Our objectives were to investigate the prevalence, phenotypic characteristics of PCOS, and cardiometabolic risk of TMNBG with and without PCOS prior to hormone therapy.</p><p><strong>Materials and methods: </strong>This was a retrospective study of 346 TMNBG adults presenting to our service for gender-affirming hormone therapy. PCOS was diagnosed according to the evidence-based international guideline for the evaluation and treatment of PCOS 2018 and the Rotterdam consensus of 2003.</p><p><strong>Results: </strong>The described transgender population had a prevalence of PCOS of 23.8%. Phenotype B was the most common one (39.1%), followed by phenotype A (33.7%). Transgender people with PCOS had statistically higher body mass index (BMI), systolic and diastolic blood pressure, plasma insulin levels, HOMA-IR, and HOMA-B than those who did not meet PCOS criteria. However, blood pressure and markers of insulin resistance were similar for both groups after controlling for BMI.</p><p><strong>Discussion: </strong>PCOS appears to be prevalent among TMNBG. Transgender people with PCOS may exhibit an unfavorable cardiometabolic risk profile compared to those without PCOS, especially due to a higher BMI. Its most frequent cause is primary hyperparathyroidism. Rarely, hypercalcemia is due to mutations in the calcium-sensing receptor (CaSR) signaling pathway that give rise to the different forms of familial hypocalciuric hypercalcemia (FHH). Two aspects are essential to suspect a probable FHH, the first is the existence of other cases in the family of elevated calcium and PTH associated with low urinary calcium excretion. In this context, a calcium clearance to creatinine clearance ratio (Ca urine 24 h. x serum Cr) / (Cr urine 24 h x serum Ca) ≤ 0.01 is highly suggestive of the diagnosis of FHH, which should subsequently be confirmed by sequencing of the genes involved in the different forms of FHH. In 2002, a mild iodine deficiency was identified in schoolchildren from La Pampa, Argentina. This study aims to reassess iodine intake in the same rural school population in 2023 by analyzing urinary iodine concentration (UIC), goiter prevalence, and salt iodization.</p><p><strong>Materials and methods: </strong>A cross-sectional study was conducted in 6- to 12-year-old schoolchildren from School No. 45 in Ataliva Roca, La Pampa. Parents provided informed consent. UIC, weight, height, blood pressure, and goiter were assessed. UIC was measured using the modified Sandell-Kolthoff method, and salt iodization was analyzed using sodium thiosulfate titration. The results were compared with those from 2002.</p><p><strong>Results: </strong>A total of 74 schoolchildren were evaluated in 2023. The median UIC was 145.45 υg/L, with 28.8% of students showing levels below 50 υg/L. Goiter prevalence was 21.6%, compared to 17.9% in 2002. In 85.5% of the salt samples, iodine levels were adequate. We describe the variables used in its assessment by echocardiography in different population groups of men and women to determine whether changes can be predicted according to their risk factors.</p><p><strong>Materials and methods: </strong>Observational and descriptive cross-sectional registry. A systematic collection of anthropometric characteristics, risk factors and specific echocardiographic variables of diastolic function was performed in an outpatient echocardiography laboratory. Quantitative variables were expressed as mean and standard deviation for Gaussian distribution variables. Discrete variables were analyzed using contingency tables, qualitative variables are expressed as percentages. Means were compared using the Student t test.</p><p><strong>Results: </strong>A total of 107 patients were admitted. Those with sedentary lifestyle had a mean left atrial volume of 29 ml/m2, a deceleration time of 218 ms, a transmittal flow ratio E/A (EA) of 1.5, tissue Doppler ratio Ee´ (Ee) of 6 and e´a´ (ea) of 1.4; those who performed physical activity 3 times a week were characterized by having a left atrial volume of 35 ml/m2, a deceleration time of 210 ms, EA of 1.8, Ee of 5 and ea of 1.8. Patients with obesity had a left atrial volume of 35 ml/m2, deceleration time of 226 ms, E/A 1.1, Ee of 9 and ea of 1.2. First-line induction treatment includes the combination of all-transretinoic acid (ATRA) and arsenic trioxide, with which complete response rates greater than 90% are achieved. Although the combination is very effective, there are adverse reactions associated with the treatment, which include cellular differentiation syndrome, headache, fatigue, elevated triglycerides, and myositis. The latter is rare and is manifested by intense muscle pain a few days after starting ATRA. Creatine phosphokinase (CPK) values are not always elevated and this can make diagnosis difficult with delayed treatment and increased risk of complications. We report the case of a woman with APL who, during the induction phase of treatment, presented myositis of the pelvic muscles and lower extremities secondary to the administration of ATRA.
