This concept has always been a topic of debate and controversy regarding its definition, nature and nosography, which is still ongoing today. Its complex management, including psychotherapeutic and pharmacological interventions, challenges the therapeutic relationship. It results in hyperpulsatility, which is characterized by an ample pulse, an accentuation of heart sounds on auscultation, and an exaggeration of heart movements on echocardiography. While it can be very troublesome, cardiac erethism has no pathological significance. However, care must be taken to exclude any underlying cardiac or extracardiac pathology before confirming the diagnosis. No treatment is usually considered except to reassure the patient and avoid contributing factors. They still represent one of the most difficult diagnostic challenge for the neurologist, who can only rely on clinical criteria. Functional gait and movement disorders represent an important subgroup of these conditions and a frequent reason for consultation. The neurobiological basis of these manifestations remains poorly understood despite the progress of functional neuroimaging. Actually, this pathology associates diffuse chronic pain with a host of somatic complaints, which may be variable from one patient to another. The difficulty resides also in establishing the diagnosis, which remains essentially clinical, in the absence of anatomical lesions, as well as biological or anatomopathological anomalies. Patients are usually confronted to a therapeutic wandering phase, of variable duration, before being recognized in their pathology. Different underlying mechanisms can lead to long COVID, none of which are mutually exclusive. Lingering symptoms can persist years after SARS-CoV-2 infection, including fatigue, muscle weakness, tachycardia, dyspnea and various neurological symptoms. The symptomatology is partly similar to that reported by people with chronic fatigue syndrome and other unwell studied long-lasting diseases that may occur after other infections. People who have experienced more severe COVID-19 illness are at higher risk of developing long COVID, although anyone who was infected can experience post-COVID conditions. Importantly, unvaccinated individuals are more likely to develop long COVID. Here we review the current knowledge and discuss key findings regarding the epidemiology and physiopathology of long COVID. Sometimes, complementary examinations are necessary to rule out an organic lesion that could explain the symptomatology. The notion of a diagnosis of exclusion is therefore very present. The physician must constantly re-evaluate the diagnosis of functional disorder in order not to «miss» a diagnosis with an organic cause.The treatment of these functional disorders is sometimes based on psychological treatment when a psychogenic dimension seems to be involved. This is not always the case. In such cases it is necessary to be able to consider a placebo approach with the hope that the placebo effect may improve the patient's condition. That causal relationship is yet far from being always established. If a disease is identified generating arterial hypotension, the symptoms reported such as loss of energy, fatigue and/or depressive mood can of course be the consequence. However, asymptomatic chronic hypotension exists. Symptoms appearance in a hypotensive patient such as fatigue, loss of vital energy, alteration of quality of life must lead to look for another explanation than (chronic) low blood pressure.
