We described a patient with celiac disease coexisting with autoimmune hepatitis and effectiveness of gluten-free diet on both diseases. A young patient with eleveted serum liver transaminases was diagnosed as celiac disease and autoimmune hepatitis according to clinical, laboratory and pathological findings. Clinical complaints and laboratory findings of patients resolved exactly after gluten- free diet. However, liver transaminases eleveted and clinical manifestations re-emerged when he left the diet. {"title":"The evaluation of kidney biopsy results in the 6 years period: single center experience -","authors":"B. In our study, we aimed to evaluate the results of kidney biopsies that have been performed in the last 6 years. Material and Methods: The results of 202 patients who underwent renal biopsy in our hospital between 2011 and 2017 were retrospectively evaluated. Results: The mean age of the patients were found as 50.7 ± 15.1 years. The male and female numbers of the patients were 114 (56.4%) and 88 (44.6%) respectively. Proteinuria in 96 patients (47.5%) was the most common indication for kidney biopsy. When we evaluated the biopsy results; 123 (60.9%) of the patients were diagnosed for primary glomerulonephritis, 43 (21.3%) of the patients were diagnosed for secondary glomerulonephritis while 36 (17.8%) of the patients were diagnosed for tubulointerstitial involvement. The most common pathological diagnosis were Focal Segmental Glomerulosclerosis in 40 patients (19.8%) and Immune Globulin A Nephropathy in 38 patients (18.8%) in our patient group who had primary glomerulonephritis. Conclusion: The most common pathology of the patients who had native kidney biopsy was primary glomerulonephritis. 61.3% of the patients who had diagnosis for Diabetes Mellitus had pathologies other than Diabetic Nephropathy shows the importance of screening any other co-existing pathologies in this group of patients. It is essential to diagnose this condition early in order to avoid the serious complications. We describe a case of 58-year-old male presented with bleeding gums and burning sensation for 6 months. Patient was previously misdiagnosed as gingivitis. Reverse flow posterior interosseous flaps as an option in hand reconstruction and the ways to obtain higher survival rates based on our experience is described and discussed in this article. Materials and Methods: From July 2003 to June 2013, 45 patients aged between 7 and 69 years (mean 34.6 years) were operated for soft tissue reconstruction of hand using distally based posterior interosseous flap. Results: Operative plan was changed ın one out of 45 flaps due to small pedicle calibre. Only one of the flaps was totally necrosed. No partial necrosis was seen. Conclusion: Reliability of posterior interosseous flaps are sometimes unpredictable due to the anatomical variability of posterior interosseous artery. We do not attempt to make pedicle dissection more proximal to the midpoint of dorsal axis line. Extension of the distal part of reverse flow posterior interosseous flap into the distal third territory is strongly recommended in every case, especially for more distant defects. In addition to the microscopic diagnosis, radiological examination, serological methods and ELISA method are used for the diagnosis of extra-intestinal helminth infections. If these investigations are not enough for diagnosis, surgical intervention and pathological examination can be required. Materials and Methods: In addition to the clinic symptoms and biochemical and hematological lab tests of three women who admitted to our hospital presenting with positive F. hepatica Indirect Hemagglutination (IHA) in microscopic and radiological examination, findings consistent with F. hepatica were detected in these patients and the difference between the starting time of symptoms and starting time of treatment has been emphasized. Results: There was no animal feeding history but a history of eating fresh vegetables in our case back grounds. Malignancy was suspected after the initial of symptoms in three of our cases and surgery was performed for two cases and the result of biopsy was compatible with abscess. F hepatica eggs were seen in direct microscopic investigation in one case. Cyst hydatic IHA test was detected positive at high titer in two cases. Eosinophilia was found in three cases as well and radiological findings were compatible with Fasciola infection. In our cases where Fasciola IHA test was positive, the duration between beginning of symptoms and diagnosis was approximately one to three months. Materials and Methods: Forty female Spraque Dawley rats were divided into 4 equal groups as follows: control group (C) animals were fed with normal rat chow and tap water, apricot group (A) animals were fed with chow contain %5 apricot and tap water, acrylamide group (AA) animals were fed with normal rat chow and acrylamide at approximately 500 µg/kg/day via tap water, acrylamide+apricot group (AA+A) animals were fed with chow contain 5% apricot and approximately 500 µg/kg/day acrylamide via drinking water. The study procedure was maintained during 12 weeks experiment period. At the end of the study, samples of liver tissue were collected for biochemical, histopathological and molecular analyses. Results: In this study, comparison of acrylamide group liver tissue GSH levels and GSH-Px activities were found lower when compared to the control group (p < 0.05). There were no significant differences between the groups based on GST activity, histopathological results and GST-Pi gene expression mRNA levels (p < 0.05). {"title":"Efficacy of monopolar radiofrequency in middle and lower face laxity","authors":"H. In this study, the efficacy of monopolar RF energy in mid-lower face skin sagging was evaluated. Material and Methods: A total of 42 patients between the ages of 34 and 67 were included in this study. All patients underwent monopolar RF and were followed for 6 months. The monopolar RF energy level was determined based on the maximum level that each patient could tolerate. Patients were photographed with a digital camera before and 6 months after the procedure. The sagging grade of the mid-lower face before and after treatment was determined using the Merz skin laxity scale (MSLS). According to the MSLS, the level of laxity was divided into 5 groups: none, mild, moderate, severe, and very severe. The patient satisfaction levels were grouped into the following categories: not satisfied, minimally, well, and perfectly satisfied. Patients were monitored for side effects. Results: After the RF treatment, the mean MSLS score decreased from 2.60±0.80 to 1.86±0.87. The decrease in skin laxity after treatment was statistically significant (p=0.0001173). Patient satisfaction was correlated with treatment response (p=3.7x10-7). Erythema and edema were observed in all 42 patients; two patients complained of fat tissue reduction. Conclusion: Monopolar RF is an effective method for treating mid-lower face skin laxity.
