The aim of this study was to compare the results of patients who underwent lower extremity Doppler ultrasonography (DUS) with a preliminary diagnosis of deep vein thrombosis (DVT) before and after the pandemic, regardless of COVID-19 PCR testing. \u0000Methods: Patients who underwent RDUS in the Department of Radiology during a total period of 35 months (01/08/2018 - 01/07/2021) were analyzed. A total of 599 patients underwent RDUS during the study period. \u0000Results: More pozitif DVT findings were observed after the pandemic (n=43, 18.3%, p=0.005) (only two of these patients had a positive COVID-19 PCR result). History of hospitalization in the last three months, D-dimer, PLT, MPV, WBC, neutrophil count, lymphocyte count, NLR values, comorbidity and antiaggregant use were not different (p>0.05). \u0000Conclusion: Even individuals (especially elderly individuals) who have been exposed to the virus during the pandemic but who do not show symptoms are at risk of DVT.

Then, a collective training lasting 2 days and 1.5 hours per day was given by an ENT specialist from the researchers and a post-test was applied. Afterwards, the topics in which the researchers made the most mistakes or remained uninterpreted were recorded and a training program including these topics was created and training was given. \u0000Results: Of the 21 family physicians over 30 years of age, 2 (14.29%) were male and 18 (85.71%) were female. When the time of graduation from medical school of the family physicians in the study was analyzed, the number of family physicians with 5-10 years of medical school graduation was 4 (19.05%), 10 (47.62%) with 10-20 years of medical school graduation, and 7 (33.33%) with more than 20 years of medical school graduation. All of the family physicians who participated in the study think that ENT education is important for primary care medicine. \u0000Conclusion: ENT diseases are common in primary care. When high PSA values are detected, a prostate biopsy is performed considering the possibility of prostate cancer. PSA elevation is not specific to prostate cancer, but may also be caused by conditions such as benign prostatic hyperplasia (BPH), urinary tract infection, and chronic prostatitis. Prostate cancer is not detected in approximately 66% of patients undergoing a biopsy, and patients are exposed to unnecessary biopsy and biopsy complications. Chronic prostatitis is detected in approximately 40% of these biopsies. The two-glass test is based on examining urine before and after rectal examination, which is used in diagnosing chronic prostatitis. In this study, we aimed to reveal the two-glass test’s effectiveness in predicting the incidence of prostatitis and inflammation in patients with a PSA value of 2.5-10 ng/ml and who underwent prostate needle biopsy. \u0000 \u0000 Methods: Fifty-two male patients, aged between 50 and 78 years, with PSA values between 2.5 and 10 ng/ml, who applied to our clinic were included in the study. EPS-two-glass test and prostate biopsy were applied to all patients. EPS; is a sample obtained by removing the fluid from the urethra after a prostate massage; VB-3; shows the urine produced by taking about 10 ml of urine voided after massage. EPS and VB3 detect prostate infection. Under the microscope, ≥10 leukocytes were considered significant for prostate inflammation. According to the pathology results, the patients were divided into 3 groups; prostate cancer, BPH, and chronic prostatitis. The chronic prostatitis group was classified according to the histopathological calcification described by Nickel. \u0000 \u0000Results: In this study, the ratio of chronic prostatitis was found to be 38%. VB3 positivity was found to be statistically significant in the chronic prostatitis group compared to the other groups (p = 0.028). Although there was no statistically significant difference between the prevalence of inflammation and PSA elevation, PSA was found to be higher in the multifocal inflammation subgroup than in the focal inflammation patient group. \u0000 \u0000Conclusion: The relationship between chronic prostatitis and PSA elevation remains a mystery. Although no statistical relationship was found between inflammation and PSA elevation in this study, the significant correlation between chronic prostatitis and VB3 positivity reinforces the possibility of this relationship. We believe that our results will form the basis for further studies to avoid unnecessary biopsies.

The identification of these individuals is essential to guide appropriate management strategies and reduce the risk of complications and the associated increased healthcare costs and mortality. Venous thromboembolism (VTE), encompassing deep vein thrombosis (DVT) and pulmonary embolism (PE), represents a major global health concern due to its substantial morbidity and mortality rates. General practitioners (GPs) play a vital role in the assessment and initial screening of patients for thrombophilia, as they perform their activities at the entrance of the health care system - in primary care. In addition, they serve a heterogeneous group of patients - from newborns to pregnant women and adults, knowing their risk factors and underlying diseases well. In general practice, an enduring doctor-patient relationship is usually established and the medical history is documented and well known, making it possible to carry out screening initiated in general practice with great success. The most common genetic defects that lead to thrombophilia are Factor V Leiden mutation, Prothrombin gene mutation, Protein C deficiency, Protein S deficiency, Antithrombin deficiency. Multiple acquired conditions have also been linked with an increased predisposition towards VTE development, including oral contraceptive use, hormone replacement therapy (HRT), pregnancy, postpartum period and malignancy. Thrombophilia screening in general practice should be guided by clear indications to identify individuals at increased risk of thrombotic events.
