Typical involvement includes the malar areas and the lower eyelids, however vulvar involvement is rare. These lesions are characterized with papules which are small, yellowish, firm seperated from each other and can be 1-5 mm in diameter, and are often associated with increased vulvar discomfort and itching. The lesions are generally multiple, bilateral and symmetrical. A 35-year-old woman presenting with a 3-year history of vulvar papules, sometimes itching and increasing in time is presented to our clinic. The patient has been diagnosed as syringoma with clinical and histopathological results. Case: 42 year-old G2 P2 patient with uterus didelphis was referred with a painful abdominal wall mass of approximately 4x5 cm size. The mass that has contrast matter enhancement on CT was excised with median laparotomy below the umbilicus. The mass which was thought as endometriosis was excised without remaining any pieces to prevent recurrence and primarily repaired. Discussion: Along with the increasing incidence due to increased cessarean rates the endometrial wall endometriosis is an important diagnosis that should not be omitted. Bu amaçla IgM, IgG ve gerekli durumlarda IgG aviditesi sonuçlar›na bak›lm›flt›r. Ayr›ca bu dönemde bak›lan hepatit B yüzey antijeni (HBsAg), “human inmmunodeficieny virus” tip 1 ve 2 antikoru (Anti HIV), sitomegalovirüs antikoru (AntiCMV) IgM ve IgG, hepatit C antikoru (AntiHCV), rubella antikoru (Antirubella) IgG ve IgM ve VDRL (“venereal disease research laboratory”) test sonuçlar› da incelendi. Bulgular: Gebelerin 313’de (%19) antikor titresi bilinmemekle beraber, 876’da (%65.71) IgG ve IgM seronegatifli¤i, 426’da (%31.95) IgG seropozitifli¤i, 13’de (%0.97) ise IgM seropozitifli¤i, 18’de (%1.35) IgG ve IgM seropozitifli¤i görülmüfltür. Di¤er taraftan avidite testi yap›lan IgM ve IgG seropozitifli¤i saptanan 18 hastan›n 13’ünde yüksek avidite bulundu ancak dördüne avidite bak›lamad›. Di¤er serolojik testlerden ise HBsAg pozitifli¤i %3 (47/1526), AntiHCV pozitifli¤i %0.8 (12/1474), VDRL %0.4 (5/1056), rubella IgM antikoru %0.7 (9/1254), rubella IgG antikoru %76.6 (940/1227) oran›nda pozitif izlendi. AntiCMV IgM 17 (17/1284, %1.3), IgG ise 1027 (1027/1279, %80.3) olguda pozitif bulundu. Sonuç: Trakya Bölgesindeki gebelerde toksoplasma ile gebelik öncesi karfl›laflma oran› yüksek (%30) olmakla birlikte ilk trimesterdeki gebe kad›nlar›n yar›s› seronegatiftir. {"title":"THE EFFECTS OF LOW DOSE HORMONE THERAPY ON SERUM LIPID PROFILE, GLISEMIC CONTROL, THYROID HORMONES AND ANDROGEN LEVELS IN POSTMENOPAUSAL WOMEN","authors":"A. Turan","doi":"10.5505/TJOD.2011.81488","DOIUrl":"https://doi.org/10.5505/TJOD.2011.81488","url":null,"abstract":"SUMMARY Objective: To compare the effects of norethisterone acetate (NETA) and drospirenone on thyroid hormones, glisemic control, serum lipid profile and androgen levels. Material and methods: 50 postmenopausal women with intact ovaries & uterus who have vasomotor complaints but otherwise healthy were prospectively enrolled into the study and 41 of them completed the study. Patients were randomly divided into two groups and prescribed 1mg E2 / 0.5 mg NETA (n=21) or 1mg 17 s E2 / 2 mg drospirenon (n=20) for 3 months. After the treatment thyroid hormones, glisemic control, lipid profile and androgen levels were compared both within the groups and between the groups. For the statistical analysis besides descriptive statistical methods paired and unpaired t test, Mann-Whitney U test and chi square test were used. Statistical significance level was accepted as p≤ 0.05. Results: There was no difference regarding body mass index (BMI), endometrial thickness and thyroid hormones both within the groups and between the groups after the treatment. Total cholesterol (p=0.03), insulin (p=0.009), Homeostasis model assessment (HOMA) (p= 0.01) and fibrinogen (p=0.01) levels were decreased in NETA group after three months. In drospirenon group there was decrease in LDL cholesterol level (p= 0.02), increase in trigliseride level (p=0.04). There was decrement in total testosterone level in both NETA (p=0.0001) and drospirenon (0.001) groups. Conclusion: NETA have beneficial effects on both lipid and carbohydrate metabolism in postmenopausal women. Drospirenon decreases LDL cholesterol while increasing trigliseride level and has no effect on carbohydrate metabolism.
