A special form of SNH is hidden hearing loss (HHL) with subjective normal hearing. Current research results indicate that these patients demonstrate a reduced wave I in the averaged signal of brainstem audiometry (ABR). Since the averaging technique is not susceptible to latency jitter and amplitude height variation, a single sweep analysis is required for a deeper insight in HHL.A total of 14 mice with significantly different calcium currents in the IHC at normal hearing thresholds were analysed. For the analysis in order to calculate four new parameters from the single sweeps in the time window of wave I. These parameters also served to describe a neural activation function (NAV).Looking at the wild type all new parameters differ significantly or highly significantly. With the transgenic mouse, there are only non-significant to significant differences. There is also a significant difference in the neural activity demonstrated in the resting EEG between the wild-type mouse and the mutant. There is a negative correlation between the wave amplitudes for the wild mouse - after a strong amplitude follows a weak amplitude and after weak amplitude follows a strong amplitude.Using new parameters based on single sweeps, surprising results are obtained. Obviously the function of the IHC correlates more strongly with the new parameters than it does with the average amplitude of wave I. The TubaInsert introducer and TubaVent Short balloon catheter form a slim single-use BDET system requiring a four-handed technique with intraoperative endoscope handover. This enables two-handed catheter insertion. This study evaluated usability.Sixteen ENT physicians (aged 26-65) without prior four-handed BDET experience performed bilateral procedures on cadaveric heads. A surgical assistant guided the endoscope. The procedure was divided into 22 steps assessed by observation and user surveys.All 16 participants completed bilateral BDET successfully. In 32 test runs, full balloon application was achieved. All procedural steps, including four-handed insertion, were performed correctly (100% success). Endoscope handover did not impair technical safety or feasibility.This study shows high usability of the TubaInsert BDET system across users with varying experience. OSAS-specific symptoms were assessed using a questionnaire (day of surgery, follow-up: 3 months) and results were presented using box-whisker plots for the overall collective and the AT and ATT surgical techniques (significance level α=5%). Bar charts were used to illustrate the pre- and postoperative risk stratification.On average, the postoperative Brouillette score (-3.41 AT: -3.33 ATT: -3.68) was significantly lower for the overall collective and the individual surgical techniques than preoperatively (-0.22 AT: -0.49 ATT: 0.7). The prevalence of hearing loss increases with age. The aim of this project was to assess the current hearing situation in nursing homes and to identify problems in the provision of care.Pure tone and speech audiograms were measured in 74 residents in four different nursing homes in the greater Hanover area and questionnaires on the hearing aid care situation were carried out with 97 residents.Of all audiograms measured, 75% showed a hearing loss requiring hearing aids, which was often inadequately cared for. Residents often stated that they could hear well subjectively, but at the same time mentioned many situations in which they would have liked to hear better. Existing hearing aids were often worn not regularly or even not at all. Those who are affected by hearing loss wished for help in using their devices.In an ageing society, a growing burden on care facilities is to be expected. Hearing loss is an omnipresent issue here, for which more awareness must be created in order to ensure adequate hearing care. Surgical (including total laryngectomy, LE, followed by adjuvant radio(chemo)therapy) and nonsurgical approaches aiming on larynx organ preservation (including definitive concomitant chemoradiation or sequential induction chemotherapy (IC) followed by radiotherapy) are available. These treatment modalities will be critically discussed in this review.In line with the national German S3 guidelines for laryngeal and oro-/hypopharyngeal cancer, patients must receive comprehensive counseling on both surgical and non surgical concepts. To date, early response to IC constitutes the most robust surrogate marker for organ preservation candidacy, guiding the early redirection of non responders to upfront LE. Late salvage LE after definitive radiochemotherapy is associated with high perioperative morbidity, often rendered unfeasible due to radiation-induced fibrosis and impaired vascularity; thus, it should not be considered as a standard treatment option.IC and response evaluation after just one cycle of IC, as applied in the DeLOS-II study, allow for early identification of non-responders, for whom LE (associated with significantly lower complication rates than salvage surgery) should be recommended prior to radiotherapy. The neoadjuvant use of immunotherapy in combination with IC likely represents the next frontier in the context of laryngeal organ preservation.
