{"title":"The effect of therapeutic exercise for intermittent claudication due to cauda equina compression","authors":"Norio Hayashi, 吉田 徹, 見松 健太郎","doi":"10.3753/YOTSU.13.165","DOIUrl":"https://doi.org/10.3753/YOTSU.13.165","url":null,"abstract":"馬尾性間欠跛行を主訴に運動療法を実施した23例を対象とし,その効果について検討した.初診時実測した歩行距離に対し,硬膜管面積とわれわれが考案した腰椎後弯可動性テスト(PLF test)において,有意な正の相関を認めた.また22例に腸腰筋と大腿筋膜張筋に拘縮を認めた.われわれが行った,股関節ならびに腰椎の拘縮改善を目的とする運動療法は,21例(91.3%)に有効であり,初診時平均102.1 mの歩行距離が,1カ月後で8例(38.0%),2カ月後で15例(71.4%)で1 km以上の連続歩行が可能となった.その他の6例も,平均640 mの歩行が可能であった.股関節の拘縮の改善は,歩行時の骨盤前傾トルクの軽減ならびに腰椎過前弯の減少に寄与すると考えられた.また,腰椎後弯域の改善による動的pumping effectの促進は,硬膜外静脈叢の還流改善に作用し,歩行改善を得たと考察した.拘縮要素が存在する間欠跛行例では,2カ月程度を目処に運動療法を試みる価値があると考えた.","PeriodicalId":198659,"journal":{"name":"The Journal of Japanese Society of Lumbar Spine Disorders","volume":"55 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124346690","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
