This information is used to individualize dosage so that drug concentrations can be maintained within a target range and to make dose adjustments. Amikacin is a kanamycin-derived semisynthetic aminoglycoside antibiotic and one of the important antimicrobial agent against Gram-negative pathogens. Aminoglycoside antibiotics have a small therapeutic index and therapeutic range due to monitoring of amikacin concentrations is necessary. For optimal antimicrobial effect of amikacin therapy and prevent toxicity, it is recommended monitoring of amikacin concentrations during therapy with this drug. The study for evaluation of safety of the therapy with amikacin intravenous infusion included 12 patients (7 male and 5 female) with cystic fibrosis. Amikacin was infused over 1 h in the dose of 30 mg/kg (maximum 1.5 g) one time daily during 10-14 days. Blood samples for determination concentrations of amikacin were obtained at the following times: immediately before administration of third dose of amikacin, at the end of infusion, 30 min. and 6 h after infusion. The concentrations of amikacin in the sample before administration of third dose of amikacin was <2 µg/ml in all treated patients. Safety of the therapy was evaluated from the results of laboratory analyses, creatinine clearance (estimated according to the Cockroft-Gault equation), anamnestic symptoms of ototoxicity or vestibular toxicity, evaluation of the whisper test and other adverse events. From the results of our study, we suggest that the dose of 30 mg/kg once daily amikacin infusion administered for 10-14 days is safety in patients with cystic fibrosis. However, the incidence of inguinodynia still present significant complication. Material and methods: The study was designed as a randomized, prospective, unilaterally blind clinical study. Forty male patients were included, to whom hernioplasty by Lichtenstein method with implantation of polypropylene mesh was performed. Patients were divided into 2 groups of twenty patients each. In the first one the technique of the ilioinguinal preservation was used, and in the second one dissection of the ilioinguinal nerve was performed. At appropriate time points two types of scales were used to assess the intensity of the pain: Numeric scale of pain (NSP) and Stanford pain scale (SPS). Results: Data from 40 patients has been analyzed, of which 20 with preservation and 20 with dissection of the ilioinguinal nerve. Inguinodynia was present in two patients, one in each group, i.e. 5%, which indicates that there was no significant difference in the occurrence of inguinodynia in the group with preservation and dissection of the ilioinguinal nerve. However, their increased production to an excessive amount can cause alterations in the cellular redox status with consecutive disruption of various normal biological functions. Oxidative stress (OS) occurs when there is an imbalance between ROS production and antioxidant defence mechanisms. Chronic OS results in many DNA modifications, and alterations in DNA repair, leading to DNA lesions of which many can be toxic and/or mutagenic. In patients at increased risk for bleeding a heparin-free regime is mandatory. Evodial dialyzer contains a heparin-grafted membrane in order to reduce patients' bleeding risk. In this study we present our experience with the use of Evodial dialyzer. We report on 106 dialysis sessions performed in 59 patients. Reasons for using Evodial: active bleeding, hematological conditions, complications of vascular access. Changes in the dialyzer or additional interventions were examined. Low-dose unfractionated heparin was used in 10 (9.4%) sessions, and was added in 6 more and in another 5 saline flushes. In 4 sessions, due to coagulation we had to terminate dialysis. The overall incidence of persistent trigeminal artery is between 0.2 to 0.76%. PTA are known to be associated with a wide range of pathology. The aim of this study was to describe the morphological characteristics of the PTA and to emphasize its clinical significance. We examined radiographs from patients who had CT angiography undertaken for a variety of clinical reasons, performed as a part of their medical treatment at the University Institute for Radiology in Skopje, RN Macedonia. The study population included 234 patients, 130 male and 104 females, mean age 57.8 years. In one patient we found PTA with overall incidence of 0.42%. CTA revealed a left PTA that arise from the internal carotid artery and communicate with the basilar artery between the origin of the anterior inferior cerebellar artery and the superior cerebellar artery. Although anatomically interesting, an awareness of the anatomy and variations of the brain arteries is clinically important for radiologists and surgeons for save performance of procedures, and forensic pathologists since variants may have forensic consequences. The highest incidence is in the countries of the so-called “silk road”. In Macedonia, the frequency of this disease is currently unknown, but it is often under-diagnosed and unrecognized. In addition to the others, one of the main manifestations is ocular, which is presented by involvement of any of the structures of the uveal tract, the retina and the optic nerve. The diagnosis is established on the basis of clinical parameters, complemented by genetic analyses-HLA-B51 typing and Pathergy positive test and ophthalmic imaging-techniques such as OCT, OCT-A, FFA. Treatment of the condition is multidisciplinary with local and systemic corticosteroid therapy, conventional immunosuppressive drugs, and biological therapy. The aim of this paper is to present our experience in diagnosis and therapeutic modalities in patients with this disease.
