Airway remodelling in asthma consists of mucus hypersecretion, epithelium erosion, extracellular matrix protein deposition under the basement membrane, smooth muscle proliferation and angiogenesis. The vascular endothelial growth factor (VEGF) plays a part in remodelling through its effect upon angiogenesis, but the extent of its contribution to asthma pathology is not yet completely understood. Irreversible airway remodelling seems to be an important contributor to asthma progressive worsening, so further developments in preventing remodelling are warranted. We present the case of a 51-year-old female patient, heavy smoker, known with chronic obstructive pulmonary disease (COPD) for 12 years, untreated, who was hospitalized for the first time in our clinic having symptoms of a severe COPD exacerbation. She has significant cardiac disease (rheumatic mitral disease, with previous episodes of pulmonary edema and cardiac arrest) and hepatitis B. The patient is hypoxic, with severe mixed ventilatory dysfunction. During the hospitalisation she received treatment of the exacerbation and after that she received recommendation of chronic inhaled bronchodilator and corticosteroid treatment. The test for alpha-1 antitrypsin deficiency has detected a plasma of 63 mg/dl, SZ phenotype. The patient returns for a second evaluation. Functional tests are significantly improved (despite inconsistent treatment) with the impressive improvement of FEV7 values and identification by plethysmography of a restrictive syndrome. Echocardiography identifies mitral valve changes likely rheumatic, severe pulmonary hypertension. Computer tomography was performed, highlighting discrete interstitial changes and denying the existence of emphysema. Marked increase in FEV1 values supported adding bronchial asthma to the list of diagnosis and recommendation to continue inhaled corticosteroid combination bronchodilator as treatment. The effect of continuous positive airway pressure (CPAP) therapy on blood pressure (BP) values has been evaluated in several short-term clinical trials with conflicting results. Our aim was to investigate the role of long-term CPAP treatment in achieving BP control in patients who associate OSA and resistant hypertension. We have included in the study 33 patients with resistant hypertension, diagnosed with OSA in our sleep lab. Data was collected initially and after a mean follow-up period of 4 years. Patients were divided into 2 groups according to the use of CPAP therapy. Patients under CPAP therapy (n = 12) exhibited a higher reduction in both systolic and diastolic pressure and BP control was achieved in 75% of cases, while patients without CPAP treatment (n = 21) remained with refractory hypertension in proportion of 90.5%. A de-escalation of antihypertensive drug regimen by discontinuation of 1 or more drugs was observed in 41.6% (n = 5) of patients from CPAP group and in the other 33.4% (n = 4) the medication remained unchanged, but BP control was reached. {"title":"Long-term gender-specific evolution of blood pressure under CPAP therapy in hypertensive patients with obstructive sleep apnea syndrome.","authors":"Oana Claudia Deleanu,&nbsp;Andra Elena Mălăuţ,&nbsp;Anca Donoaica,&nbsp;Ana-Maria Nebunoiu,&nbsp;Florin Mihălţan","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Reduction of blood pressure (BP) under CPAP treatment in patients with obstructive sleep apnea syndrome (OSAS) associating hypertension (HT) is controversial and short-term evolution is often measured using the sphygmomanometer.</p><p><strong>Purpose: </strong>To study the variation in BP (sphygmomanometer and Holter) after 3 and 6 months of CPAP in OSAS patients associating hypertension.</p><p><strong>Methods: </strong>We applied the exclusion criteria (hypoventilation, respiratory diseases, secondary hypertension, antihypertensive treatment modification during study, non-compliance) on 96 consecutive patients (SPSS 17.0: Chi test, T-test).</p><p><strong>Results: </strong>15 hypertensive patients (8.53 years from diagnosis) with OSAS succeeded six months of following: 3 women (20%), 12 men (80%) were comparable as age, body mass index and Epworth score; women had more severe OSAS. Sphygmomanometer measuring in men showed a decrease in systolic BP (SBP) (142 ± 8.9 to 128.7 ± 11.7 mmHg, p = 0.005) and diastolic BP (DBP) (82 ± 17.19 to 69.1 ± 6.6 mmHg, p = 0.040) at three months of treatment. Women had no changes at 3 and 6 months of assessment using the sphygmomanometer. BP Holter showed no significant changes in men; women exhibit a significant increase in maximum DBP/24 hours (104 ± 13.4 to 169.5 ± 27.5 mmHg, p = 0.034) and mean daytime DBP/24 hours (100 ± 14.1 to 166 ± 32.5 mmHg, p = 0.046) from 3 to 6 months. No group presents dipper status change to 3 or 6 months.</p><p><strong>Conclusions: </strong>The trend in both groups of increase in BP for 3 to 6 months is explained by the natural evolution of an old HT history. Pulmonary hypertension (PH) has severely bad influence on prognosis in COPD patients. Hence, early diagnosis of it is important for appropriate therapy. Echocardiography is used for this purpose, which requires cardiologist and expensive equipment which may not be available anywhere. CA-125, a biomarker of ovarian cancer, has shown to be associated with left ventricular failure. We aimed to show the relationship between CA-125 levels and PH in patients with COPD.</p><p><strong>Methods: </strong>Ninety patients with stable COPD were enrolled into the study. Levels of CA-725 were measured from venous blood, and in the same day systolic pulmonary artery pressure (sPAP) was measured by transthoracic echocardiography.</p><p><strong>Results: </strong>Of 90 Patients 57 had PH and 39 had not. Patients with PH had significantly higher CA- 125 levels compared with controls (mean 39.15 U/ mL vs. 24.22 U/mL, P < 0.04). Levels of CA-125 were correlated with sPAP (r=017, P=0.01).</p><p><strong>Conclusions: </strong>The CA-125 biomarker can be used to identify COPD patients with pulmonary hypertension. The bacteriological diagnosis, genetic and phenotypic, becomes essential for the epidemic control. The resistance appears as a phenotypic expression of mutations from M. tuberculosis genome. The mutations that appear for Rifampicin are in region rpoB, for Isoniazid in region katG and inhA, for Ethambutol-embB, Quinolone-gyrA, Aminoglicozid and Cyclical Peptides-rrs. To follow the concordance of results of drug sensitivity test (DST) through phenotypic and genetic method, we analyzed a group of 40 patients with TB-DR. We performed drug susceptibility testing on Lowenstein-Jensen medium according to the instructions of the manufacturer. The strains were tested indirect genetic too, Genotype MTBDR plus forl INH and RIF and Genotype MTBDRsl for the second line drugs. The concordance between genetic method and the phenotypic method is 95%, 5% from the patients have different sensitivity to INH and RIF, butphenotypical they are resistant, meaning that they have other mutations undetected by the strip. The most common mutation in region rpoB is MUT3 (52%) associating the absence of band W8. Mutations in the region rpoB MUT1 and MUT2A are 12.5%, and 15% respectively. For high resistance to INH, the mostcommon isMUT1 forkatG 95% and forlow resistance to INHMUTI from region inhA-30%. For the second line drugs, the most frequent concordance between genetic method andphenotypic method is for EMB, of 30%, geneticallyspeaking the strains display no mutation in region embB, but are resistantin phenotypic method. For FQ, KAN, AMKand CAP the concordance between the two methods is of 100% to all tested strains.
