The secondary objective was to study the clinical profile of patients with CSU.\u0000\u0000\u0000\u0000This was a hospital-based analytical cross-sectional study comparing the serum lipid levels in 45 patients with CSU with that of age- and gender-matched normal subjects who attended a tertiary care center during a period of 1 year (January 2020 to December 2020). A structured questionnaire was used to record history and examination findings. Lipid levels were estimated by collecting blood samples after 12 hours of fasting. The data were analyzed by independent sample t-test to compare the mean values between the different groups and Chi-square test for comparing proportion. P ≤ 0.05 was considered statistically significant.\u0000\u0000\u0000\u0000Most of the study participants belonged to the age group of 21–30 years (mean 29.3 years, standard deviation 8.4 years). The male-to-female ratio was 1: 2.2. The majority of the patients in this study (27, 60%) were either overweight or obese. There was a statistically significant elevation in mean serum triglyceride level (TG) in CSU patients in comparison to normal controls (P = 0.008). Patients with a disease duration of more than 6 months had a significantly higher levels of non-high-density lipoprotein-cholesterol (non-HDL-C) (P = 0.026) and remnant cholesterol (P = 0.038), and significantly lower levels of mean low-density lipoproteincholesterol (LDL-C) (P = 0.027) and mean LDL/HDL (P = 0.026) in comparison to patients with a disease duration of 6 months or less.\u0000\u0000\u0000\u0000Small sample size and the cross sectional design were the main limitations.\u0000\u0000\u0000\u0000Patients with CSU showed significantly higher levels of serum TG in comparison to controls. Patients with a disease duration of more than 6 months showed a significant increase in mean remnant cholesterol and non-HDL-C levels, and a significant decrease in LDL-C levels and LDL-C/HDL-C, when compared to those with a disease duration of 6 months or less.\u0000

We report a 72-year-old female who presented with erythematous papulopustular lesions over both upper and lower eyelids and a few similar lesions on the cheeks of 2 weeks duration with one similar episode in the past. A cyanoacrylate standardized skin surface biopsy showed increased mite density and the patient was successfully treated with acaricides. Demodicosis is often misdiagnosed as contact dermatitis, papulopustular rosacea, or seborrheic dermatitis. A high index of suspicion of demodicosis is needed to arrive at an accurate diagnosis.

The procedure involves identifying an intact blister, deroofing it, scraping the erosion as well as the undersurface of the blister, transferring the material on a glass slide, staining the slide appropriately, and viewing under the microscope. The characteristic cells can be identified under the microscope that can aid the diagnosis.

IADVL Kerala is not just the first branch. It also has several other firsts to its credit. The first annual conference of the association under the banner of the IADVL at Thiruvananthapuram in 1975 and the first South-Zone Regional Conference in 1990 at Kochi are some of the firsts. The state branch has always been at the forefront of all academic and IADVL activities. The branch undertakes community service projects and delivers information through print, radio, and visual media as a regular feature to increase patient and public awareness. Dermatology Clubs and Societies in different towns and cities are not only active academically, but are also socially committed. The continuing medical education series paved the way for the birth of the “Kerala Journal of Dermatology, Venereology, and Leprology” which metamorphosed into the highly acclaimed “Journal of Skin and Sexually Transmitted Diseases”, a great achievement indeed, destined to catch attention worldwide. The Kerala IADVL has had a glorious past. Kerala has produced some of the finest dermatologists and the tradition continues. We have as our wealth, a committed younger generation that has already made its presence felt in the national and international scenario. Since the inception of the Kerala branch, the committed office bearers who struggled selflessly with single-pointed devotion have been our asset. Activity levels have steadily soared to unimaginable heights.

It has utility in a wide array of dermatological conditions, tele-dermatopathology, and dermatological research. The image contrast is imparted by the differential refractive properties of subcellular structures. Certain stains are also used to accentuate the image contrast. CFM enables the visualization of tissues both in vivo and ex vivo (after excision). Both reflectance (in vivo and ex vivo) and fluorescence modes (ex vivo) of CFM are utilized for imaging. One can view 3D images with a near histological resolution, but with a limited depth of penetration. CFM has reduced the number of biopsies required in the evaluation of skin cancers. However, conventional histopathology remains the gold standard in tumor diagnosis. The in vivo reflectance mode CFM has found applications in dermato-oncology, inflammatory dermatoses, cutaneous infections and infestations, skin aging, and pigmentary disorders. The ex vivo CFM facilitates the immediate perioperative examination of excised tissues. The limited depth of view, photobleaching (in fluorescence mode CFM), high cost, and steep learning curve are the disadvantages. The future of CFM appears promising with the introduction of modified microscopes, the use of specific stains, and the incorporation of artificial intelligence.
