge. Its tensioning by repeated movements of anteflexion and contralateral lateroflexion-rotation contributes to the expression of a painful symptomatology of the posteromedial iliac crest. Pain in the posterior inter-apophyseal joint L5-S1 is characterised by a more medial lumbosacral pain, accentuated on facet palpation and reproduced on combined heterolateral extension-rotation of the spine. Manual therapy centred on the dorsolumbar or lumbosacral hinge is recommended as a first-line treatment to control these different diagnoses. Rehabilitation of the motor control of the deep stabilising muscles of the dorsolumbar spine is essential. {"title":"[SGLT2 inhibitors : on the borders of diabetology, cardiology, nephrology and… primary care].","authors":"André Scheen,&nbsp;Patrizio Lancellotti,&nbsp;Pierre Delanaye,&nbsp;Didier Giet","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Sodium-glucose cotransporter type 2 inhibitors (SGLT2is or gliflozins) are now considered as a therapeutic breakthrough in clinical practice, not only for the management of type 2 diabetes (T2D), but also for the treatment of heart failure and chronic renal disease. Patients with T2D are exposed to a higher risk of atheromatic lesions, heart failure and renal insufficiency, all complications that can be reduced by a gliflozin as shown in several placebo- controlled randomised trials in at high risk patients. Unexpectedly, such cardio-renal protection has also been observed among non-diabetic patients with heart failure (both with reduced and preserved ejection fraction) or with chronic kidney disease (especially with albuminuria). Because of these properties, SGLT2is now occupy a privileged place in diabetology, cardiology and nephrology. The symptomatology is often poor, but it classically includes muscle weakness, myalgia and red-brown urine. The causes may be multiple but are most frequently traumatic : the so-called \"crush syndrome\". The diagnosis is based on the increase in serum creatine kinase, which is sometimes associated with myoglobinuria. This clinical disorder is often mistaken for infectious pneumonitis and treated as such, with as consequence an increased mortality of these patients. The diagnosis of acute mitral insufficiency is therefore essential for the optimal management of these patients. The etiologies of mitral insufficiency can be of primary or secondary origin. Cardiac ultrasound (trans-thoracic and trans-esophageal) represent the key to the diagnosis of acute mitral insufficiency and to ensure an adapted management. It consists initially in stabilizing the patient's condition (which may be critical) via pharmacological treatments and/or mechanical support. It allows to reduce postoperative morbidity and the duration of the hospital stay. {"title":"[Long COVID, diagnosis and management : a survey among general practitioners in the Bas Rhin in France].","authors":"Nicolas Ambiehl,&nbsp;Clement Ambiehl,&nbsp;Léa Docquier,&nbsp;Xavier Jannot,&nbsp;Noel Lorenzo-Villalba","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The long COVID includes non-specific symptoms affecting patients of all ages.</p><p><strong>Methods: </strong>Monocentric, prospective, quantitative and descriptive study interested in comparing the knowledge of general practitioners in Alsace (France) with the positioning and recommendations of the World Health Organisation (WHO) on long COVID in the period November 2022-February 2023. A multiple-choice questionnaire was elaborated based on the WHO positioning and recommendations on the long COVID.</p><p><strong>Results: </strong>126 questionnaires were received, 65 (51.6 %) women, 42 % aged between 25-35 years. 84.9 % had knowledge about long COVID. The need for an impact on daily life, unexplained symptomatology and symptoms persisting at two months were criteria retained (84.9 %, 91.3 % and 82.5 %, respectively). A history of anxiety disorders (86.5 %) and severe acute forms of COVID-19 (57.9 %) were considered as risk factors. Fatigue, dyspnea, joint pain, headache and cough were associated with long COVID in 65.9 % of responders. About 74,6 % performed a complete blood test, 96.03 % recommended rehabilitation, 76.2 % referred to the pulmonologist and 71.4 % were in favour of a consensus document.</p><p><strong>Conclusions: </strong>General practitioners are aware of the existence of long COVID as a pathology and agree with the recommendations of scientific societies, among which those of WHO. In addition, the age at which consumption begins as well as the consumed quantity are determining factors.
