{"title":"[Teichoplanin: a clinical study of treatment of serious infections in hospitalized patients].","authors":"R Di Rosa, S Maira, F Guido, M F Cossu, A Amoroso","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The study was performed to evaluate the efficacy and the safety of teicoplanin in the treatment of serious Gram positive bacteria infections in hospitalized patients at high risk of complications. In a twelve month period, 13 patients were treated. Six of whom presented generalized infections with bacteremia and seven severe localized infections. Safety evaluation included pre, during and post treatment measurements of haematological and serum chemistry parameters. Clinical successes with microbiological eradication of pathogens were obtained in 11 out of 13 cases (84.6%). One persistence of infection (7.7%) and one Gram negative superinfection (7.7%) presented. Only in one patient we noted a transitory increase of creatinine level. This study was carried out on a group of 40 puerperae who lived in Southern Italy, delivered at term and feeded with common mediterranean diet. End-milk samples were collected between the 15th and 40th day (in mean at 27th day) after delivery. Analysis of fatty acids was performed using high resolution gaschromatography and mass spectrometry. One of the major problems related to PTCA is myocardial ischemia following balloon catheter occlusion of the coronary artery. A new device utilized in an attempt to counteract myocardial ischemia is the perfusion balloon catheter which allows passive transmission of pulsatile blood flow to the distal myocardial bed. Our purpose was to compare immediate results following traditional PTCA and autoperfusion balloon PTCA in two groups randomly assigned to receive one of the two treatments (total patients n. = 158). The results show that coronary angioplasty was successfully performed in 98.87% of patients treated with autoperfusion PTCA and in 97.10% of those treated with conventional PTCA. In the autoperfusion PTCA group we observed a lower incidence (statistically significant) of ST elevation and of minor complications. In view of this pulmonary perfusion by radionuclide method, arterial blood pressure by the femoral artery, mean right ventricular pressure and pulmonary wedge pressure by Swang-Ganz catheter were detected in 7 medium sized healthy dogs. In addition PaO2, SaO2, PaCO2 and blood pH were also determined by current methods. All the dogs received an acute bolus of purified heroin at dosage of 12.5 mg/Kg. All the measurements were made at baseline and at 30 seconds, 10 minutes, 20 minutes and 30 minutes after heroin administration. Our data indicated an impaired pulmonary perfusion by the analysis of the slope and the area under the time/activity curve following heroin overdose. In addition a significant reduction (p < 0.001) in blood pressure and a significant rise (p < 0.01) in mean right ventricular pressure and in pulmonary wedge pressure were observed after 30 seconds by heroin overdose. Moreover heroin administration promoted a significant increase (p < 0.001) in PaCO2 and a significant decrease (p < 0.001) in both PaO2 and SaO2. These results indicated that early pulmonary perfusion abnormalities may be recognized in dogs receiving an acute heroin overdose and they were associated with impaired gas-exchange. {"title":"[The treatment of decubitus lesions].","authors":"G Fugazza, S Moroni, F Bona","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors present a plan for pharmacological treatment of pressure sores in patients affected by neurological pathologies: cerebrovascular accidents, head injuries, spinal cord injuries. This plan is easily applicable to all pressure sores included between first and third degree of the Reuler and Cooney classification. Authors identified some drugs specifically usefull in different cutaneous lesion degrees. Skin lesions and employed medicines are described as follows: Erythema: semi occlusive bandage with porous adsorbing membrane. This dressing must be left in for five days at least. Excoriation: bactericidal or bacteriostatic medicines if it's situated in a non pressed area while the same dressing utilized for erythema if it's localized in a pressed area. Pressure sores: if there is local infection cleanse the wound from bacterial defilement using topic antibiotics apply compresses with vitamin C if the cutaneous lesion is larger than deeper, Cadexomero lodico if it's deeper than larger. Fistulas: wadding with tablets of collagen. Necrobiosis: complete or partial surgical removal of eschar preceded by the use of enzymatic drugs when eschar is firmly adherent to subcutaneous tissues. The first group collects 9 patients with stroke and head injury: 8 with sacral and 1 with heel pressure sores. First degree pressure sores heal within 45 days and third degree lesions within 160 days. The second group collects 10 spinal cord injury patients mostly with complete lesion among which: 7 sacral, 1 heel, 1 ischiatic and 1 malleolar lesions. First degree pressure sores heal within 30 days, third degree pressure sores heal within 200 days. Healing time are considered acceptable. {"title":"[The hypotriglyceridemic effects of polyunsaturated fatty acids: pathogenetic hypothesis].","authors":"A Del Ponte, G Bitti, G Di Giacomo, S Marinari","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A large number of experimental and clinical studies focused the attention on the role played by omega-3 polyunsaturated fatty acids on the primary prevention of vascular diseases of atherosclerotic origin, especially influencing plasma triglycerides decrease. Aim of the present work was to investigate whether a relationship between triglyceride synthesis and human energy metabolism could exist, in order to explain the hypotriglyceridemic effect of omega-3 fatty acids that could be due to any thermogenic action. Eight male subjects aged 45 years (from 20 to 62) were studied, selected on the basis of high levels of triglycerides (536.7 +/- 204.3 mg/dl), normal or high levels of total serum cholesterol (219.4 +/- 51.2 mg/dl), normal or low HDL-cholesterol (33.6 +/- 6.3 mg/dl) and moderate obesity (BMI 30.3 +/- 1.7). Anthropometric parameters (body weight, height, waist to hip ratio), body composition measurements (fat free mass, fat mass), biochemical parameters were evaluated before and 10-days after PUFAs supplementation (student t paired test) at the compressive dose of 60 mg/kg BW bis in die. At the end of the observation significant decrease of triglycerides was documented (p < 0.005), while total and HDL-cholesterol did not vary significantly, although improved values were noticed. No change in anthropometric, body composition and energy metabolism parameters was demonstrated, except for respiratory quotient and substrates oxidation: a significant increment of respiratory quotient value (p < 0.05) due both to augmented carbohydrates oxidation (p < 0.05) and correspondent lipid oxidation reduction (p < 0.05) and correspondent lipid oxidation reduction (p < 0.05) without any change in the daily total calorie consumption. In conclusion, from a clinical point of view it is possible to hypothesize, even if in a small number of subjects, that PUFAs hypotriglyceridemic effect could derive from a reduced substrate availability, due to the higher carbohydrate oxidation. The aim of our study was to identify those patients who, on the basis of the presence of cardiovascular risk factors such as diabetes mellitus, hypertension and hyperlipidemia, show more evident signs of such dysfunction. The study was conducted on 333 male patients, aged > 49 years, with normal renal function, who underwent coronary artery by-pass surgery in extracorporeal circulation.
