Eighteen months previously, the patient had complained of chest discomfort, and was then admitted for the first time to our hospital for evaluation of chest pain. Electrocardiography showed poor R wave progression in leads Vl through V4, and diffuse nonspecific STsegment and T wave abnormalities with low voltage. However, no definitive diagnosis could be made at this initial admission and a calcium-channel blocker was prescribed. Despite this treatment, the patient was readmitted with worsening dyspnea and lower extremity edema. The diagnosis of heart failure and nephritic syndrome was made at the second admission. In addition, immunoelectrophoresis showed a monoclonal IgD (l) M protein and increased plasma cells in the bone marrow, suggesting a diagnosis of multiple myeloma. The patient was thus given dexamethasone (20 mg per day for 4 days) intravenously, but his symptoms did not improve. Two weeks later, the patient deteriorated further with congestive heart failure and renal failure, and subsequently died of cardiac arrest with ventricular fibrillation. On autopsy, IgD (l)-positive plasma cell proliferation was found in the bone marrow, confirming the diagnosis of multiple myeloma. In addition, amyloid deposition was detected in various organs including the heart, kidneys, esophagus, duodenum, ileum, colon, tongue, and lungs. In particular, the weight of the heart was 650 g demonstrating a hypertrophic septum and amyloid deposition in the myocardium and even coronary arteries. In summary, the final diagnosis was IgD (l) multiple myeloma associated with systemic amyloidosis. {"title":"1996 Epidemiological Survey of Dermatophytoses in Japan.","authors":"Epidemiological Investigation Commi","doi":"10.3314/JJMM.41.187","DOIUrl":"https://doi.org/10.3314/JJMM.41.187","url":null,"abstract":"日本医真菌学会疫学調査委員会の継続的疫学調査の一環として,1996年次の全国15施設における皮膚真菌症例を集計し,前回1991,1992両年の集計結果と比較検討した.皮膚真菌症総例数は8,402例で,新来患者の13.1%に相当.うち白癬患者は7,395例と皮膚真菌例の88%を占める.白癬の病型別では足白癬が4,764例(64.4%)と3分の2弱を占め,次いで爪白癬1,487例(20.1%),体部白癬558例(7.5%),股部白癬369例(5.0%),手白癬195例(2.6%),頭部浅在性白癬11例,ケルスス禿瘡7例,白癬菌性毛瘡1例,汎発性白癬1例,その他2例であった.白癬の原因菌種別分離頻度は培養陽性の2,615株中T.rubrum1,828株(69.9%),T.mentagrophytes743株(28.4%),E.floccosum20株(0.8%),M.canis13株(0.5%),M.gypseum5株,T.violaceum2株.T.r/T.m比2.46.爪白癬は増加傾向を,股部白癬は減少傾向を示し,全体に罹患年齢分布が高年齢側に増加.M.canis分離頻度も引き続き減少.15歳未満の小児の白癬は113例(全白癬の1.3%)と前回調査より著しく減少.病型別には成人に比して体部白癬,頭部白癬の比が高く,足白癬の比が低く,手・爪白癬も少ない.カンジダ症は総計722例(皮膚真菌症の8.6%),病型別には,間擦疹299例(41.4%),指趾間びらん95例(13.2%),乳児紅斑89例(12.3%),口腔カンジダ症63例(8.7%),爪囲爪炎56例(7.8%),外陰カンジダ症56例(7.8%),爪カンジダ症24例,CMCC1例,上記以外の非定型カンジダ症39例であった.癜風は265例(真菌症例の3.2%),マラセチア毛包炎15例(同じく0.2%).深在性真菌症はスポロトリコーシス3例,黒色真菌症1例,原発性膿皮症様アスペルギルス症1例であった.","PeriodicalId":19301,"journal":{"name":"Nippon Ishinkin Gakkai Zasshi","volume":"24 1","pages":"187-196"},"PeriodicalIF":0.0,"publicationDate":"2000-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"84409363","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
