This essay includes a proposed method (matrix) to visualize virus-related and psychosocial risks for different populations side by side in a systematic and concise manner. COVID-19-related and psychosocial vulnerability, stressors, direct and indirect consequences are derived on a theoretical and empirical basis. An exemplary quantification of the matrix for the vulnerable group of people with severe mental illness revealed a very high risk for severe COVID-19 consequences, as well as a pronounced risk for psychosocial collateral effects. The proposed approach could be further discussed for a risk-graded pandemic management, crisis recovery, and future preparedness to adequately address psychosocial collateral effects and better identify and protect vulnerable groups in this regard.</p>","PeriodicalId":20711,"journal":{"name":"Psychiatrische Praxis","volume":" ","pages":"381-388"},"PeriodicalIF":1.1,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://ftp.ncbi.nlm.nih.gov/pub/pmc/oa_pdf/09/03/10-1055-a-2051-7613.PMC10567134.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"9404563","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Längst vorbei die Zeiten, als die Exponenten einer biologisch orientierten Psychiatrie auch in der Deutschen Fachgesellschaft die Wirksamkeit von Psychotherapie generell bezweifelten. Global betrachtet, ist Psychotherapie unbestritten wirksam, und zwar mit etwa gleicher Effektstärke wie Pharmakotherapien, wie Leichsenring et al. in einem großen Umbrella Review und einer Metaanalyse festgestellt hatten [1]. Die Kehrseite dieser erfreulichen Nachricht ist, dass beide großen Therapieansätze bei einer metaanalytischen Bewertung der Evidenz nur schwache Effektstärken in der Größenordnung zwischen 0.3 und 0.4 zeigten. Dies stellt nicht in Abrede, dass manche Therapien wie die Behandlung akuter Psychosen oder Angstzustände pharmakologisch wesentlich bessere Effektstärken zeigen und auch psychotherapeutisch bei ausgewählten Indikationen sehr gute Erfolge erzielt werden. Ungeachtet der insgesamt überschaubaren Effekte werden ambulante psychotherapeutische Leistungen in Deutschland in großem Umfang, wenngleich in sehr unterschiedlicher regionaler Verteilung, von den gesetzlichen Krankenkassen vollständig übernommen. Mit dem Direktstudiengang für Psychologische Psychotherapeuten wird sogar ein neues Berufsbild geschaffen, der Bedarf wird als hoch eingeschätzt und nicht nur die Bundespsychotherapeutenkammer, sondern auch einige Kassenärztliche Vereinigungen unterstützen den Wunsch nach einer staatlichen Finanzierung der Weiterbildung. Letzteres wäre ein bemerkenswerter Vorgang, zumal Weiterbildung im Gegensatz zur Ausbildung keine staatliche Aufgabe ist und auch die ärztliche Weiterbildung folglich bisher staatlich nicht gefördert wird. Etwas zu denken gibt ob der vermeintlich prosperierenden Aussichten allerdings nun ein neues großes Umbrella Review [2], das die Ergebnisse von „Face-to-Face“ kognitiver Verhaltenstherapie mit Internet-basierter kognitiv-behavioraler Verhaltenstherapie vergleicht, bei der Therapeut*innen nur über unregelmäßige Textnachrichten mit Patient:innen kommunizieren und sie bei der Nutzung Internet-basierter Therapieprogramme anleiten, Rückmeldungen geben etc., ohne dass die Behandlung aber feste Termine in der Sprechstunde oder per Video beinhaltet. Diese Behandlungsform ist somit sehr viel weniger ressourcenintensiv. Nachdem in einer 2016 erschienen Metaanalyse mit 20 randomisiert-kontrollierten Studien (RCT) keine substantiellen Unterschiede in den Ergebnissen im Hinblick auf Symptombesserung gefunden werden konnten, konnten jetzt weitere 11 RCTs eingeschlossen werden, womit die Metaanalyse sich auf 31 RCTs mit über 3000 Teilnehmenden aus 9 Ländern stützen konnte. Die Qualität der eingeschlossenen Studien wurde als akzeptabel beurteilt, die behandelten Störungen umfassten Depression, soziale Ängstlichkeit, Panikstörung, Insomnie, Tinnitus, Tierphobien, Bulimie, Gesundheitsstörungen, Fibromyalgie, psychische Belastungen bei Krebserkrankungen, posttraumatische Belastungs","PeriodicalId":20711,"journal":{"name":"Psychiatrische Praxis","volume":"156 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135661186","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
