{"title":"[Acute ischemia of the lower limbs].","authors":"G Illuminati, A Bertagni, F G Calio, A Ciulli, R Guglielmi, F Vietri, V Martinelli","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>One hundred nineteen patients operated upon for acute ischemia of the lower limbs have been retrospectively reviewed, in order to evaluate the influence of the condition on outcome, and the patterns of treatment. Fifty six per cent of the patients were males and 42% females, 68.4% had an history of aorto-iliac obstructive disease; 51.2% of the ischemias were due to arterial thrombosis and 48.8% to embolism, in 12.5% of the cases etiology was unknown. Thrombosis were more frequent in younger patients' population (mean age 59.8 years) compared to embolism (mean age 69 years), even if the mean age of patients bearing an ischemia of embolic type has risen, in the last 10 years: 62.1% were due to atrial fibrillation and 50% occurred on pathologic arteries, patients with arterial obstructive disease presented a higher incidence of arrest of embolic material at the common femoral artery bifurcation. Women's mean age was significantly higher then that of men (70.9 vs. 59 yrs.). Embolism was more frequent in women than in men (64 vs 42.2), whereas thrombosis was more frequent in men than women (57.8 vs 36%). There were more deaths in women than men (30.8 vs 20.8%), but more amputations in men than women (37.3 vs 17.3%). Embolism of arterial origin include 15.2% of all acute ischemias and 31% of all embolisms, and their site of origin is often unknown, particularly when it is located in the thoracic aorta, which mandate a complete arteriography and eventually a CT-Scan of the thorax and the abdomen. A lower limb phlebitis, associated with an acute ischemia mandates the search of a patent foramen ovale, and an heparin induced thrombocytopenia. Surgical treatment is directed towards obstruction removal (embolectomy, by-pass) and towards the treatment of the causative agent (aortic endarterectomy, caval filter, anticoagulants). Post-operative thrombolysis may be beneficial in case of distal residual thrombus after embolectomy. An higher mortality rate has been observed in acute ischemias due to embolism rather than thrombosis (31 vs 19.6%) and more major amputations in thrombosis than in embolism (37.7 vs 17.2%). Overall mortality rate has been of 25%, with an amputation rate of 28%. {"title":"Type 2 autoimmune hepatitis and hepatitis C viraemia.","authors":"O Leri, S Paparo Barbaro, M T Sinopoli, V Marcelli, F G De Rosa, D De Luca","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We present 10 Italian patients with type 2b autoimmune hepatitis (anti-LKMI positivity) and HCV infection. 6 patients had IgG concentrations above the upper limit of normal and all had histological features of chronic autoimmune hepatitis or chronic persistent hepatitis or cirrhosis. ANA and SMA were positive in 2 patients, pANCA in 3 patients. Anti-GOR were negative in all patients, 6 of them were HLA B8 DR3 and 2 HLA B8 DR4. Antibodies to HCV (tested by ELISA 2nd and 3rd generation) were positive in all patients and in 9 subjects were detected HCV RNA. The two patients with positivity for ANA and SMA were treated successfully with corticosteroids, but they relapsed after the drug withdrawal; the others received interferon, that had to be suspended in 2 patients because inducing an autoimmune thyroiditis. After the administration of 10,000 U of rHuEPO three times a week for at least five times instead of the 1000 U ordered, the patient showed a progressive increase of the plasmatic creatinine values from 2.2 to 3.3 mg/dl. In the same period we pointed out an increase of hematocrit (from 29% to 35%) and of the Hb (from 9.3 to 11.2 g/dl). The patient, suffering from a slightly high blood pressure, furthermore showed important pressure increases. The suspension of the treatment by erythropoietin permitted the restoration of the creatinine values to levels similar to the ones pointed out before the treatment and a reduction of the blood pressure values. {"title":"Pectus excavatum in adults: destructive surgery or simple correction of an aesthetic defect?","authors":"A Paolini, M Ruggieri, F L Leone Sossi, G Paolini, G Dal Prà, N Scuderi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Pectus excavatum, a congenital depressive malformation of the sternum, is described since 1594 by Johannes Schenk and has successively been the subject of many studies because of its various inherent problems. The repercussions on the respiratory and cardiac dynamics caused by the compression on the mediastinal structures and by the reduction of the respiratory volume have only recently been valuated completely with the application of the latest techniques to diagnosis, cardiology and radiology. The aesthetic defect, often the first aspect to attract the attention of patient and doctor, has serious psychological implications, especially for what concerns the affective life and relationships in general. The surgical therapy consists in radical interventions with large sternocostal resections and repositioning of the sternum (turnover), or in interventions with a less invasive technique to correct only the aesthetic defect. In this study have been examined 7 cases of patients affected by pectus excavatum who were operated at the Institute of General Surgery IV and the Department of Plastic surgery of the University of Rome \"La Sapienza\" between 1983 and 1993. The patients were divided in two groups: a first group of 4 patients who underwent a sternal turnover, and a second group of 3 patients who were operated with conservative methods to correct the aesthetic defect. In both groups the results were good without any post-operatory complications. The experience gained during these years consents us to propose radical interventions for patients who have a severe malformation with evident physiopathological implications; this intervention is especially indicated if the patient is young, to prevent further disabling complications. In this study the Authors measured in 25 subjects (15 with essential hypertension and 10 normal) plasma levels of endothelin-1. Patients with hypertension had significantly higher plasma ET-1 concentration than normal subjects (2.18 +/- 1.07 vs. 1.36 +/- 0.40 pg/ml, p < 0.02). Plasma arginine vasopressin, renin activity and aldosterone concentration did not show significant differences between hypertensive and normotensive subjects. The determination of anti-HAV antibodies was carried out by ELISA-test. Of the 287 examined sera, 27 cases turned out HAV positive antibodies (9.7%), with the following distribution, according to the groups of age: 0-3 months: 2 of 6 children were positive (mother's antibodies); 3 months-2 years: 6 of 112 (5.35%); 2-6 years: 10 of 93 (10.75%); 6-12 years: 11 of 67 (16.41%). With regard to distribution of anti-HAV antibodies by sex, 23 (15.03%) males of 153 resulted positive, whereas 4 (3.2%) females of 125 resulted positive. The decline of HAV infection in the paediatric age involves a possible shift of the risk to the adult age. For the evaluation of this risk an important role is played by bone mass peak (measurable with mineralometric techniques) and the rate of bone resorption. The latter can be assessed by means of many laboratory indexes, taken as markers of bone cells activity. The latest markers of osteoblastic activity are osteocalcin and the terminal fragment of type I pro-collagen. On the other hand pyridinolines are considered markers of bone resorption. {"title":"Functional results of sub-total laryngectomy.","authors":"M De Vincentiis, A Di Nardo, P Calcagno, R Turchetta, A Minni, G Ruoppolo","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Sub-total laryngectomy (crico-hyoido-pexy and crico-hyoido-epiglotto-pexi), while achieving a radical oncological control results in a functional neolarynx efficacious in swallowing as well as in phonatory activities. 102 out of the 122 patients, treated with sub-total laryngectomy, underwent functional evaluations: the results appear to be extremely variable within each group. A better performance in crico-hyoido-epiglotto-pexi (CHEP) rather than in crico-hyoido-pexi (CHP) has been observed in these patients.
