{"title":"Stress and fatigue in nurses in critical care units and their association with salivary cortisol.","authors":"Dnieber Chagas de Assis, Deisy Vivian de Resende, Robinson Sabino-Silva, Maria Helena Palucci Marziale","doi":"10.1590/0034-7167-2024-0282","DOIUrl":"10.1590/0034-7167-2024-0282","url":null,"abstract":"<p><strong>Objectives: </strong>to evaluate the association between stress, fatigue and salivary cortisol levels in nurses working in emergency units (EU) and surgical units (SU).</p><p><strong>Methods: </strong>cross-sectional study conducted in two university hospitals with participation of 66 nurses. The Nursing Stress Inventory and the Fatigue Assessment Scale were applied, and saliva samples were obtained to determine cortisol concentration.</p><p><strong>Results: </strong>high levels of stress and fatigue were observed in 50.8% and 46% of the professionals, respectively. Nurses in EU had higher levels of stress and fatigue and lower secretion of salivary cortisol during the work shift compared to nurses in the SU, without statistical significance. A socio-occupational questionnaire and the Professional Quality of Life Scale 5 were used.</p><p><strong>Results: </strong>higher averages of satisfaction with compassion were evidenced, followed by burnout syndrome and Secondary Traumatic Stress. Age and number of children were related to satisfaction with compassion. These variables and years of experience in the health field were inversely correlated with burnout scores. In April 2023, we searched PubMed, EBSCO, BVS, Web of Science, and Scopus databases, selecting nine studies. Inclusion criteria comprised year and language of publication; we excluded foreign publications and those unrelated to feminist bioethics and nursing.</p><p><strong>Results: </strong>we identified a gap in research on this topic from 2006 to 2020; most studies discussed the theme of valuing nursing and concluded that political engagement in health is a viable strategy.</p><p><strong>Final considerations: </strong>feminist bioethics encourages nurses to discuss gender roles in the workplace and to address vulnerabilities in care delivery. The bundle for validation, composed of six items presented in Likert format, was evaluated online by the experts. The Content Validity Index with values above 0.80 were considered for analysis.</p><p><strong>Results: </strong>The final version of the bundle consisted of four items. The proposed care is related to measuring the length for insertion of the gastric catheter and methods for verifying its adequate positioning in newborns. We assessed the ideal cardiovascular health using three behaviors (smoking abstinence, body mass index, and physical activity) and three factors (cholesterol, blood pressure, and absence of diabetes and cardiovascular disease). The outcome measured was preeclampsia.</p><p><strong>Results: </strong>preeclampsia developed in 24.75% of participants, while 9.90% had ideal cardiovascular health. We identified an association between body mass index (p < 0.01) and blood pressure (p < 0.01) with preeclampsia. Data were collected from May to December 2021 and analyzed based on articles on vocational education and educational policies at national and state levels.
