Objectives: To investigate the association between early-life air pollution exposure and change in lung function until age 15. Methods: In the GINIplus and LISA birth cohorts, 915 children from two German regions, Munich and Wesel, were investigated, who had at least two spirometric measurements at the age six, 10 and 15. Z-scores of FEV1 and FVC were calculated according to reference values for children in Germany. Annual average concentrations of nitrogen dioxide, particulate matter with diameters Results: In Wesel, for per interquartile range increase in air pollutants during the first year of life, z-scores declined annually by -0.009 (95%CI: -0.011; -0.007) to -0.021 (95%CI: -0.026; -0.015) for FEV1, and by -0.004 (95%CI: -0.006; -0.002) to -0.010 (95%CI: -0.016; -0.004) for FVC. In Munich, the attenuations were only statistically significant for FEV1 [range: -0.017 (95%CI: -0.027; -0.007) to -0.027 (95%CI: -0.047; -0.008)]. Reduced lung function was higher for children who developed asthma between 3-15 years (although insignificant) and for those with breastfeeding less than 12 weeks. Conclusions: Early-life exposure to higher air pollution was associated with reduced lung function until adolescence. Breastfeeding may potentially reduce the risk of early air pollution on impaired lung function.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"14 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-09-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124638214","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

No data are available to date about RSV genotypes circulating in Tunisia. Aim and Objectives: The aim of the present study was to investigate the genetic variability of the glycoprotein G (GPG) gene in RSV strains prevalent in Tunisia. Methods: Nasopharyngeal aspirates (NPA) were collected from hospitalized infants and neonates with LRTI. All specimens were screened for RSV by Direct Immunofluorescence Assay (DIFA). To understand the evolution and origin of these strains, a phylogenetic analysis was conducted. Randomly selected positive samples were subjected to reverse transcription polymerase chain reaction (RT-PCR) amplifying the second hyper-variable region (2nd HVR) of the G gene of RSV. Results: Among a total of 427 samples collected between 2016 and 2017, 156 (36.5%) were positive for RSV by DIFA. Analysis of 21 randomly selected RSV strains revealed that Group A RSV (66.7%) predominated during the study as compared to group B RSV (33.3%). The phylogenetic analysis showed that 92.9% of the RSV-A sequences possessed a 72-nucleotide (nt) duplication in the 2ndHVR of the G gene and clustered ON1 genotype, while 7.1% clustered NA1 genotype. Concerning Tunisian RSV group B strains, all sequences contained a 60-nt insertion in the 2ndHVR and clustered genotype BA10. Conclusion: Since viruses continuously evolve, continued surveillance of RSV strains circulating worldwide is required to aid in the development of reliable diagnostics and future RSV therapeutics or vaccines.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"108 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127964130","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Caregivers must act immediately if tracheostomy obstruction or decannulation occurs. The Canadian Thoracic Society recommends caregivers complete a formal education program. In the literature, simulation-based learning has improved emergency skills. A module and simulation-based training program for primary caregivers was developed. Objective: Assess effectiveness of new program and retention of caregiver knowledge and skills. Methods: A single-center prospective study to identify components of knowledge and skills retained at different intervals after training. All primary caregivers of children with tracheostomies born from 2007–2017 were invited to participate. Group I was trained using modules and simulation. Group II had been trained prior to the new program. Caregiver skills were evaluated with simulation to assess management of tracheostomy obstruction. Pre and post self-efficacy questionnaires were completed by caregivers. Results: Caregiver simulation evaluations were completed (n=9) including Group I (n=5) and Group II (n=4). Overall, 89% of tracheostomy changes were done in less than 2 minutes. 45% of caregivers scored higher on the self-efficacy survey after simulation. Group I caregivers scored higher on average than Group II, but numbers did not reach statistical significance. Conclusion: Simulation-based learning is effective for training caregivers of children with tracheostomies. This study will extend to other caregivers to improve the training program. Caregivers require ongoing education and simulation opportunities to manage tracheostomy emergencies safely.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"19 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130425976","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
