Accurate fibrosis staging is essential for clinical management, yet liver biopsy remains invasive. Non-invasive tools such as transient elastography and serum biomarkers provide alternatives, but conventional markers like APRI and FIB-4 have limitations. Mac-2 binding protein glycosylation isomer (M2BPGi) has emerged as a novel biomarker for fibrosis assessment across various liver diseases. M2BPGi levels correlate with fibrosis severity in chronic hepatitis B, outperforming APRI and showing comparable performance to FIB-4, especially in patients with ALT flare-ups. Its predictive value for HCC has been shown in both untreated and treated CHB patients, with distinct cutoff values for risk stratification. In chronic hepatitis C, M2BPGi aids in fibrosis staging and predicts HCC occurrence post-sustained virologic response. In patients with metabolic dysfunction-associated steatotic liver disease, it correlates with advanced fibrosis and cirrhosis but requires further validation. We explored factors affecting nutritional status and body composition in RTT individuals using dietary assessments and bioelectrical impedance analysis (BIA). This study involved 40 participants and found that underweight individuals with severe growth deficits often had reduced body fat, visceral fat, and arm circumference. Protein and zinc intake percentages were significantly lower in those with higher severity scores. Adolescents with the highest growth deficit and underweight ratios had caloric, protein, and calcium intakes below the Dietary Reference Intakes (DRI). Over 88 % of adults had insufficient calcium, magnesium, iron, zinc and fiber intake. Among the 15 females with RTT, sarcopenia was present in 14 (93.3 %), while sarcopenic obesity was present in 4 (26.67 %). Therefore, comprehensive dietary and body composition assessments, including body mass index, body fat, skeletal muscle, and mid-upper arm circumference, are crucial for identifying nutritional disorders in RTT. Its clinical manifestations are age-related and affect multiple systems, ranging from the characteristic café-au-lait macules, visible on the skin from birth, to plexiform neurofibromas (PNs), which have the potential to transform into malignant tumors during adolescence and adulthood. The Taiwan Child Neurology Society convened a series of meetings with experts from various specialties to review the latest evidence and updates related to NF1, including the recent approval of selumetinib for pediatric patients with symptomatic and inoperable PN in Taiwan. A consensus on diagnosis and management was established with the goal of enhancing disease understanding and providing guidance to local clinicians. Emphasis was placed on the importance of early recognition, diagnosis, and continuous health supervision, with the use of a multidisciplinary team (MDT) approach. Surgical stabilization of rib fractures (SSRF) has emerged as a promising approach to reduce pulmonary complications. We aimed to analyze the benefits of SSRF beyond flail chest and make the study applicable to clinical practice.</p><p><strong>Methods: </strong>This study compared SSRF and conservative treatments at a tertiary trauma center. A time series analysis and propensity score matching were applied to evaluate the outcomes. Data from a trauma registry (January 2017 to December 2020) were used. Exclusion criteria included patients with non-salvageable injuries and those transferred after 72 h. Primary outcomes were ventilator-free days on day 28 (VFD28) and non-procedural pulmonary complications (NPRCs).</p><p><strong>Results: </strong>Of 770 eligible patients, 692 were enrolled, with 164 undergoing SSRF. SSRF patients had significantly longer VFD28 (22.5 vs. 19.0; P = 0.042) and reduced NPRCs (8.0 % vs. 14.5 %; P = 0.086). Subgroup analysis showed that SSRF patients requiring mechanical ventilation had longer ICU-free days (IFD28) and hospital-free days (HFD90) and lower mortality rates. Those not receiving ventilation but with more than three rib fractures also benefited from SSRF in terms of reduced NPRCs.</p><p><strong>Conclusion: </strong>SSRF improved clinical outcomes in more than just flail chest cases, benefiting patients with multiple rib fractures, both with and without mechanical ventilation.
