The external and internal pathway of thrombus formation proceeds with the participation of coagulation factors. This process involves the formation of fibrin, a protein that forms the basis of a blood clot. Fibrin as a scaffold gives the thrombus strength. With coagulopathy, due to a lack of coagulation factors, fibrin formation does not occur. Without fibrin, a thrombus is not able to reliably anchor at the site of injury. Uncontrolled bleeding occurs, which can lead to death.\u0000The purpose of this review was to assess the frequency, clinical presentation, genetic basis, possibilities and difficulties in diagnosing these deficiencies. Rare blood clotting disorders (RBCD) include monogenic coagulopathies caused by a deficiency of plasma proteins involved in hemostasis, not related to von Willebrand disease and hemophilia A or B. RBCD include hereditary deficiencies or abnormalities of fibrinogen, prothrombin (factor II), coagulation factors V, VII, X, XI, XII, XIII. All these violations in the overwhelming majority of cases lead to violations of fibrin formation. The reasons for the development of RBCD are, as a rule, recessive inheritance of unique or rare nucleotide changes in genes encoding coagulation factors, or in proteins required for post-translational modifications of these factors. There is neither any history for allergy nor a family history of hereditary allergy. She was on metformin with vildagliptin and a combination of bisoprolol and hydrochlorothiazide for a considerable length of time. In March 2021, she admitted with manifestations of dry cough, fever, with rapid onset of deterioration. After an itemized examination including chest CT and testing for Coronavirus, she was diagnosed with SARS-CoV-2 infection. Days after, her state became worsened. She was treated with Ceftriaxone injection, Gemifloxacin, Rivaroxaban, and salbutamol. She goes forward adequately, with 4 days of fever and dry cough, followed by 5 days of anosmia and ageusia from which she wholly improved. On day +13 of disease progression, afterward resolution of the previous symptoms, quickly diffusion Pinkish urticarial plaques developed in numerous positions on her body: face, trunk, abdomen, and limbs (Figure. 1). She has not received any non-steroidal anti-inflammatory drugs or ACE inhibitors. As a symptomatic treatment, only an antihistaminic drug was taken Cetirizine 10 mg tablet twice a day. The cutaneous manifestation was partially controlled within forty-eight hours. She well-ordered to keep the guidelines of home isolation concerning SARS-CoV-2. L, Pyary joy","doi":"10.55529/jpdmhd.11.1.6","DOIUrl":"https://doi.org/10.55529/jpdmhd.11.1.6","url":null,"abstract":"Aim: To study the prevalence of Scedosporium in fungal keratitis infection from a rural tertiary care hospital.\u0000Method: Corneal scraping samples from October 2018 to October 2019 had come to Microbiology central laboratory at Narayana nethralaya hospital,Bommasandra Bangalore. 94 samples were collected using a surgical scalpel blade no. 15 and it was subjected to microscopic Gram stain examination and 10%Pottasium hydroxide (KOH) mount and culture techniques. 16 samples on Sabouraud dextrose agar (SDA) started growing after 72 h and these were initially floccose, with a white hairy growth with greyish surface and yellow reverse. No growth was seen on blood agar and chocolate agar after 1 week of incubation. Lacto phenol cotton blue (LPCB) staining was performed after seven days of incubation by tease mount method and observed by binocular compound microscope with low power x10 and high power ×45, which shows septate hyphae, ovoid conidia with truncated bases suggestive of Scedosporium species.\u0000Results: 16 samples on Sabouraud dextrose agar (SDA) started growing after 72 h and these were initially floccose, with a white hairy growth with greyish surface and yellow reverse, suggestive of Scedosporium species.10 samples showed white, floccose, cottony myceleal growth on SDA and LPCB mount shows slightly curved micro and macro conidia accumulating around tips of phialides, suggestive of fusarium species.\u000010 samples revealed phaeoid, brown fungus with hairy growth on SDA and LPCB mount showed septate, slightly tapering curved conidia suggestive of Curvularia species.\u0000Conclusion: majority of Scedosporium growth was observed in our study(44%), compared to Fusarium(27%) and Curvularia(27%) species.
