{"title":"Acute isolated ptosis after dental treatment","authors":"Reema Mahmoud DMD, Sofi Matot MD, Avraham Abergel MD, Shimrit Arbel DMD","doi":"10.1016/j.adaj.2025.06.009","DOIUrl":"10.1016/j.adaj.2025.06.009","url":null,"abstract":"<div><h3>Background</h3><div>Isolated ptosis due to sinusitis without accompanying symptoms, such as ophthalmoplegia or orbital cellulitis, is rare, with few cases reported in the literature and none linked to dental procedures.</div></div><div><h3>Case Description</h3><div>The authors describe a rare instance of isolated left-sided ptosis in a 5-year-old girl after dental treatment under nitrous oxide sedation. Computed tomographic imaging revealed pansinusitis without intraorbital complications. Proposed mechanisms included inflammation exacerbated by means of bacterial spread after the dental procedure and the use of nitrous oxide as well as possible compression of the superior division of the oculomotor nerve. Treatment with intravenous antibiotics led to near-complete resolution of ptosis within 5 days.</div></div><div><h3>Practical Implications</h3><div>Outcomes of this case highlight the importance of recognizing isolated ptosis as a rare complication of rhinosinusitis, particularly in pediatric patients. In addition, they highlighted the clinically significant overlap between headaches and temporomandibular disorders (TMDs).</div></div><div><h3>Types of Studies Reviewed</h3><div>Information was sourced from the <em>International Classification of Headache Disorders</em>, Third Edition, for headache diagnoses and from the <em>International Classification of Orofacial Pain</em> for orofacial pain diagnoses. Management guidelines were from the European Academy of Neurology. Data on the overlap between headache disorders and TMDs were drawn from a systematic review and observational studies.</div></div><div><h3>Results</h3><div>The authors provided a concise, practical, clinical guide for identifying and managing primary and secondary headaches commonly encountered in dental practice, which is grounded in established guidelines and robust scientific evidence. The authors emphasized the importance of interdisciplinary management and a thorough diagnostic approach to enhance treatment outcomes for patients with concurrent headaches and TMD, highlighting the frequent overlap of these disorders.</div></div><div><h3>Practical Implications</h3><div>For dental care clinicians and other health care practitioners, recognizing the intersection of orofacial pain, headaches, and TMD is crucial. The symposium addressed this gap by means of highlighting innovations and implementation strategies to help clinicians prepare for the technological changes reshaping oral health care.</div></div><div><h3>Types of Studies Reviewed</h3><div>The authors summarized presentations and panel discussions from the symposium, encompassing developments in AI-assisted cariology, maxillofacial radiology, diagnostic and presurgical planning, clinical workflows, remote monitoring, and the ethical and regulatory landscape. The review is based on content submitted by the symposium’s presenters, reflecting their research, operational experience, and emerging innovations.</div></div><div><h3>Results</h3><div>The presentations established that AI technologies can standardize diagnostics, reduce interoperator variability, and enhance clinical decision making through predictive analytics, digital simulations, and multimodal data integration. AI applications exhibited high specificity in detecting pathologies, improved early intervention capabilities, and optimized treatment planning.
