The share of internally displaced persons (IDPs) has increased as well. Under the infl uence of stressful situations, they develop exacerbation of chronic diseases or occurrence of acute ones requiring rehabilitation measures. With severe combined injuries, restoration of lost or disturbed functions of the victims requires a specialized approach for rehabilitation, expensive equipment, availability of rehabilitation specialists, and current rehabilitation programs and protocols. The Ministry of Health of Ukraine carries out direct management of rehabilitation activities. It provides legal regulation on rehabilitation in health care, develops and approves legal acts, approves and implements protocol on giving rehabilitation aid in Ukraine. The Ministry monitors and evaluates the quality of rehabilitation performance at the rehabilitation institutions, participates in ensuring quality control over rehabilitation technical means, provides organization of rehabilitation aid at health care institutions, participates in international cooperation, ensures cooperation with rehabilitation institutions abroad. Its purpose is establishment and development an eff ective rehabilitation system in Ukraine, implementation of international protocols on giving rehabilitation aid, realization of rights of individuals with limited everyday functioning. The National Health Service in Ukraine (NHSU) has become a central executive body in the rehabilitation area. The NHSU implements the state policy in the fi eld of medical care of the population ensured by the program of medical guarantees. Losieva","doi":"10.24061/2413-4260.xiv.2.52.2024.21","DOIUrl":"https://doi.org/10.24061/2413-4260.xiv.2.52.2024.21","url":null,"abstract":"The article presents the results of a review of the literature on the problem and attempts to give an assessment of the existing forecasting methods to describe the problem fi eld of the system of physical rehabilitation of people with dysfunction of the pelvic organs as a result of spinal cord injury. The research used theoretical analysis and synthesis, comparison of methods, documentary method. It was revealed that the main areas of research in the fi eld of rehabilitation of people with spinal cord injuries include: rehabilitation in acute injuries, medical rehabilitation, rehabilitation interventions during spa treatment, rehabilitation process and implementation of new approaches to it. The insuffi ciently improved system of rehabilitation of people with pelvic organs dysfunction as a result of spinal cord injuries at remote stages of treatment requires search and introduction of modern, mobile, accessible and eff ective forms and programs of active rehabilitation. Effi ciency of rehabilitation depends on rational coordination of many specialists. It actualizes the issue of work of multidisciplinary teams in medical institutions providing both primary care of such patients and their support throughout the rehabilitation process. It is emphasized that the process of rehabilitation and social adaptation of patients with pelvic organs dysfunction as a result of spinal cord injuries should include appropriate rehabilitation measures and technologies that would adapt the patient to everyday, domestic and professional conditions by improving the quality of life. It is proved that this problem is still relevant and the issue of integrated use of rehabilitation measures in the process of rehabilitation treatment requires new scientifi c developments that will help to reduce the number of negative consequences of the disease that aff ect the quality of life of patients. The problem of hypertension is also relevant during pregnancy, as high blood pressure can be dangerous for both the mother and the fetus, causing pre-eclampsia and premature birth. According to recent data, the number of patients with hypertension will increase. Because of the polygenic and multifactorial nature of the therapeutic response to drugs, further research in this area is needed to provide evidence- based guidelines for clinicians to optimize antihypertensive therapy. The purpose of this review was to summarize information from scientifi c publications, meta-analyses, guidelines for theyears 2018-2023 regarding variants in genes that aff ect the metabolism of diff erent classes of drugs used in the treatment of hypertension, including during pregnancy, and related to the development of AH. The pathogenesis of hypertension is based on both a decrease in vasodilatation and an increase in circulating blood volume. Arterial stiff ness leads to a decrease in vasodilation, and water and sodium retention leads to an increase in blood volume.Additional factors such as the renin- angiotensin-aldosterone system, the sympathetic nervous system, and gene variants affect both vasodilation and blood volume. In addition, there are complex interactions among these factors. As an innate factor, gene variants can aff ect all of the above simultaneously.The American Heart Association (AHA) and European Society of Cardiology (ESC) guidelines recommend the use of medications from the following 5 classes: diuretics, calcium channel blockers (CCBs), beta-adrenergic receptor blockers (betablockers), angiotensin- converting enzyme inhibitors, and angiotensin II receptor blockers. The studies included in this review used two main approaches: candidate gene analysis and genome-wide association analysis. The polygenic nature of hypertension greatly complicates the search for clinically relevant variants and relationships between individual genes and response to medications used to treat hypertension in diff erent ethnic groups. Candidate genes that may infl uence the risk of hypertension include voltage- dependent calcium channel genes (CACNA1A, CACNA1C, CACNA1S, and CACNB2), NEDD4L, ADD1, and miR. A number of genetic polymorphisms are associated with both the infl uence on the development of arterial hypertension and the response to treatment – eNOS, TRIB3, CYP, POR, ADRB1, ADRB2, ACE. When treating pregnant women with hypertension, the effi cacy of the antihypertensive agent must be balanced against the risk to the fetus. Initial antihypertensive therapy should include an acceptable fi rst-line agent. In turn, in order to reproduce the events preceded death from TBI, investigative bodies need to know as precisely as possible the time of hemorrhage formation, which can often be close to the moment of death. Therefore, scientists in the fi eld of forensic medicine are working on the development of express methods that would allow to geta quick and objective answer to the question of establishing the age of the formation of a hemorrhage in the substance of the human brain. The purpose and tasks of the research. To develop, within the framework of the statistical analysis of optical anisotropy maps, universal forensic medical criteria for the determination of time of haemorrages formation due to TBI, cerebral infarction of ischemic and hemorrhagic genesis by the method of diff use tomography of the polycrystalline component of native histologicalsections of the brain with algorithmic reproduction of fl uctuations in the value of linear birefringence.Research materials and methods. Native slices of human brain from the parietal area were taken for the study from the dead with a known time of hemorrhage formation from 1 to 7 days, according to medical documents. The cause of death was traumatic hemorrhage – II group (n=100), ischemic cerebral infarction – III group (n=110), non-traumatic hemorrhage – IV group (n=105), acute coronary insuffi ciency – I group – control (n=20). Soleiko","doi":"10.24061/2413-4260.xiv.2.52.2024.7","DOIUrl":"https://doi.org/10.24061/2413-4260.xiv.2.52.2024.7","url":null,"abstract":"Aim of the study – To improve the treatment tactics of perianal CK in children with the aim of improving the treatment results by using a multidisciplinary combined approach with the use of minimally invasive surgical techniques and conservative therapy. Materials and methods. The results of the diagnosis, treatment, and follow-up of 12 pediatric patients with perianal CK were analyzed. Complaints, anamnestic data were collected for all patients. Laboratory, physical and necessary instrumental examinations and morphologic study of samples of clinical material (tissues of removed fi stula ducts, polyps, rectal crypts, condylomas, etc.) were performed. The leukocyte index of intoxication (LII) and the hematological index of intoxication (HPI) were calculated for each of the studied group of hospitalized patients. The work uses generally accepted methods of statistical processing of medical and biological research data.The results. In the studied group, 66.7 % were boys, 33.3 % were girls. In 25 % of patients HC was diagnosed at the age of 2-12 years, in 75 % – at the age of 13-15 years. On hospitalization all patients had complaints and local symptoms characteristic for perianal HC of the corresponding localization, were in severe or moderate condition with signs of intoxication. 75 % of patients have a wide range of extraintestinal manifestations of CK. The mean values of LII and GPI indicators were 4.26 and 6.82, respectively. In all patients the presence of anemia, leukocytosis, increased ESR, level of C-reactive protein, fecal calprotectin, hypoalbuminemia, hyperglobulinemia (IgG), intestinal dysbiosis with Candida albicans colonization were determined. The necessary physical and instrumental examinations to determine the anatomy of the perianal lesions were performed under anesthesia. Drainage of open purulent foci and fi stulae was performed with permanent draining ligature, which was removed after 3-6 months in the absence of signs of infl ammation and determination ofhealing of the aff ected area. Determination and correction of specifi c and anti-relapse therapy for CK, anemia, laboratory- confirmed infl ammatory changes, dietary nutrition was carried out jointly with a gastroenterologist, a hematologist and an immunologist. A geneticist was consulted for all patients.Conclusions.1. The majority of cases of perianal HC in children occur during adolescence – 75 % of patients in the study group were 13-15 years old.2. The diagnostic search and treatment process of perianal HC in children requires a multidisciplinary approach with the participation of a gastroenterologist, a pediatric surgeon, a hematologist, a geneticist, an immunologist, a morphologist, etc. Supervision of the patient by the specifi ed specialists should be carried out both during inpatient and outpatient treatment with the aim of timely correction of the manifestations of HC.3. Aim of the study is to present the results of a literature search and demonstrate a clinical observation of Williams- Beuren syndrome in 10-month-old monochorionic twins with congenital malformations of the cardiovascular system in combination with kidney pathology and an additional spleen. Results. Williams- Beuren syndrome (WBS) is a rare congenital disorder characterized by specifi c craniofacial dysmorphisms (elphic face) and a hoarse voice in combination with cardiovascular damage, mental retardation, musculoskeletal disorders, and hypercalcemia. WBS occurs in the population with a frequency of 1:7,500-1000 infants. The presence of a specifi c phenotype is associated with a hemizygous microdeletion of the long arm of chromosome 7 at region 7q11.23. The size of the deletion varies from 1.5 to 1.8 Mb and results in the loss of several neighboring genes. The diagnosis is made syndromologically and confi rmed by modern molecular cytogenetic methods. Pathologically signifi cant WBS mutations include loss of the ELN gene and loss of neighboring genes such as LIMK1, RFC2, BAZ1B, GTF2I, STX1A, CLIP2, GTF2IRD, NCF.Haploinsuffi ciency of ELN gene is the main marker of WBS and causes insuffi cient synthesis of elastin protein, which leads to development of pathology of heart and blood vessels (elastin arteriopathy), disorders of connective apparatus of joints, abnormalities of vocal cords and skin. LIMK1 hemizygosity is associated with impaired visual- spatial constructive cognition. Deletion of the RFC2 gene can cause growth retardation and developmental delay. Reduced intelligence can be caused by a mutation of the GTF2I gene and hypercalcemia by a mutation of the BAZ1B gene. The phenotypic manifestations of WBS arealso thought to be infl uenced by the reduced expression of fl anking intact genes. The diagnosis, treatment, and adjustment of patients with WBS require an interdisciplinary team of specialists. The presented clinical case demonstrates multisystem pathology in 10-month-old monochorionic dizygotic twins in whom Williams- Beuren syndrome was clinically diagnosed and confi rmed by FISH: ish del (7)(q11.23q11.23)(ELN-). Conclusion. To confi rm the genetic component in congenital multisystem pathology, it is necessary to use modern molecular genetic diagnostic methods. Determination of genetic mutation, its size and origin is important for medical genetic counseling. The methods of system approach, bibliosemantic, comparative analysis and statistical data processingwere applied.The results of the study: A comprehensive study of the impact of the factors of the full-scale war of the Russian Federation against Ukraine, conducted at the population level, proved their negative impact on the frequency of low birth weight newborns from 5,99 % in 2021 to 6,09 % in 2023, OR with 95 % CI 1,04 (1,01-1,07), p<0,0012, the number of born sick and sick newborns from 196,93 to 269,47 ‰, OR with 95 % CI 1. 48 (1,46-1,50), p<0,0001 against the background of rapid decrease in the number of newborns (31,5 % loss rate) and preservation of suffi cient resources for neonatology service. According to the data of geospatial analysis, a greater infl uence of negative consequences of military aggression was found in the regions with active hostilities and temporary occupation (I group) compared to the rear territories (II group) according to the frequency of low birth weight newborns – 6, 2 % in the I group and 6.05 % in the II group, OR with 95 % CI 1.16 (1.1-1.2), p<0.0001, and the frequency of sick and diseased newborns – 311.8 and 227.9 ‰, OR with 95 % CI 1.53 (1.49-1.57), p<0.0001. A violation of the logistics of medical care for newborns in the regions of the I group in comparison with the regions of the II group is shown, in particular, according to the data on the indicators of mortality (1,7 and 2,51 ‰, the rate of loss 30,68 %), lethality (0,55 and 1,1 %, the rate of loss 50,0 %) of newborns and early neonatal mortality (2,84 and 3,1 ‰, the rate of loss 8,38 %).Conclusions. The microbiota of preterm infants diff ers from that of term ones. This is because preterm infants and their mothers have more complicated prenatal and postnatal health conditions and anatomically- functional immaturity of organs and systems depending on gestational age. Maternal conditions, antibiotics, type of feeding, and use of probiotics can signifi cantly aff ect the gut microbiota of preterm infants in the early neonatal period; however, these eff ects decrease with age. Although some factors and processes are diffi cult to intervene or avoid, understanding the potential factors and determinants will help to develop timely strategies to promote a healthy gut microbiota in preterm infants. Premature birth under hypoxia causes signifi cant metabolic disorders in the body, is accompanied by a high risk of severe organ dysfunction, which requires in-depth research to clarify the features of intracellular metabolism in order to further improve methods of diagnosis and treatment of newborns with various forms of perinatal pathology.Aim of the research. To study the trends of changes in energy metabolism in premature infants with severe forms of perinatal pathology to investigate the possibility of using them as additional criteria of hypoxic damage to the body in conditions of morphological and functional immaturity.Materials and methods. Clinical and laboratory examinations were performed on 68 newborns with severe forms of perinatal pathology at the gestational age of 32 to 34 weeks; the comparison group consisted of 27 conditionally healthy newborns at the gestational age of 34 to 37 weeks. Inclusion criteria for the main study group: gestational age at birth from 32 to 34 weeks, clinical signs of severe perinatal pathology. Exclusion criteria: gestational age at birth < 32 and ≥ 37 weeks, diagnosed congenital malformations and septic conditions. The list of diseases was determined according to the International Classifi cation of Diseases, X Revision. The following blood serum laboratory parameters were studied: lactate level, glycerol-3-phosphate dehydrogenase (GPDH) (EC 1.1.99.5), succinate dehydrogenase (SDH), NADH dehydrogenase (NADH) (EC 1.6.5.3); the electron transport chain coeffi cient (ETC) was calculated. Laboratory studies were performed using micromethods. The study was conducted in accordance with the provisions of GCP (1996), the Convention of the Council of Europe on Human Rights and Biomedicine (April 4, 1997), the Declaration of Helsinki of the World Medical Association (1964-2008), Order of the Ministry of Health of Ukraine No. 690 dated September 23, 2009 (as amended by Order of the Ministry of Health of Ukraine No. 523 dated July 12, 2012). Protocol of scientifi c research of the Biomedical Ethics Committee of BSMU dated September 12, 2015. Informed written consent was obtained from the parents of the patients before the study with an explanationof the purpose, objectives and methods of laboratory testing. Statistical processing of the results was performed using STATISTICA software (StatSoft Inc., USA, version 10). Comparison of quantitative indicators with normal distribution was performed using Student’s t-test, and the probability of diff erences was considered statistically signifi cant at p<0.05. Thanks to the development of the latest technologies and methods, this branch of medical genetics and clinical pharmacology is developing very actively. Data are being accumulated, special databases are being created with the aim of creating individual genetic passports in the future, which will allow the selection of personalized treatment schemes.Anesthesiology is a special area of pharmacogenetic research because, more than any other medical specialty, it ischaracterized by polypharmacy-the simultaneous or sequential administration of many drugs. The same dose of a drug may be inadequate for some patients and may be life-threatening or cause unwanted side eff ects for others. Today, information about genetic factors is being used by clinicians to prescribe drugs to tailor drug therapy to a patient’s genome. In anesthesiology, the principles of pharmacogenetics have been explained for neuromuscular blocking agents, opioid metabolism, different types of anesthetics, and postoperative nausea and vomiting. On the other hand, a large number of anesthetics have a narrow therapeutic index.This review summarizes the most recent data from the scientifi c literature on the pharmacogenetics of diff erent types ofanesthetics. Inhalational anesthetics are halogenated derivatives of methyl ethyl ether, the exact mechanism of action of which is not yet fully understood. One of the rare but very serious side eff ects of all halogenated anesthetics is malignant hyperthermia, a genetically determined autosomal dominant disorder that manifests as a hypermetabolic response to drug administration. The dosage of intravenous anesthetics should also be carefully determined, taking into account the patient’s age, cardiovascular, hepatic, and renal status, concomitant drug therapy, and genetic factors. Ontogeny and genetic variability of drug-metabolizing enzymes are interrelated because genetic variability in drug-metabolizing enzyme expression cannot be assessed until the required protein is suffi ciently expressed. Pharmacogenetic variants may contribute to unpredictable drug exposure at the same weight- based drug dose.There are a number of potentially clinically applicable pharmacogenetic data in newborns, but more research is needed toconfi rm these fi ndings and understand how to incorporate them into clinical care.The selection of drugs and dosing regimens based on a patient’s pharmacogenomic profi le may be an important part of thefuture of medicine. Personalized treatment based on the specifi c variants in the genome will ultimately reduce the incidence of side eff ects and length of hospital stay for patients and save healthcare costs.
