The goal of the study was to verify hypothesis that diagnosed and undiagnosed asthma differ in terms of coexisting allergic disorders. Questionnaire-derived data were used to compare both groups in terms of allergic symptoms (sneezing/runny nose, watery/itchy eyes, recurrent itchy rash) and allergic diseases diagnosed by physician in the past (hay fever, eczema, allergy). Children with diagnosed asthma as compared with children with undiagnosed asthma had more nasal (54.2% vs. 37.3%, p=0.03) and ocular (40.7% vs. 23.3%), p=0.01) symptoms, had more frequent hay fever (37.2% vs. 11.2%, p","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"77 4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128366299","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of the study was to present our experience in treating SPM in healthy adolescents and identify predictors leading to prolonged hospital stay for these patients. Methods: We completed a retrospective review of 39 adolescents treated at our hospital for SPM between 1998-2018. Their medical records were reviewed for symptoms, length of hospital stay and complications. Patients with any type of underlying lung disease, trauma or previous pleural interventions were excluded from the study. Results: The majority of cases were males (80%), with an average age of 15±4 years. Notably, 21% of the patients had a prior history of asthma, 23% were smokers while 2 (5%) admitted to the use of illicit drugs. Chest pain was the main symptom described in 31 cases (80%), followed by subcutaneous emphysema (38%) and persistent cough (20%). All patients received CT scan, while radiography was diagnostic in 32 of our cases. Conservative treatment consisted of high-flow nasal cannula oxygen therapy and analgesics led to rapid resolution of SPM. There were no significant complications, although the presence of one pneumothorax treated with a chest tube and medium-grade fever in 5 cases should be mentioned. Patients with asthma history tend to had longer hospital stay (mean 9 ± 3), while the only case of early recurrence responded well to conservative treatment. Conclusions: SPM should be considered in the differential diagnosis of acute chest pain in adolescents. Asthma patients may necessitate prolonged hospital stay and require careful surveillance, eliminating complications that may influence outcome.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"164 3","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133352381","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Aims and Objectives: The primary objective was to determine efficacy of AZI maintenance therapy on respiratory exacerbations in PCD. The secondary objectives were to determine efficacy on lung function, health-related quality of life, sputum microbiology, and hearing impairment, and assess safety. Methods: Patients with confirmed diagnosis of PCD, age 7-50 years, were enrolled in this European multi-centre, parallel group, double-blind, randomized, placebo-controlled trial. Participants were randomly assigned to AZI 250/500 mg according to body weight ( Results: A total of 90 participants were randomized - 49 assigned to AZI and 41 to placebo. The AZI group had significant lower rate of exacerbations (rate ratio 0.46, 95%CI: 0.27; 0.80, p=0.006) and pathogenic bacterial species in sputum (rate ratio 0.39, 95%CI: 0.24; 0.64, p=0.0002), compared to the placebo group. There was no significant difference between the treatment groups in lung function, hearing level, or Respiratory symptoms, Sinus symptoms and Ear & hearing symptoms on the quality of life measure. Gastrointestinal complaints were more common in the AZI group. Conclusions: This first multi-national randomized controlled trial on pharmacotherapy in PCD showed that maintenance AZI for 6 months was safe and reduced rate of respiratory exacerbations and rate of pathogenic bacterial species in sputum to less than half.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133357488","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Obesity has been implicated as a pathogenic factor in asthma, but underlying mechanisms remain unclear. We hypothesized that, compared to general fat, organ fat plays a greater role in the development of childhood asthma. Methods: In a population-based prospective cohort study among 5,421 children aged 10 years, we measured general fat including body mass index (BMI) and fat mass index (total fat mass/height4) by dual-energy X-ray absorptiometry, and organ fat including subcutaneous fat index (subcutaneous fat mass/height4), visceral fat index (visceral fat mass/height3), pericardial fat index (pericardial fat mass/height3) and liver fat fraction by Magnetic Resonance Imaging. Lung function was measured by spirometry and physician-diagnosed asthma by questionnaire. Results: A higher BMI and fat mass index were associated with a higher FVC (Z-score difference (95% CI): 0.19 (0.17, 0.22) and 0.07 (0.04, 0.10)) per standard deviation score increase, respectively) and FEV1 (0.16 (0.14, 0.19) and 0.06 (0.03, 0.09)), a lower FEV1/FVC ratio (-0.07 (-0.10, -0.05) and -0.03 (-0.06, -0.00)), and not with asthma. A higher visceral fat index, conditional on fat mass index, was associated with a higher FVC (0.07 (0.03, 0.10)), a lower FEV1/FVC (-0.05 (-0.09, -0.01)) and a higher risk of asthma (Odds Ratio (95%CI): 1.20 (1.01, 1.43)). No other organ fat measures were independently associated with lung function or asthma. Conclusion: The obesity-asthma link is mainly driven by visceral fat, independent of total fat mass, which implies that abdominal fat accumulation might play a role in asthma development.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"63 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130797372","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Aims and Objectives: To investigate the effects of air pollution on mortality. Methods: We separately investigated associations of pollution exposures of particulate matter with aerodynamic diameter ≤10µm (PM10), nitrogen dioxide (NO2) and sulphur dioxide (SO2) with all cause infant, neonatal and post-neonatal mortality in 7,984,366 live births between 2001-2012 in England and Wales, adjusting for confounders (deprivation, birthweight, maternal age, sex and multiple births). Each pollutant was banded into quintiles by ranking scores for each pollutant over each LSOA and for each year for each pollutant individually. Results: When lowest pollution exposure quintile was used as the reference, increasing exposure to all pollutants resulted in increased deaths with highest exposure quintile resulting in odds ratios of 1.24 to 1.43 for infant deaths, 1.21 to 1.38 for neonatal deaths and 1.32 to 1.54 for post-neonatal deaths. The results decreased after adjustments for confounders - ORs for infant deaths were significantly increased for NO2, PM10 and SO2 (1.07, 1.04, 1.19). Neonatal death was significantly associated with SO2 (1.21) but non-significantly with NO2 and PM10 (1.04, 1.01). All pollutants were associated with post-neonatal deaths (1.11, 1.12, 1.15) albeit at different effects. Conclusion: NO2, PM10 and SO2 are differentially associated with all cause infant, neonatal and post-neonatal mortality with SO2 possibly acting via the mother leading to neonatal mortality and all three acting directly resulting in post-neonatal mortality.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"11 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125733001","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Failure to choose correct antibiotic, dose or treatment duration worsens CAP outcomes and contributes to global antibiotic (AB) resistance. Aim: to analyze and compare AB treatment among children with hospitalized CAP in 2012 and 2017. Methods: A retrospective analysis of previously healthy children with hospitalized CAP in 2012(n=85) and 2017(n=96). Cases were divided into CAP during flu and non-flu seasons; subdivided according to age ( 5y). Statistical analysis was performed with Microsoft Excel and IBM SPSS Statistics software. P value Results: There was no difference in children age, gender and duration of hospitalization between all groups. Most cases were hospitalized during flu season (68% (2012) vs 77% (2017)). Most frequent AB for CAP in 2012 was Penicillin (PEN) (59%), Cefuroxime(CXM) (28%) and Clarithromycin (CLR) (12%). Ampicillin-sulbactam (SAM) was started in 35% of children in 2017, while PEN-38%, CXM-8%, CLR-12%. Younger children were more likely to receive SAM (p=0.047). Initial dose of PEN was 0.117±0.026 mioU/kg in 2012 with slight increase in 2017 (0.125±0.022 mioU/kg) (p=0.07) with tendency of higher doses in flu season (p=0.0564). Elevation was observed in CLR dose (11.49±3.62mg/kg (2012), 17.47±5.24mg/kg (2017), p=0.0344). It was significantly higher between flu seasons (p=0.0128). Second-line AB was chosen in 31% in 2012 and 28% in 2017. CXM as second-line AB was mostly used in 2012, while SAM in 2017. Conclusion: Significant change in first-line AB (PEN to SAM) between 2012 and 2017 is observed with higher AB doses linked to flu season.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"31 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132895761","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
