This study examines the contribution of personality, cognitive, and autistic traits to manifestations of SA in adults. To assess how personality, cognitive, and autistic traits affect SA and to define the impact of autistic features on its development.</p><p><strong>Methods: </strong>The study included 72 adults (42 women, 30 men) over 18, with different levels of SA and autistic traits. Participants completed several questionnaires: Social Anxiety and Social Phobia Questionnaire, HEXACO-60-PI-R60 Personality Questionnaire, Hostility Scale, Cognitive Styles Questionnaire, Autism Spectrum Quotient, Behavior Self-Regulation Style Questionnaire, and an Emotional Intelligence Test.</p><p><strong>Results: </strong>Two groups were studied: (A) high SA with low autistic traits, and (B) high SA with high autistic traits. T-tests and regression analyses were used. No significant differences in overall SA were found between the groups; however, individuals with pronounced autistic traits exhibited greater anxiety in situations requiring initiative and being under observation. Group B demonstrated higher scores on all autism spectrum scales and distinctive personality profiles (low extraversion, high honesty/humility, vigilance, and negativism. Regression analysis revealed distinct predictive factors for SA in the two groups. For Group A this included the following parameters: Liveliness, lower Sociability, decreased Modeling of Conditions, heightened Sentimentality and increased Attention to Detail (89% of the variance in SA scores). For Group B they included low Social Self-Esteem, reduced Flexibility, diminished Reliability, elevated Dependence, and stronger Field Independence (92% of the variance in SA scores).</p><p><strong>Conclusion: </strong>Personality and cognitive predictors of SA differ depending on the presence of autistic traits. The data confirm the necessity of a differentiated diagnostic and therapeutic approach. This study aims to characterize the clinical and psychosocial features of adolescents and young adults referred for psychiatric consultation after a suicide attempt.</p><p><strong>Subjects and methods: </strong>We conducted a retrospective observational study at the University Hospital of Perugia, Italy, analyzing 72 patients aged 14-35 who received their first psychiatric evaluation during medical hospitalization. Patients were divided into two groups: those referred after a suicide attempt (SA group, n=36) and those referred for other psychiatric concerns (non-SA group, n=36). Data were extracted from structured consultation reports and included sociodemographic, clinical, and psychopathological variables. Bivariate analyses compared the two groups using appropriate statistical tests.</p><p><strong>Results: </strong>Compared to the non-SA group, the SA group had significantly higher rates of unemployment, positive psychiatric family history, previous suicide attempts, insomnia prior to admission, anxiety symptoms with both psychic and somatic features, personality disorders, and mood stabilizer use. SA patients also showed lower cooperativeness during interviews and were more likely to be assessed with suicidal ideation. More than one third of SA patients were assessed as euthymic post-attempt.</p><p><strong>Conclusions: </strong>Key clinical markers of suicide risk in youths may include unemployment, family psychiatric history, insomnia, anxiety with somatic and psychic features, and personality disorders. The clinical profile of suicide attempters suggests a possible contribution of bipolar spectrum diathesis and affective dysregulation. Psychological consequences such as eco-anxiety, depression, and post-traumatic stress disorder (PTSD) following climate-related disasters are being reported with growing frequency.</p><p><strong>Subjects and methods: </strong>This study was conducted with mental health professionals who attended the Medforum Psychiatry Congress in 2024. A 22-question, closed-ended questionnaire was used to assess perceptions of the impact of climate change on mental health, the prevalence of eco-anxiety, and workplace preparedness for responding to climate-related disasters.</p><p><strong>Results: </strong>A total of 208 individuals participated in the survey. Of those, 51.9% rated the impact of climate change on mental health as moderate, and 13.9% considered it to be very significant. Although eco-anxiety is increasingly recognized by professionals, most admitted that they rarely encounter patients who report this issue directly.</p><p><strong>Conclusions: </strong>Climate change affects mental health in two ways: directly, through the experience of disasters, and indirectly, by inducing chronic anxiety. Standard pharmacological treatments are often limited by efficacy and tolerability issues in this population. Neuromodulation has emerged as an essential therapeutic modality, offering targeted treatment that can circumvent systemic side effects. This narrative review provides a critical synthesis of the evidence for established neuromodulation techniques - Electroconvulsive Therapy (ECT), Repetitive Transcranial Magnetic Stimulation (rTMS), Transcranial Direct Current Stimulation (tDCS), and Vagus Nerve Stimulation (VNS) - and explores emerging approaches like Transcranial Focused Ultrasound (tFUS) within psychogeriatric populations. The application of these techniques is governed by a trade-off between efficacy, safety, and logistical burden. ECT remains the gold-standard for severe presentations, such as psychosis or catatonia, despite its cognitive risks. rTMS provides a powerful balance of efficacy and tolerability for non-psychotic TRD. tDCS and non-invasive VNS offer excellent safety profiles and potential for home-based administration, expanding access and showing promise for cognitive indications. While the clinical roles of these modalities are clarifying, the path forward requires addressing critical gaps. Future research must prioritize methodologically rigorous, geriatric-specific trials, the development of biomarkers to guide personalized treatment, and an unwavering focus on the ethical complexities of assessing capacity and obtaining informed consent. In recent decades, clinical psychology has played a pivotal role in understanding and managing the psychological aspects of chronic illnesses. This paper aims to synthesize current research on the bidirectional relationship between psychological factors and chronic disease progression, emphasizing recent advances in theoretical models, empirical findings, and clinical interventions. Defensive functioning, alexithymia, and psychological flexibility emerge as crucial dimensions. A greater use of mature defense mechanisms appears to be linked to better psychosomatic well-being, while reliance on immature defenses is associated with increased anxiety-depressive symptoms and a more negative perception of health. Similarly, higher levels of alexithymia are connected to greater psychological distress and reduced interoceptive awareness. In contrast, psychological flexibility shows a protective role, promoting better mental health and adaptive functioning. These findings highlight the need to deepen our understanding of the psychological underpinnings of chronic illness, especially through approaches that account for individual differences in emotional regulation and coping. While existing research has offered promising directions, the field would benefit from longitudinal studies and the systematic inclusion of psychological variables in chronic care pathways. This study, conducted in a closed psychosis unit, aimed to explore the feasibility of a 1-hour weekly walk activity, alongside assessing self-esteem.</p><p><strong>Subjects and methods: </strong>From February 7 to July 11, 2025, patients at the Gaudi unit in Mons, Belgium, specializing in psychotic disorder care, were offered participation in a weekly supervised walk. Out of 64 evaluated inpatients, eligible individuals could choose between a 3 km or 5 km circuit. The study also planned to assess self-esteem using the Rosenberg Self-Esteem Scale (RSES) and to conduct qualitative analysis upon admission.</p><p><strong>Results: </strong>Over 24 weeks, the average walk participation rate was 59.86% (255 out of 426 possible participations). Most preferred the 3 km circuit (75.69%), while fewer chose the 5 km (14.9%) or the 8 km combination (9.41%). However, comprehensive assessments (RSES, qualitative evaluation) were only offered to 12 of the 64 patients. Only 6 completed the qualitative evaluation, and 10 completed the RSES. This low assessment completion rate suggests the evaluations might be too challenging for this population.</p><p><strong>Conclusion: </strong>Implementing a weekly walk activity in a closed psychosis unit is feasible. Nevertheless, integrating a study protocol for comprehensive physical and psychological assessments proved challenging. Self-efficacy has been identified as a key factor in the prognosis of depression, as it influences how affected individuals cope and manage depressive symptoms.</p><p><strong>Subjects and methods: </strong>This study aims to gain a deeper understanding of the factors that contribute to the development of personal self-efficacy by using a framework inspired by the Research Domain Criteria (RDoC). A total of 41 adult inpatients meeting DSM-5 criteria for major depressive disorder were assessed on a comprehensive battery of measures spanning emotional, cognitive, and social domains.</p><p><strong>Results: </strong>Multiple linear regression analyses revealed that trait emotional intelligence and particularly its well-being and self-control subscales, was the most robust predictor of self-efficacy. Female patients often demonstrate better cognitive profiles than males, suggesting a potential modulatory role of sex hormones. Estrogens, particularly estradiol, have been hypothesized to exert neuroprotective effects via modulation of neurotrophic, neurotransmitter, and inflammatory pathways. However, evidence regarding the relationship between sex hormone levels and cognition in schizophrenia remains scattered and inconclusive.</p><p><strong>Methods: </strong>This scoping review synthesized evidence from clinical and preclinical studies examining the association between endogenous estrogen levels or estrogen-based treatments and cognitive functioning in women with schizophrenia. We included peer-reviewed literature addressing hormonal fluctuations across the menstrual cycle, menopause, and in response to antipsychotic treatment, with specific focus on estradiol and selective estrogen receptor modulators.</p><p><strong>Results: </strong>Findings suggest that low estrogen levels may be linked to more severe negative symptoms and poorer cognitive performance in female patients. Studies investigating menstrual cycle phases report mixed results, while evidence from animal models indicates potential cognitive benefits of estrogenic compounds. Raloxifene has shown promise in mitigating cognitive deficits when administered during sensitive developmental periods. However, large-scale hormonal interventions in postmenopausal women have produced inconsistent results, likely due to differences in timing, dosage, and individual neurobiological context.</p><p><strong>Conclusions: </strong>Sex hormone levels, especially estradiol, appear to influence cognitive outcomes in women with schizophrenia, though findings are heterogeneous. Future research should address methodological inconsistencies and explore phase-specific, personalized hormonal strategies. {"title":"COERCIVE PSYCHIATRIC TREATMENT: UNRAVELING SOCIODEMOGRAPHIC PATTERNS AND THE POLYPHARMACY CHALLENGE.","authors":"Karina Cernika, Jelena Vrublevska","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>This study aims to analyse certain populations' sociodemographic factors and to investigate the use of polypharmacy for inpatients assigned to involuntary treatment at the National Center of Mental Health, Riga, Latvia.</p><p><strong>Methods: </strong>A retrospective review of clinical data was undertaken in adult inpatients assigned to involuntary psychiatric treatment in the period from December 17th, 2024, till February 4th, 2025. The data about the participants' sociodemographic factors, hospitalisation duration, diagnosis, and treatment strategies were obtained from the hospital's databases. Only data from a fixed 30-day window at the time of analysis were considered, irrespective of hospitalization duration, to ensure consistency and avoid bias from ongoing treatment modifications. The Antipsychotic Total Daily Dose Calculator was used to assess polypharmacy load by estimating the BNF (British National Formulary) coefficient relative to the recommended maximum daily dose. All data were collected in MS Excel and analysed using IBM SPSS 29.0.1.0.</p><p><strong>Results: </strong>The study included 88 inpatients with compulsory medical measures. Results showed a mean age of the participants equal to 41.49±2.41 years, males' dominance making 86.36% (n=76) of the study group, and schizophrenia (F20, ICD-10) noted as the most dominant diagnostic category. Polypharmacy with more than one antipsychotic medication was observed in 89.77% (n=79) of the inpatients. The most common treatment combination was an antipsychotic with a mood stabilizer, prescribed to 44.32% (n=39) of participants. Treatment was primarily guided by antipsychotics, with haloperidol emerging as the most commonly used drug, representing 72.72% (n=64) of the cases, and mood stabilizers received by 57.95% (n=51) of the inpatients. The analysis revealed that 34.09% (n=30) of participants had a BNF coefficient within the recommended maximum daily dose, while the majority, or 65.9% (n=58), exceeded the recommended threshold.</p><p><strong>Conclusions: </strong>This study contributes to existing knowledge of currently available data about polypharmacy in psychiatric practice and certain population groups undergoing involuntary hospitalisation. We describe two cases which are in the public domain.These cases are not isolated incidents but part of a broader, systemic failure to provide sustained, assertive, and well-resourced community-based mental health care. It is crucial that the techniques of Assertive Outreach we have discussed are not only adopted in principle but also fully integrated into everyday practice, backed by proper funding, staff training, and institutional support. Without this commitment, we risk continuing the tragic cycle of preventable deaths, where both patients with severe mental illness and innocent bystanders pay the price for gaps in the system.
