{"title":"THE VALUE OF COLPOSCOPY, HIGH RISK HPV-DNA AND HISTOPATHOLOGIC EXAMINATION IN THE MANAGEMENT OF ABNORMAL PAP SMEAR RESULTS","authors":"V. Bayramov, Y. E. Sukur, S. The aim of our study was to assess the value of HPV-DNA, colposcopy and histopathologic examination in the management of patients with abnormal cervical cytology. Materials and methods: This prospective cohort study was conducted in Ankara University Cebeci Hospital gynecology outpatient clinic. The study compromised 86 patients with smear results of ASCUS, AGC, L-SIL and H-SIL. Age, gravity, parity, age at first coitus, smoking status, number of partners, high risk HPV-DNA status and pathologic colposcopy findings were investigated. Results: Mean age of the patients was 35 years and the most common abnormal smear result was ASCUS, with 55 patients (63%). There was no significant difference between the groups regarding demographic characteristics except age at first coitus and pathologic colposcopy findings. Among patients diagnosed as ASCUS 34.5% had been upgraded with histopathologic examination. Among patients diagnosed as L-SIL 73.6% had been down-graded with histopathologic examination. However, after the histopathologic exam of 9 H-SIL patients two (22.2%) were diagnosed as CIN II, two (22.2%) were diagnosed as CIN III and one (11.1%) was diagnosed as cervical cancer. In the groups of ASCUS, L-SIL and H-SIL the presence of high risk HPV-DNA were 11%, 5% and 33%, respectively. Conclusion: If we are sure that the patient will attend the follow-up visits, then for the ones with ASCUS and L-SIL a repeat smear test would be the appropriate. Material and method: We reviewed 24 patients that underwent hysterescopy with the complaints of amenorrhea, hypomenorrhea, recurrent pregnancy loss between 2004-2008. The most complaints of patients were infertilty amenorrhea. Results: Adhesions occurs mainly as a result of trauma to the gravid uterine cavity in 14 patients. When classifying patients with their intrauterine adhesions, Grade 3 was the most frequently seen. Adhesiolisis was performed with hysteresopic scissors in all patients. In postoperative period following synechiolysis, 10 patients were treated with estrogen and progestogen, 11 of them used intrauterine device with estrogen and progestogen therapy, foley catheter was used in 3 patients. Hysterescopy was performed in 5 patients for second time because of adhesion suspicions. The re-adhesiolysis performed to 3 patients because of determined to mild adhesion. Conclusion: After hysterescopic adhesiolysis, all patients with the complaint of amenorrhea had regular menstruation. Material and methods: The research was conducted on 100 healthy pregnant women who applied to gynecology clinic in Research and Practice Hospital of Ahmet Necdet Sezer University. The data collected by face to face interview. \"Patient Recognition Form\" was used to collect information about individual socio-demographic characteristics and data about their pregnancy. Sleep quality was evaluated by using Pittsburgh Sleep Quality Index (PSQI), sleep apnea risk by Berlin Sleep Questionnaire and sleepiness by Epworth Sleepiness Scale. Observations: 86% of the pregnant women involved in our study were found to have bad sleep quality. It was determined that sleep quality in pregnant women is in relation with age, obesity, frequency of physician visits and presence of pregnancy affecting diseases (p<0.05). Blood volume and erythrocyte mass increase, physiological anemia occurs and blood pressure decreases. Cardiac flow rate and stroke volume increase. Although healthy pregnant women can tolerate these changes, pregnant women with cardiac disease may get worse and the changes may cause risk for both the women and their fetuses. A valve disease with a favorable outlook before pregnancy may get worse when pregnancy is superimposed and pregnancy may have to be terminated. In such cases pregnancy plays an important part in selection of the type of treatment. Diagnostic methods and treatment alternatives should be appropriate for individual patients. Health professionals should give priority to maternal health rather than fetal health, mothers should be informed about possible risks and treatment alternatives should be discussed with them. Any treatment given to mothers may affect their fetuses and treatment should be selected accordingly. Mothers with a valve disease should undergo a thorough examination, should be informed about the risks of pregnancy and should be referred to tertiary health care centers before pregnancy and thereby they can give birth without problems. Management of valve diseases in pregnancy involves replacement of contraindicated drugs with safe ones, controlling conditions likely to cause cardiac loading, close follow-up and aggressive treatment of conditions aggravating the disease. Echocardiography should be performed to determine pulmonary pressure and all cardiac hemodynamics including the valves and when there are changes in symptoms, the patients should be evaluated in each trimester. Even if pregnancy is tolerated well, infections, anemia, arrhythmia, pulmonary embolus, pain and anxiety may worsen the condition. In these cases aggressive treatment is required. Liposomal doxorubicin is the long-acting formula of doxorubisin and it causes PPE in 50% of the patients when used with the dosage of 50 mg/m2. The disease causes burning sensation in the hands and feet and then shows progression by edema and erythema.
