The aim of this study was to conduct the first systematic analysis of Google reviews of German university departments of otorhinolaryngology to assess patient-reported experiences. In total, 3,533 reviews from 39 clinics (2016-2023) were thematically coded and analyzed by region.The reviews showed pronounced polarization, with the majority consisting of one- and five-star ratings. Core medical domains, including physician care, nursing, and surgical treatment, were rated predominantly positive. In contrast, organizational aspects were the main sources of criticism, particularly long waiting times despite scheduled appointments, limited accessibility, and perceived staff shortages. Regionally, higher satisfaction indices were observed in eastern and northern Germany compared with western and southern regions. In addition to structural factors, patient-centered conditions such as catering, quality of stay, and digital infrastructure substantially influenced overall ratings.In conclusion, Google reviews indicate a high level of perceived medical care quality alongside notable deficiencies in organization and communication. However, due to the complex anatomy of the tongue and multiple tumor spread pathways, this approach may fail to achieve adequate oncological control.Methods: Compartmental surgery has been proposed as an alternative approach, based on en-bloc resection of the tumor, cervical lymph nodes, and tumor-node tract, following anatomical pathways of spread. A literature review was conducted using the keywords: (tongue carcinoma) OR (tongue cancer) OR (oral cavity cancer) AND (compartmental surgery).Results: Fourteen relevant studies were analyzed. The reviewed literature describes an anatomy-based, function-preserving surgical concept. The pathogenesis of both acute and chronic otitis media results from a complex interplay of anatomical, microbiological, and immunological factors. Identified risk factors include patient-related aspects such as nutrition, allergies, ethnicity, and genetic predisposition, as well as environmental factors such as socioeconomic status, tobacco use, attendance at daycare centers and recurrent respiratory infections.During the COVID-19 pandemic, strict measures led to a significant decline in the incidence of acute otitis media. However, infections with variants such as Omicron showed an increased rate of secretory otitis media. In the post-COVID phase, complication rates due to middle ear infections rose again, with prevalence returning to pre-lockdown levels.Preventive measures focus on ensuring adequate Eustachian tube function, treating allergies, and consistently administering pneumococcal vaccinations. In contrast, surgical treatment remains the gold standard for chronic middle ear infections and cholesteatoma. However, radical removal can be associated with high morbidity rates, underscoring the need for innovative approaches. These include optical coherence tomography (OCT) and minimally invasive laser surgery.Furthermore, new insights into the role of the immune system in the pathogenesis of both acute and chronic otitis media could open up new therapeutic possibilities for the future.
