Streptococcus pneumoniae is stated to be an common etiological agent for it. Hence Pneumococcal Conjugate Vaccination(PCV) has been introduced in India since 2017(phased-wise). Objective: To study effect of PCV on radiological findings of chest in infants hospitalized with WHO-defined CAP. Methods: Prospective hospital-based pneumonia surveillance is ongoing since 2015 in Lucknow & Etawah districts, Uttar Pradesh and Patna & Darbhanga districts, Bihar, India. Recruitments were done from a network of hospitals formed for project. Infants(2-11months)hospitalized with WHO-defined CAP from index districts with Results: From May2017-October2018, 282(22.7%females) infants with PCV vaccination (cases) and 570 (29.8%females) without PCV vaccination (controls) with interpretable CXRs were analyzed. Primary end point pneumonia (PEP)+other infiltrate (OI) were found in 43(15.2%) cases and 140 (24.7%)controls (p=0.001);OI in 30(10.6%) cases and 69(12.1%) controls (p =0.5) and normal CXR in 209 (74.1%) cases and 361(63.3%) controls (p=0.001). There was one death among cases whose CXR showed OI. There were 22 deaths among controls whose CXRs showed PEP + OI in 12(54.6%); OI in3(13.6%) and normal7(31.8%). Crude odds ratio for death among cases was 0.088 (95 %: CI 0.012-0.66). Conclusion: Among hospitalized patients of CAP, radiological findings differ by PCV vaccination status in infants.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"45 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115745461","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In November 2015, funding was secured for a Specialist Clinical Psychologist to join the team as the first to hold a dedicated post with this population in the UK. In the first year, 12 referrals were made to Psychology representing approximately 15% of the patient population. In year two, a further 6 families were referred, followed by 7 more in year three. Projected data for year four suggests a further 12 families will be referred for psychological support. Referral numbers by end of year four will reach approximately 46% of the total patient population. In year two, 100% of referrals were for parents presenting with clinical levels of anxiety, distress and low mood associated with their child’s medical condition, predominantly those whose children required tracheostomy-ventilation. Current data indicates a trend for increasing numbers of fathers requiring support with managing feelings associated with having a child on LTV. Themes seem to include feelings of responsibility and guilt, managing shifts in perceived paternal role, and high anxiety relating to performing care tasks. Future service delivery will need to be shaped to account for differences in psychological needs between family members. Specific psychological interventions aimed at fathers can be piloted alongside other, family-based interventions. More research is needed into the long-term psychological impact of having a child requiring LTV on all parts of the family system and at different points in the journey.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"95 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127906486","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Intake of fatty acids and antioxidants may attenuate some effects of PM but the role of overall dietary intake has not been studied. We aimed to investigate the modification of the association between air pollution and childhood asthma by the dietary inflammatory potential effect. In a cross-sectional study, 501 (48.1% males, aged 7 to 12 years) of 858 children attending 71 classrooms from 20 schools were selected. Spirometry and airway reversibility, exhaled nitric oxide, skin-prick testing and current symptoms (breathing difficulties and irritative cough) were assessed. Dietary inflammatory potential was evaluated by the Dietary Inflammatory Index (DII) and calculated from a 24-hour dietary recall. Indoor air quality measurements were conducted in classrooms for one week. The proportion of effects explained by the exposures to PM2.5 and PM10 were measured by generalized linear mixed model. After adjustments, an inflammatory diet increased the risk of asthma for PM2.5 (OR=1.67, 95% CI 1.03, 2.73) and PM10 (OR= 1.75, 95% CI 1.07, 2.87) levels. Considering the dietary inflammatory potential in PM2.5 and PM10 exposure, the risk of asthma increased in parallel with the DII, being significant for children with asthma (OR= 0.89, 95% CI 0.81, 0.97 and OR= 0.92, 95% CI 0.87, 0.97). These findings provide further support to the role of diet’s inflammatory characteristics modulating the effects of indoor air pollution, highlighting the importance of children’s diet as a potential solution to reduce the asthma risk.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"143 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127487121","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We examined the impact of asthma control shortly after diagnosis in preschool children on long-term control. Methods: We constructed Canadian birth cohorts from 4 provincial administrative databases. Preschool asthma was defined as 1 hospitalization or 2 physician visits for asthma within 2 years in children Results: Of the 118365 preschoolers with asthma, 50188 (42.4%) were included. Compared to children controlled throughout the 2 years post-diagnosis, suboptimal control was associated with a higher pooled RR for long-term poor control in a dose-response manner towards worse control patterns. Conclusions: Suboptimal control after diagnosis in preschool is a strong predictor of long-term poor control, suggesting a window of opportunity to improve future morbidity.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"78 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115532579","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
