Dynamic computed tomography revealed a solitary cystic tumor 14 cm in diameter with multiple septa. The cyst wall was occasionally irregular with hyperarterial inflow. After admission, he suffered from fever and right upper abdominal pain. We suspected cystadenocarcinoma with intraluminal infection. Percutaneous transhepatic drainage was performed. However, neither cytologic examination nor culture test was positive. The cystic tumor had been decreasing in size, and hepatic resection performed. Macroscopically, the tumor was a gray-yellow solid tumor with a fine boundary between tumor and liver parenchyma, and the cystic lesion collapsed. Microscopically, the tumor consisted of hepatic infarction, degenerated Glisson's sheath, and chronic inflammation, and chronic liver abscess was diagnosed. Most cases of bacterial liver abscess can be diagnosed because progression is accompanied by typical signs. However, it is difficult to diagnose liver abscess in the chronic phase because chronic liver abscesses exhibit various features on imaging series without typical signs or symptoms. Acceleration time is one of parameters of flow velocity waveforms, but only few reports had discussed acceleration time of fetal circulation.</p><p><strong>Methods: </strong>We analyzed Doppler flow velocity waveforms of fetal middle cerebral artery, descending aorta and abdominal umbilical artery obtained from 70 normal pregnant women between 18 and 40 weeks. Acceleration time was cross-sectionaly examined throughout pregnancy course.</p><p><strong>Results: </strong>The acceleration times revealed no remarkable changes throughout pregnancy course in the middle cerebral artery, but significant increases were observed in the acceleration time of the descending aorta between 18-23 and 24-32 weeks. It also significantly decreased after 33 weeks compared to that of 18-32 weeks in the umbilical artery.</p><p><strong>Conclusions: </strong>The acceleration time in fetal descending aorta is significantly shorter than that in the abdominal umbilical artery between 18 and 32 weeks of gestation. The umbilical arterial acceleration time apparently decreases throughout pregnancy course. Its values are almost double for that in the descending aorta between 18 and 23 weeks, but it becomes to be equal to the aortic acceleration time. The aim of this study was to identify the prognostic factors for survival in cutaneous melanoma in Japan by performing univariate and multivariate analyses.</p><p><strong>Methods: </strong>We studied a total of 103 patients with cutaneous melanoma, treated in Osaka City University Hospital during the period 1991-2003. Eleven factors were evaluated by uni- and multivariate analyses. Kaplan-Meier analyses were performed to estimate and compare five-year survival rates. After checking for independence, prognostic factors were then evaluated applying a Cox proportional hazards model in localized melanoma patients and in all invasive melanoma patients.</p><p><strong>Results: </strong>On univariate analysis, anatomic location, ulceration, tumor thickness, Clark's level, lymph node metastasis, distant metastasis and TNM staging were significantly related to five-year survival rates. Clark's level and TNM staging were excluded after checking for independence. Multivariate analysis revealed that significant prognostic factors were tumor thickness and anatomic location in localized melanoma patients, and distant metastasis, tumor thickness, sex and anatomic location in all invasive melanoma patients.</p><p><strong>Conclusions: </strong>Thinner tumors, female gender and location on the extremity are favorable prognostic features in malignant melanoma. Tumor thickness whose best cut-off point is 4 or 5 mm is the most important prognostic factor in predicting survival in the melanoma patient. The aim of this study is to validate the novel IVUS analysis software (NICORAS) compared to the previously validated software (EchoPlaque).</p><p><strong>Methods: </strong>Selected from clinical cases, we used 30 IVUS images that utilized motorized pullback at 0.5 mm/sec. Judging from at least two vascular landmarks, identical coronary artery segments were selected and analyzed with two software systems (NICORAS and EchoPlaque). Lumen and external elastic membrane (EEM) borderlines were traced at every 0.5 mm interval. Plaque area was calculated as EEM minus lumen area. Then, volumetric IVUS data were calculated using interpolated frame data from which lumen, EEM, and plaque volume were computed.</p><p><strong>Results: </strong>The average analyzed coronary segment was 15.4 +/- 4.3 mm. We examined the comparison of fundus autofluorescence (FAF) from a fundus camera (FC) and a confocal scanning laser ophthalmoscope (cSLO) in patients with age-related macular degeneration (AMD).</p><p><strong>Methods: </strong>Two hundreds ninety-two eyes of 202 AMD patients were examined for FAF. FAF images were captured with a FC and a cSLO system, and were compared for serous retinal detachment lesion (SRD), subretinal fibrovascular tissue lesion (FVM), pigment epithelial detachment lesion (PED), geographic atrophy lesion (GA), macular aria, and choroidal vessels.</p><p><strong>Results: </strong>Clear FAF images were obtained using both instruments in 212 eyes of 147 patients (72.6%). Correlations of FAF findings between the 2 systems existed for SRD, FVM, and GA, but not for PED. The FAF accordance rates between the 2 systems were 61.1% at SRD, 68.4% at FVM, 35% at PED, and 83.4% at GA; and GA had a significantly higher accordance ratio than other lesions. FAF images with the cSLO tended to hypoautofluorescence compared to those with a FC, except at GA. Maculas without any abnormal lesion of 41 eyes apparently hypoautofluorescence more with the cSLO than the FC. Some choroidal vessels autofluoresced in 21 eyes only with the FC.</p><p><strong>Conclusions: </strong>FAF examination differed between the two systems. The present study elucidates the similarities and differences of ADHD symptoms in PDD children compared to ADHD children.</p><p><strong>Methods: </strong>Twenty-seven male children with PDD+ADHD Combined type (ADHD-C), 17 males with ADHD-C, and 9 males with PDD without ADHD-C, were compared on measures of ADHD symptoms and related behaviors using parent ratings and teacher ratings on the ADHD Rating Scale (ADHDRS), the Child Behavior Checklist (CBCL) and the Teacher Report Form (TRF).</p><p><strong>Results: </strong>The PDD+ADHD-C and the ADHD-C groups had significantly higher ratings than the PDD only group for all the scores on both the parent-rated and the teacher-rated ADHDRS. We examined its effects on the spatial dispersion of repolarization (SDR) after implantable cardioverter-defibrillator (ICD) shock.</p><p><strong>Methods and results: </strong>In twenty five patients with an ICD, we recorded the 87-lead ECG during sinus rhythm (the CONTROL group) under general anesthesia, after NIFE administration alone, and just after termination of induced ventricular fibrillation (VF) by ICD shock with or without NIFE administration. In all recordings, the corrected QT interval (QTc) was measured in each lead. The dispersion of QTc (QTc-D; maximum QTc minus minimum QTc) was also measured. Compared with the CONTROL, the QTc-D exhibited significant deterioration after ICD shock (61 +/- 12, 91 +/- 24 ms(1/2), respectively, p < 0.001). However, the QTc-D after NIFE administration either with or without ICD shock did not differ from the CONTROL group (65 +/- 20, 61 +/- 18, and 61 +/- 12 ms(1/2), respectively, p = 0.99).</p><p><strong>Conclusions: </strong>NIFE suppressed the deterioration of SDR by ICD shock. Moreover, recent spread of surgical indication to elderly people and patients with severely damaged pulmonary function may lead postoperative complication of contralateral pneumothorax more lethal. We herein describe 2 cases of contralateral pneumothorax following lung cancer surgery with a review of recent literatures. Case 1 underwent left lower lobectomy with combined partial resection of the chest wall for lung cancer. Five months later, he suffered from contralateral pneumothorax and respiratory failure. Immediate chest drainage followed by bullectomy and pleurodesis were performed. Case 2 was surgically treated for left lung cancer. Preoperative computed tomography incidentally demonstrated contralateral pneumothorax. Chest drainage was started immediately, followed by left lower lobectomy. Contralateral bullectomy and pleurodesis were performed 6 days after lobectomy. However, few studies have evaluated the prognosis of asymptomatic PVD in patients with coronary artery disease (CAD).</p><p><strong>Methods: </strong>The aim of the study was to determine the prevalence of asymptomatic and symptomatic PVD in patients with ST-elevated myocardial infarction (STEMI) or unstable angina pectoris (UAP) who underwent percutaneous coronary intervention (PCI), and to conduct a 3-year follow-up for cardiovascular events. Systemic atherosclerosis was evaluated, using duplex ultrasonography in 380 consecutive patients who had undergone PCI from January 2003 to December 2005.</p><p><strong>Results: </strong>Twenty-four percent of the CAD patients had PVD. Patients with PVD had significantly more risk factors for atherosclerosis, including increased age, diabetes mellitus, smoking (p < 0.05), and multivessel CAD (p < 0.001). Multivariate regression analysis demonstrated that STEMI, PVD and multivessel CAD were independent predictors for major adverse cardiac events (MACE) (p < 0.05). Kaplan-Meier curves for MACE in all the patients showed that the MACE rates were significantly higher in STEMI and PVD cases compared with other cases (p < 0.05). Thus, the aim of this study was to evaluate the cross-sectional relation between conventional risk score or metabolic disorder and non-stenotic LMCA atherosclerosis using intravascular ultrasound (IVUS).</p><p><strong>Methods: </strong>We analyzed procedural and demographic data from 217 consecutive patients undergoing intervention for a left anterior descending or left circumflex coronary artery lesion, including their cardiovascular risk (Framingham risk score) and degree of adiposity. IVUS measurements for subclinical LMCA plaque were obtained in all patients and compared to their risk score (low, intermediate or high risk [< or =10%, 10-20%, and > or =20%, respectively]), with volumetric IVUS analyses being performed for the entire LMCA. Patients were further stratified by body weight (normal vs overweight/obesity [body mass index > or =25 kg/m2]).</p><p><strong>Results: </strong>Plaque volume index was greater in patients with high and intermediate risk groups compared to the low risk group (plaque volume index; 9.8 +/- 2.9 mm3/mm and 10.1 +/- 3.0 mm3/mm vs. 8.3 +/- 2.7 mm3/mm, p=0.017 and p=0.0002, respectively). Plaque volume index was also greater in patients in the overweight/obesity group compared to the normal weight group (plaque volume index; 10.2 +/- 2.9 mm3/mm vs 9.1 +/- 3.0 mm3/mm, p=0.005).
