The prevalence of hearing loss increases with age. The aim of this project was to assess the current hearing situation in nursing homes and to identify problems in the provision of care.Pure tone and speech audiograms were measured in 74 residents in four different nursing homes in the greater Hanover area and questionnaires on the hearing aid care situation were carried out with 97 residents.Of all audiograms measured, 75% showed a hearing loss requiring hearing aids, which was often inadequately cared for. Residents often stated that they could hear well subjectively, but at the same time mentioned many situations in which they would have liked to hear better. Existing hearing aids were often worn not regularly or even not at all. Those who are affected by hearing loss wished for help in using their devices.In an ageing society, a growing burden on care facilities is to be expected. Hearing loss is an omnipresent issue here, for which more awareness must be created in order to ensure adequate hearing care. {"title":"[Tracheotomy in children and adolescents - results from 15 years of clinical practice].","authors":"Lilo Stroink, Florian Hoppe, Elisabeth Rehberg","doi":"10.1055/a-2535-0366","DOIUrl":"https://doi.org/10.1055/a-2535-0366","url":null,"abstract":"<p><p>Tracheostomy in children and adolescents is a rare procedure that differs from tracheostomy in adults in terms of indications, implementation and treatment challenges.This study is a unicentric retrospective analysis of the group of children and adolescents who were treated with a tracheostomy in the ENT Clinic at Hospital between 2007 and 2023.A total of 47 patients were treated with a tracheostomy. Anatomical changes in the airways and long-term ventilation were the most common indications. In 61.7 % of the children, the cannula was removed successfully during the course of treatment. {"title":"[Balloon dilatation of subglottic stenoses, especially in granulomatosis with polyangiitis].","authors":"Jeannette Wichmann, Markus Hoffmann, Martin Laudien","doi":"10.1055/a-2357-8895","DOIUrl":"10.1055/a-2357-8895","url":null,"abstract":"<p><strong>Introduction: </strong>Subglottic stenoses (SGS) are constrictions in the region below the vocal folds, that can pose a life-threatening problem for those affected. The aim of this research project was to identify the patient group for which the treatment of SGS by balloon dilatation can lead to long-term success.</p><p><strong>Methods: </strong>14 patients with SGS were examined (before and up to12 months after intervention) using pulmonary function tests, laryngoscopies and two questionnaires (Clinical Chronic Obstructive Pulmonary Disease Questionnaire, modified Medical Research Council Dyspnoea Scale) regarding the respiratory situation. Additionally, the number of necessary interventions, the degree of stenosis (according to Myer Cotton and measured in millimeters) were documented.</p><p><strong>Results: </strong>Four patients who required only one intervention with the balloon showed better pre-interventional respiratory function and a higher subjective stress level. Lung function tests and evaluation of the questionnaires showed an improvement in the respiratory situation and state of health of all 14 patients after the initial intervention. The degree of stenosis measured in millimeters shows that patients with a stenosis diameter of less than 4 mm only required one intervention. All patients who received an intervention using a 12-mm-balloon had to be treated several times.</p><p><strong>Conclusions: </strong>Patients with a high degree of stenosis and a tendency towards greater subjective stress and less objective restriction of the respiratory function appear to have a higher probability of long-term successful treatment with the balloon.
