This study aimed to investigate the difference in stress levels among women in North Macedonia, both before and during the COVID-19 pandemic. A cross-sectional study was conducted using a questionnaire, which incorporated the Perceived Stress Scale (PSS) to assess stress levels pre- and mid-pandemic. More than 500 women aged 18 to 45, who sought examinations at the University Gynaecology and Obstetrics Clinic from March 2022 to March 2023, were invited to participate; 206 women accepted the invitation. The findings indicated that the participants' stress level scores were significantly higher post-March 2020 compared to pre-March 2020 (Z = 8.13, p < 0.001). These results suggest that the COVID-19 pandemic has substantially affected the stress levels experienced by women in North Macedonia. This is aligned with previous studies indicating increased stress levels during the pandemic, primarily attributed to health concerns, financial difficulties, and social isolation. This study provides substantial evidence of the significant impact of the COVID-19 pandemic on women's stress levels in North Macedonia. Future research should focus on identifying factors contributing to heightened stress levels among women during the pandemic. This will help formulate effective interventions to counteract the negative impact of the pandemic on this population's mental health. This makes NSCLC in most of the cases inoperable at the exactmoment of diagnosing, which leads to a necessity of using a chemotherapy that has the purpose ofnarrowing the extended masses in the lungs and/or near/further metastases. Alectinib is a tyrosine-kinase inhibitor (TKI) that is currently being used as a first line target therapy in treating theinoperable ALK rearranged NSCLC, but still the effectiveness of the treatment is not definitely knownand examined. Herein, we report a case of a 51-year-old male patient presented to our hospital withhemoptysis for 2 weeks. Contrast-enhanced computerized tomography (CT) of the chest showed anapproximately 28x23 mm soft tissue mass infiltrating into the lumen of the right bronchus and causingobstruction. On the same side at the base, a hypodense nodule of 19 mm with some surroundingpneumonic reaction and irregular contours was detected. Several significant lymph nodes were detectedin the hilar and mediastinal regions. Transobronchial biopsy of the mass showed pulmonaryadenocarcinoma and immunohistochemical testing results confirmed ALK rearrangements. TKIalectinib was given at a dosage of 600mg twice per day for 13 cycles, achieving a complete response ofthe disease with complete regression of the mass in the right bronchus, complete regression of the rightnodule and hilar and mediastinal lymph nodes were not detected after treatment . The patientcontinued to receive alectinib and did not report any specific discomfort at his 13 month follow-up. The development of the technique for minimally invasive percutaneous plate osteosynthesis (MIPPO) is a well-accepted step towards this direction. In our study we explore the benefits and shortcomings of two different plate types used with this technique: the 3.5 mm anatomically pre-shaped titanium plate, and the 4.5 mm LC-DCP. The study was a prospective – retrospective clinical intervention case control study of 100 patients with closed, unstable, extraarticular and partially articular fractures of the distal tibia and fibula, classified as AO type 43A1, 43A2 and 43А3. Patients were divided in two groups of 50 patients. Group A were patients prospectively treated with a 3.5 mm pre-shaped plate, and Group B were patients with retrospectively collected data treated with a 4.5 mm LC-DCP. Patietns age was 38.15 years with 68 male and 32 female patients. Operation duration in Group A was 57.14 ± 8.30 and 66.67 ± 5.55, in Group B which was statistically significant. Partial and full weight bearing, as well as time to union in both groups was achieved within a similar timeframe. Functional according to Johner and Wruh's criteria and showed that most patients in both groups (61) had excellent results (Group A: 32, Group B: 29) and 21 had good results (Group A: 10, Group B:11). Rate of complications were comparable between the two groups, in regard to superficial infection, deep infection and ankle stiffness. Only complication parameter which showed a significant difference between the two groups was rate of implant irritation, which was higher in the 4.5 mm LC-DCP group. Both 3.5 mm and 4.5 mm plates are reliable when used with a MIPPO technique. This method is minimally invasive, preserves the periosteal blood supply and ensures optimal conditions for biological repair of distal tibia fractures, regardless of the type of plate used. The aim of this study is to determine the incidence, obstetric risk factors, maternal mortality and morbidity and perinatal outcomes in women presented with placenta previa. A total of 100 pregnant women with placenta previa were analyzed from October 2021 to December 2022. year. After applying the criteria for inclusion and exclusion, these women were analyzed in terms of their age, parity, gestational age and clinical characteristics in presentation, history of warning bleeding, duration of hospitalization, need for blood transfusion, period of pregnancy in childbirth and pathway of childbirth. A total of 90% of the births were complicated by Placenta previa, including 30.0% of women over 30 years of age and 80% were multipara. 60.4% had large-scale placenta previa, 33% had previous birth by caesarean section, 6.4% had a previous abortion, 12% of the cases had postpartum bleeding and 3.7% had adherent placenta. From the standard methods of the International Federation for Clinical Chemistry (IFCC), we used the kinetic method for the determination of alanine aminopeptidase (microsomal AAP), γ-glutamyl transferase ((γ-G) and MEIA (Microparticles Enzyme Immunoassay (Abbott A x sym system)) for determination of β2microglobulin in the urine. We examined samples (serum and urine) from 70 participants (35 Psa untreated, 35 healthy control group). RF and CRP were determined with the latex agglutination test in the same participants. Of the 35 examined patients with Psa, 12 (34.28%) showed presence of APP enzymuria, 8 patients (22.85%) showed presence of γ-GT, and a low percentage (0% showed presence of β2-microglobulin in urine. Also, in two thirds of MM cases, the presence of Bence Jones proteins is observed. The aim of this study was to demonstrate the role that chromosomal abnormalities and Bence-Jones proteins play as crucial biomarkers in MM patients. Materials and Methods: This retrospective study was carried out at the University Clinic for Hematology in Skopje, North Macedonia, between January 2009 and December 2019. MM patients were divided into different treatment groups: those younger than 65 years old, without comorbidities, and eligible for autologous peripheral blood stem cells (PBSCT) were included in the Cyclophosphamide-Thalidomide-Dexamethasone (CyThalDex) protocol group. The Melphalan Prednisone Thalidomide (MPT) protocol was used in patients over the age of 65 who were unsuitable for aggressive treatment options such as PBSCT due to comorbidities and renal failure. The third group's treatment did not include new immunomodulators like thalidomide. Results: Molecular and chromosomal analyses were performed in 46 MM patients. The survival time of patients with MM concerning molecular and chromosome stratification showed that 20% of them were high-risk [hypodiploid (gain1q, loss1p) Del17p, Del13q, t(11;14) t(4;14) and multiple mutations] who survived 60 months, and the median survival time in these patients was 20.8 months. In patients with MM who had a standard risk, death outcomes were not registered during the observation period. Taking into account all MM patients included in our study, Bence Jones proteins in the urine were present in 35.8% of patients, while their presence was not observed in 64.2%. The percentage difference was statistically significant. In addition to the parenchyma and interstitium, the mediastinal lymph nodes are also very often affected. To analyze the distribution and characteristics of interstitial lung lesions and the involvement of mediastinal lymph nodes in pulmonary sarcoidosis by the method of high-resolution computed tomography. 15 patients diagnosed with pulmonary sarcoidosis were included in the study. Computed tomography with high resolution was made on 128 slice CT scanner PHILIPS INCISIVE, using 1 mm thin-slice thickness and high spatial frequencies algorithm for image reconstruction. Lymph nodes are classified as hilar and mediastinal with a maximum diameter of short axis of more than 10 mm taken as their enlargement. Pulmonary changes are classified as nodules, reticular opacities, fibrous lesions, ground glass opacities and consolidations. The predominant distribution of lesions in the upper and middle zones of the lungs compared to the lower zones was noted. The disease is graded in 5 stages with the Scadding classification. 15 cases of patients diagnosed with sarcoidosis were analyzed all of whom are women in the age group of 30-60 years old. Two patient are in stage I and three are in III stage of the disease, 6 patients are in stage II of sarcoidosis and 4 are in stage IV of the disease. Dry cough as a symptom predominates in all patients, while dyspnea is graded according to the mMRC scale. Mediastinal lymphadenopathy with and without calcifications was present in 11 patients. The type of lung changes as well as their distribution are presented in graphs.HRCT is the method of choice in the evaluation of pathological changes in pulmonary sarcoidosis. It very precisely shows us the characteristic appearance of nodules and lesions, their distribution and atypical changes and helps us in grading the disease and its treatment. We present a case of native valve endocarditis in a 51-year-old male with transplanted kidney that had atypical clinical course. The patient experienced prolonged subfebrile temperature with paroxysmal arrhythmia and development of cardio-pulmonary insufficiency. Transthoracic echocardiography (TTE) set the diagnosis of aortic valve vegetation with severe aortic regurgitation and pulmonary edema. We failed to isolate a microbiological agent, but all blood cultures were taken under antibiotic therapy. The patient was treated with surgical replacement of the native aortic valve with mechanical heart valve with significant clinical improvement. Ten days after the intervention, he was discharged with reduced markers of inflammation and proper function of the kidney graft. Immunosuppressive therapy was gradually reinstated. One year later, the patient was clinically stable and with proper graft function. Early diagnostic and therapeutic intervention, particularly intensive antibiotic therapy and surgical management can preserve the patient and the kidney allograft.
