Worldwide, research continues into various factors that may influence the success of catheter treatment of atrial fibrillation. In this regard, the influence of the state of the sympathoadrenal system of the body has not been studied enough. Aims to evaluate the effectiveness of interventional treatment of atrial fibrillation (AF) in patients with and without myocarditis, depending on the level of beta-adrenergic responsiveness of the organism. Materials and methods. The study sample included 40 patients. Of these, 27 (67.5%) are men. The age in the sample was 49 (44; 55) years. The study included 25 (62.5%) patients with paroxysmal AF, 10 (25.0%) with persistent and 5 (12.5%) with long-term persistent. All patients underwent surgical treatment of AF using radiofrequency (RFA) or cryoablation (CBA). Endomyocardial biopsy was performed in 18 patients to verify the diagnosis. Beta-adrenergic responsiveness (beta-ARM) was assessed in dynamics 3 days after surgery, 3 months and 12 months. The follow-up period for patients was 12 months. Results. All patients underwent surgical treatment of AF, in 7 cases (17.5%) CBA of the pulmonary veins was performed, in 33 (82.5%) cases RFA was performed. According to the biopsy results, signs of myocarditis were detected in 9 patients (22.5%). Focal myocarditis was found in 7 patients, diffuse in 2. The effectiveness of catheter treatment in general was 72.5%, for CBA 71.5%, for RFA 73.5%. Beta-ARM before ablation was 19.16% [12.46; 27.46], 3 days after ablation 24.43% [15.38; 33.65], after 3 months was 20.27% [9.90; 27.71], in 4 patients after 12 months, beta-ARM was 32.5% [20.0; 43.2]. The dynamics and influence of beta-ARM on the effectiveness of interventional treatment of AF in patients with early and late relapses, the presence of myocarditis was assessed. Conclusions. The results obtained showed no effect of the level of beta-ARM on the effectiveness of RFA and CBA in patients with and without myocarditis. Beta-ARM showed no association with the development of early arrhythmia recurrences after ablation. This publication discusses the primary mechanisms of inflammatory diseases development of the myocardium and pericardium based on innate and acquired immunity disorders, including data from our research confirming the autoimmune and autoinflammatory nature of these diseases. The paper presents the main clinical manifestations and laboratory markers that enable differential diagnosis between autoimmune and autoinflammatory heart diseases. At the same time, vascular tissue engineering has diverse methodological approaches for the development of effective functionally active small-diameter vascular prostheses suitable for adaptive growth and regeneration in situ. An important aspect is the possibility of personalizing the created prostheses not only by taking into account the individual anatomy of the patients vascular bed, but also by using autologous components to create such a prosthesis, which can be obtained directly from the recipient. The presented problematic article reflects the main results on the creation of biodegradable vascular prostheses of small diameter, obtained at the Research Institute of the Research institute for complex issues of cardiovascular diseases (Kemerovo). The functionality of the prostheses was provided both through the incorporation of biologically active components with proangiogenic potential for the purpose of complete remodeling in situ, and the formation of cell-populated vascular prostheses using autologous cells and proteins from patients with coronary heart disease. Over the past decades, there have been large positive shifts in survival in this cohort of patients. An integral specialty of cardio-oncology is rapidly developing, which is aimed at improving the results of treatment of oncological patients. Due to increased life expectancy and improved long-term prognosis in cancer patients, in particular, the adverse effects of anticancer therapy have attracted great attention from researchers. The causal relationship between cancer and CVD can be partly explained by a common profile of modifiable and non-modifiable risk factors. There is also strong evidence that a systemic inflammatory response is a common pathophysiological determinant of cancer and CVD. General risk factors for development and progression, as well as pathogenetic mechanisms, contribute to high mortality rates. Increasing knowledge of pathophysiological mechanisms will help elucidate the general molecular aspects of carcinogenesis and cardiovascular disease. Over the past few years, scientific data have demonstrated that patients with non-obstructive coronary artery disease can have high risk for adverse cardiovascular events (ACE) despite the absence of severe coronary obstruction. From this point of view, some patients require special monitoring and treatment; we have to find new methods for stratification of the risk of ACE. Aims to study the association of coronary artery calcification (CCA) with indicators of myocardial blood flow (MBF) and myocardial flow reserve (MFR) in patients with non-obstructive coronary artery disease (CAD). Methods. The study included patients (n = 52) with non-obstructive CAD ( 50%), identified by CCTA. All patients received dynamic myocardial SPECT according to the two-day rest-stress protocol with the radiopharmaceutical agent 99mTc-methoxy-isobutyl-isotnitrile and the pharmacological stress-test (adenosine triphosphate 160 mcg/kg/min) to determine quantitative indicators of MBF and MFR. Depending on the calcium index (CI), three groups of patients were formed: 1 without CCA (CI = 0 Agatston units), 19 patients; 2 Mild CCA (CI = 1100 Agatston units), 21 patients; 3 Moderate CCA (CI = 101400 Agatston units), 12 patients. Results. The study included 52 patients (age 55.0 9.8 years, 36 men). The groups differed statistically significantly (p 0.05) in terms of quantitative scintigraphic parameters: stress-MBF, MFR and MBF. When analyzing the groups in pairs, it was found that stress-MBF and MBF were significantly lower in the group with moderate CCA compared to the group without CCA, and MFR was lower in the group with moderate CCA compared to the groups without CCA and with mild CCA. Correlation analysis revealed significant relationships between CI and scintigraphic parameters: stress-MBF ( = 0.46; p = 0.003), MFR ( = 0.48; p = 0.001), MBF ( = 0.48; p = 0.0008), SSS ( = 0.34; p = 0.02) and SDS ( = 0.28; p = 0.046). Conclusions. It has been found that -Klotho is closely associated with aging and has anti-aging properties. Besides, it is involved in development of malignant tumor diseases. Its universal influence on many biological mechanisms leading to hyperplastic changes in cells and tissues has been proven experimentally. Recent studies demonstrated inhibitory effect of -Klotho on the classical carcinogenic pathways such as insulin-like growth factor 1 (IGF1), fibroblast growth factor (FGFs), transforming growth factor 1 (TGFb1), Wnt, P53/p21, oxidative stress response pathways. Impeding effect of -Klotho protein on the development of such types of malignant tumors as breast, pancreatic, lung, stomach, hepatocarcinoma, and others is proven in a number of cases. In order to achieve the technological sovereignty of the Russian Federation, the transformation of existing scientific reserves into in-demand technologies and technologically independent sectors of the economy, special attention is paid to the frontier areas of world science. At the same time, frontiers are understood as thematic areas on the basis of which Russian critical technologies will be developed in the import-saving paradigm. Aims the purpose of the study is to identify the most promising niches of the global pharmaceutical market and research areas focused on their expansion. Methods. Analysis of the volume of RD financing by the worlds leading pharmaceutical companies in the period 20152022, multi-criteria scientometric analysis and thematic mapping of the collection of the most highly cited reviews published in 20202022 and indexed in the Scopus database. Results. The analysis of the volume of RD financing by the worlds leading pharmaceutical companies in the period 20152022, and their average annual growth rates, was carried out. It is shown that with an annual increase in the corporate RD budget of 2.8%, by 2022 the total RD expenditures of key players in the global pharmaceutical market reached $182 billion per year. At the same time, 60% of these funds ($109.4 billion) fell in 2022 to the top 20 global pharmaceutical companies investing in the development of new medicines. For comparison, the internal costs of research and development of the Russian Federation are given, which, taking into account the GDP deflator as of 08.04.2022, are estimated at $48 billion, of which the amount of budget funding provided in 2023 for the priority direction of the Strategy of Scientific and Technological Development of the Russian Federation Transition to personalized medicine, high-tech healthcare and technologies health savings, including through the rational use of medicines (primarily antibacterial), will amount to 39.5 billion rubles. Given the relatively low level of funding for domestic research and development of new medicines (compared to the budgets for RD of pharmaceutical companies in the world), it seemed important to identify the most promising niches of the global pharmaceutical market and research areas focused on their expansion. Conclusion. To date, many attempts have been made to establish a certain role of immune cells and the microenvironment in the development of endometriosis. Nevertheless, despite intensive studies of endometriosis, the role of inflammatory cells and molecules has not yet been fully studied. As we know, the pathobiology of endometriosis is not fully understood, and its progression is associated with a local and systemic inflammatory reaction. It is important to clarify the role of the immune system to better understand its significance in the pathogenesis of endometriosis, especially in the case of atypical and endometriosis-associated ovarian tumors. Over the past 10 years, the incidence of benign prostatic hyperplasia (BPH) over 80 cm3 has increased 34 times. With indications for surgical treatment, transurethral methods are increasingly used today with the use of laser or plasma techniques for resection and enucleation of the prostate. The article presents a comparative analysis of the results of plasma transurethral enucleation of the prostate (P-TUEP) and open prostatectomy, methods of surgical and drug prevention of dysuric complications, which indicates the primacy of the choice of P-TUEP in large and giant BPH surgery. Aims to prove the effectiveness of the use of plasma surgical methods and the scheme of complex medical postoperative prevention of late dysuric and cicatricial complications in the treatment of benign prostatic hyperplasia of large and giant sizes. Methods. The design of a retrospective cohort trial included several stages conducted at the urological center of the clinical hospital RZD-Medicine, Nizhny Novgorod in 20082019. The object of the study were patients with a confirmed diagnosis of large BPH (over 80 cm3) and giant (over 250 cm3) size, divided into the main and control groups according to the methods of surgical intervention and postoperative prevention of dysuric complications. The selection of patients was carried out by the method of paired design with an equal number of observations and maximum comparability of the initial parameters. Data analysis was carried out using the methods of parametric and nonparametric statistics, multivariate statistical analysis. Results. Comparison of the results of P-TUEP (n = 122) and open prostatectomy (n = 122) in groups of patients with large and giant BPH showed the possibility of reducing intraoperative blood loss by 1.5 times, the duration of bladder catheterization by 3.8 times, the frequency of early postoperative complications by 1.7 times and the duration of hospitalization by 2.9 times, which significantly saves hospital costs. Modification of the initial stage of P-TUEP, which consists in creating access to the prostate using new anatomical landmarks to maximize the preservation of the anterior fibrostromal complex, reduces the risk of trauma to the external urethral sphincter, which was proven by a three-fold decrease in the frequency of stress incontinence after surgery. In order to prevent late dysuric and cicatricial complications of P-TUEP, a scheme of complex medical postoperative prophylaxis with the use of a hyaluronic acid preparation was developed. It has been histologically and clinically confirmed that instillations of a hyaluronic acid solution into the bladder in combination with standard postoperative therapy make it possible to accelerate reparative processes by 24 times and get rid of irritative symptoms at 612 postoperative weeks, depending on the volume of removed BPH.
