Hemşinli","doi":"10.5455/jtomc.2017.11.140","DOIUrl":"https://doi.org/10.5455/jtomc.2017.11.140","url":null,"abstract":"Objective The aim of this study was to investigate whether the neutrophil/lymphocyte ratio is an independent predictor of poor prognosis, mortality, co-morbidities, critical limb ischmemia, amputation, or a potential prognostic biomarker predicting pathological and survival outcomes in thromboangiitis obliterans. We also investigated whether there was any relation between an increased neutrophil/lymphocyte ratio in patients with Buerger’s disease and amputation, the ultimate outcome of poor prognosis. Materials and Methods The hospital records of patients treated, followed-up and diagnosed with thromboangiitis obliterans were analyzed retrospectively. Surgical procedures performed and medical treatments applied were recorded from patients’ clinical data. Extremities exposed to the disease during its course were identified. Amputations were recorded as minor and major. Neutrophil and lymphocyte values were recorded from peripheral blood samples at time of first presentation to hospital, with the neutrophil/lymphocyte ratio being calculated from these. One hundred ten subjects were finally enrolled as the patient group (TAO; n=110). A control group (n=49) was established consisting of smoker volunteers with no health problems. Statistical changes in neutrophil/lymphocyte ratio values between the groups were investigated. Results No statistically significant difference was observed between the two groups’ basic characteristics. In terms of neutrophil/lymphocyte ratio values, a statistically significant increase was observed in the patient group compared to the control group. We observed no statistical correlation between amputations and neutrophil/lymphocyte ratio values. This clinical condition; called as “compulsive water drinking”, “psychogenic polydipsia” and “water intoxication”. Psychogenic polydipsia which is characterized by excessive sensation of thirst and compulsive water drinking has been reported in 6-20% of patients in psychiatry clinics and most commonly seen (80%) in patients with diagnosis of schizophrenia. It can also be seen in patients with diagnosis of mental retardation, bipolar disorder, alcohol dependence, eating disorders and organic mental disorder. Nonspecific skeletal pain can also be seen in patients with vitamin D deficiency. The association between low levels of vitamin D and non-specific musculoskeletal pain, including FMS, is controversial. The aim of this study is to assess the relationship between vitamin D deficiencies and disease activity, life quality of FMS patients. Materials and Methods: Patients with Fibromyalgia (FMS) classified according to American College of Rheumatology (ACR) criteria. 25-OH vitamin D levels below 20 ng/ml were accepted as vitamin D deficiency. Study groups classified as first group; patient with isolated FMS (n:36), second group; isolated vitamin D deficiency (n:41) and third group was FMS in together with vitamin D deficiency (n:70). Pain level was measured by the visual analog scale (VAS). Functional and life quality assessments were determined by using Short Form Heath Survey 36 (SF-36) and Fibromyalgia Impact Index (FIQ). Results: In isolated FMS group, vitamin D level was found higher than other two groups (p>0.05). Prescribing an inappropriate drug is a global problem. Objective: The objective of this study was to assess the adherence of prescribers towards the basic principles of prescription order writing. Materials and Methods: A retrospective study was conducted from February 18 - 22, 2013 to assess the prescriber adherence to basic principle of prescription in Shashemene Referral Hospital (SRH) & Malka Oda District Hospital (MODH) pharmacies on oneyear prescriptions. Results: A Total of 384 prescriptions were analyzed of which 234 &150 prescriptions were collected from MODH & SRH pharmacies respectively. The main findings were, name of the patients and sex was mentioned in 100% & nearly 98% of the prescriptions in both hospital pharmacies. Some prescriptions were found with missing date and legible handwriting which accounted 58.12%, 10% (MODH) & 59.17%, 10% (SRH). However, similar results were obtained for illegible handwriting (10%) in both hospital pharmacies. The most frequently missed drug information by prescribers was the dosage form which accounted 63.25% & 56.67% for MODH & SRH respectively. Conclusion: The results showed that there was relatively poor adherence of prescribers following the basic principles of prescription order writing in both hospital pharmacies. However, the results of SRH were better than MODH. Materials and methods: 10 patients diagnosed with primary immune deficiency and receiving regular intravenous immunoglobulin (IVIG) were randomly included to the study, then their IVIG replacement therapy changed as rapidly infused SCIG in same monthly dose. Patients were evaluated in different times for following aspects; serum IgG levels, frequency of infections, side effects, local reactions, and improvement of life quality. IgG levels of patients were measured at the beginning, 3, 6, and 12 months of SCIG replacement treatment. Results: Local reactions were high at the beginning, then decreased with recurrent infusions. Any severe systemic reactions were not observed in patients. Less infection rate was seen in four patients who were not receiving IVIG regularly before with good compliance in all patients. Infection frequency remained same in 4patients. Increased levels of IgG were achieved eight of the patients at end of the 6months and their levels remained as stable at the end of a year. Minimal surgical procedures can be performed on an outpatient basis. In this study, the early results of our minimally invasive operative approach cases are compared with Karydakis flap reconstruction results. Material and methods: From January 11, 2012 to January 23, 2013, 312 patients were diagnosed with pilonidal sinus and treated in our clinic. Eighty-two of these patients (35 treated by a minimally invasive procedure (group I), 47 by the Karydakis flap procedure [Group II]) were included in the study. Data were collected by reviewing the patients’ records and by conducting telephone interviews. Results: The follow-up time of each case for Group I was 35 months and the average follow-up time was 32.5 months for Group II (interval, 24-40 months). Group I patients returned to work in 1 day; however, 40 patients (85.1%) in Group II returned in 20 days and 7 (14.9%) returned to work in 30 days. In group I, 32 patients (94%) were satisfied and 2 patients (5.7%) were not satisfied. In Group II, 32 patients (68.1%) were satisfied with the results of the surgical procedure and 15 patients (31.9%) were not satisfied. Our recurrence rates for group I and II were 5.7% and 6.4% for the first year, respectively. The goal of this study was to investigate the levels of total glutathione, which plays a central role in cellular antioxidant defense, in the gingival crevicular fluid (GCF) and plasma of individuals with gingivitis and periodontally healthy. Materials and Methods: Fifteen periodontally healthy subjects and 15 individuals with gingivitis were enrolled in the study. Samples of GCF, plasma, and clinical measurements were attained before and one month after non-surgical periodontal therapy. GCF and plasma levels of total glutathione were analyzed using spectrophotometric assay. Results: It was found that the levels of total glutathione in GCF were lower in gingivitis group than those in the periodontally healthy group (P<0.05), while there was no statistical difference in the plasma levels of total glutathione between gingivitis and periodontally healthy groups (P>0.05). Also, a statistically significant rise in GCF and plasma total glutathione levels in gingivitis group was found after periodontal therapy compared to baseline (P<0.05). Furthermore, there was a negative correlation between total glutathione and gingival index (P<0.05). Conclusion: Findings of this study showed that GCF and plasma levels of total glutathione could be influenced by gingival inflammation. A 9-month-old girl underwent central venous catheterization via the right subclavian vein. Catheter tip was evaluated mistakenly as in the right atrium by plain radiography. The catheter was retracted 2 cm and 1/3 normal saline infusion was initiated at 15 ml/hr. The patient became hypoxemic at the 20th hour of fluid infusion. On radiography, the right lung was completely opaque and the catheter tip was positioned in the right lung. Bedside sonography revealed a massive pleural effusion in the right lung and the catheter tip in the right pleural space. The catheter was removed and the pleural effusion was drained via chest tube. The location of the catheter tip should be determined by an accurate technique before infusing fluid.
