Wallach","doi":"10.5455/AJDI.20180426034053","DOIUrl":"https://doi.org/10.5455/AJDI.20180426034053","url":null,"abstract":"Objective: To study the perceptions of Medical College of Georgia residency directors and clerkship directors, and students on the utilization of point-of-care ultrasound (POCUS) in medical education and clinical practice. \u0000 \u0000Methods: An online, anonymous survey was distributed to clerkship and residency directors and undergraduate medical education (UME) students. Participants were asked to indicate their level of experience and agreement for teaching specific POCUS applications. \u0000 \u0000Results: The response rate was 40.13% among UME students (N = 274), 100% among residency directors (N = 17), and 100% among core clerkship directors (N = 9). Majority of residency and clerkship directors agreed to include most of the proposed POCUS applications except for pediatric bladder catheterization (35%) and inferior vena cava to evaluate the presence of hypovolemia (46%). There was no more than 12% disagreement to include any application among both cohorts of responders. Within medical students, the most desired application to learn was ultrasound-guided central line placement (58% strongly agreed; 33% agreed) followed by ultrasound-guided peripheral venous access (88% strongly agreed or agreed), and assessment for presence of pericardial and pleural effusion, abdominal / pelvic fluid collections, and cholelithiasis. \u0000 \u0000Conclusions: This survey documents the desired learning and teaching applications for the POCUS curriculum content. Choriocarcinomatous pulmonary embolism represents a rare and aggressive extra-gonadal form of gestational trophoblastic disease, most often occurring after a normal or pathological pregnancy. The presentation is similar to that of cruoric embolism, but it is characterized by local invasion and significant hematogenous dissemination leading to distant metastases. Increased Human chorionic gonadotropin hormone (hCG) levels are suggestive of the diagnosis. We report a case of choriocarcinomatous pulmonary embolism involving the inferior lobar artery of the left lung in a 44-year-old female patient admitted for multiple ischemic strokes. The diagnosis of choriocarcinoma was done on CT scan in the presence of high levels of Human chorionic gonadotropin hormone (hCG) contrasting with negative pelvic imaging and a history of previous spontaneous abortion. The depth of the carcinoma invasion has been demonstrated to be a predictor of cervical metastasis, recurrence, and disease-specific survival; depth of invasion (DOI) is also clinically used as a criterion for performance of elective neck dissection in early oral SCC. Both post-con-trast three-dimensional Fast Field Echo (3D-FFE) imaging and Dixon T2-weighted imaging (T2WI) may be useful to predict pathological DOI and can contribute to a successful treatment plan and patient prognosis. The purpose of this study was to evaluate the usefulness of post-contrast 3D-FFE imaging compared with Dixon T2WI for assessing radiological DOI, compared with pathological DOI, in oral tongue SCC. Methods: Twenty-two patients who have been diagnosed with tongue SCC were included in this study. The Wilcoxon singed-rank test was used to compare radiological DOI with pathological DOI, using both 3D-FFE imaging and DIXON T2WI. Results: The correlation coefficient between radiological DOI and pathological DOI using post-contrast 3D-FFE imaging was 0.79 ( p < 0.01). The correlation coefficient between radiological DOI and pathological DOI using Dixon T2WI was 0.72 ( p < 0.01). Positioning errors and artifacts were the most significant factors for reject analysis in chest radiography. Positioning errors are the most common cause of rejection (16%), and artifacts are the second reason (11.33%). In consequence to positioning error, the scapular shadow was partially located at the lung field in 38% of the images. \u0000Conclusion: chest radiographs are the most common x-ray examination requested. Positioning errors are still challenging, which affect the quality of the image. It may be seen as the intramuscular involvement of disseminated disease, as an extension from adjacent bone or lymph nodes or as primary extranodal lymphoma. An 83-year-old female patient presented with the complaint of pain and swelling at her right calf. Ultrasound (US) examination of the region demonstrated a hypoechoic appearance of the affected muscle and subcutaneous tissue. Magnetic resonance imaging of the right calf demonstrated the long segmental involvement of the flexor digitorumlongus muscle and the adjacent subcutaneous tissue with the increased T2 signal, vivid enhancement, and diffusion restriction. US-guided core needle biopsy of the muscle was performed and histopathological evaluation demonstrated a B cell lymphoma. Further imaging workup with computed tomography for staging purpose revealed no other organ/system or nodal involvement.
