Chondrosarcoma of the Mandible: A Challenging Lesion 下颌骨软骨肉瘤:一个具有挑战性的病变

Clinical and radiographic findings are non-specific hence histopathological evaluation forms the basis for definitive diagnosis. This is a case of a 27-year-old female with a complaint of pain and swelling in her lower right back tooth region for one month. The swelling caused gross facial disfigurement with no secondary changes. Intraorally the growth involved both buccal and lingual sulci causing mobility of involved teeth. Radiographically severe periodontal bone loss was noted surrounding 47. Incisional biopsy was taken and sent for histopathological evaluation. The histopathological examination revealed lobular pattern of growth filled with sheets of round to polygonal cells showing pleomorphism and bizarre hyperchromatic nuclei along with a typical mitotic figures surrounded by densely packed spindle-shaped cells. In some areas chondroid matrix deposition with lacunae containing atypical chondrocytes was evident. Ki-67 IHC was done which showed 10 positivity. Hence the lesion was diagnosed as Grade I Chondrosarcoma. It must be differentiated from chondroblastic osteosarcoma as it bears close resemblance with it. The treatment is completely different for both entities. Chondrosarcoma is chemo-resistant and radioresistant whereas osteosarcoma is a highly chemosensitive tumor. Methodology Thirty-four teeth having abfraction were randomly divided into two groups n17 per group depending on the restorative material used. Group Andash Microhybrid composite and Group B ndash Activa composite. Initially occlusal contacts were evaluated for the patients and required occlusal adjustments were carried out. Restorations were done according to the manufacturerrsquos guidelines. Evaluation of the restorations was done at baseline 1 month 6 months and 1-year time interval by two blinded examiners as per the modified United States Public Health Service USPHS criteria. The statistical analysis was carried out and the results were tabulated.Results The clinical performance of microhybrid and Activa showed no statistically significant difference at post-op 1 month 6 months and 1 year pgt0.05. In intragroup assessment between baseline and 6 months both groups yielded similar clinical behavior whereas at 1 year a statistically significant difference p0.046 was noted with marginal adaptation in microhybrid group.Conclusion Acceptable clinical performance was shown by both groups by the end of 1 year. However discrepancies were noted in the marginal adaptation criteria for microhybrid. Such ridges require a combination of techniques in placing implants. One such technique is the ridge split technique which helps the expansion of the ridge with or without greenstick fracture of cortical plates. Before placing implants the width of the alveolar ridge can be effectively restored utilising the modified ridge splitting approach employing autologous bone grafts. This case report describes the ridge splitting and placement of a dental implant in the left maxillary central region followed by guided bone regeneration. Ridge expansion was done in relation to 21 and osteotomy was prepared to the desired depth. After proper treatment planning implant measuring 3.5 mm in diameter and 11.5 mm in length was placed. Four months later final cementation was done with PFM crown. The ridge split technique with the immediate implant placement resulted in minimal resorption of alveolar bone along with maximum expansion of the bone. So the present study evaluates the effect of dentin thickness on the pulp in severe chronic periodontitis.Objectivesi To assess the radicular dentin thickness histologically.ii To histologically assess the pulpal changes due to periodontitis.iii To correlate the effect of radicular dentin thickness and periodontitis on dental pulpMethods Single rooted periodontally compromised tooth was atraumatically extracted. Access opening was done using an extracted tooth and the pulp fixed in formalin solution. After sufficient fixation decalcification was done followed by processing and histological examination. The radicular dentin thickness was measured at 3 reference points and the mean value was compared with histological parameters to assess for any changes.Results Results revealed that various histological changes were observed in the pulp. The mean dentin thickness when compared with various histological parameters revealed varying results. Inflammation and edema did not corelate with changes in dentin thickness whereas fibrosis presence of pulp stone pulp vessels and odontoblastic integrity showed a positive correlation but were not statistically significant. The stresses may cause micromovement that jeopardises the healing process in early stages. These stresses aggravate as the implant goes deeper into the bone and somewhat accounts for increased friction and torque. Excess torque at the insertion time may lead to bone compression which if crosses its physiologic limit may lead to a bloodless field i.e. ischemia following necrosis or sequestrum formation which in turn will cause implant failure. This is termed as compression necrosis. Too much torque levels may result in high levels of strain transmitted to adjacent bone. The crestal area around the implant is mainly comprised of cortical bone which has minimum blood supply and is exposed to extreme strain making it more vulnerable to bone necrosis and which is paramount to the failure of implant. Most commonly found among these is horizontal pattern of bone loss which received least attention while attempting regenerative periodontal therapy. Hence the objective of this research was to assess the ability of autologous Platelet Rich Fibrin PRF to promote periodontal regeneration in individuals with chronic periodontitis who have horizontal defects.Methods A total of 20 sites with horizontal bone loss were taken in five patients who had met the inclusion criteria. Informed consent was obtained from the patient. All the clinical as well as radiographic parameters were assessed. Open flap debridement with intramarrow penetration and placement of PRF was done for these patients and were followed up for nine months.Results All the patients were consistent with the follow up till nine months. Intergroup comparison of Probing Pocket Depth PPD and Clinical Attachment Level CAL showed significant difference with p lt0.05. Radiographic assessment also showed significant difference compared to baseline with p lt0.05. It can be used for a variety of treatments ranging from simple procedures like removal of caries to more complex procedures such as soft and hard tissue surgeries. The reasons for its increasing demand in dentistry includes decreased operator time enhanced efficiency and better clinical outcome. In the previous years Light Amplification by Stimulated Emission of Radiation LASER has also been used for haemostasis in which a ldquochar layerrdquo or an ldquoescharrdquo is usually formed. Recently with the advancements in LASER technology and knowledge about LASER the term LASER bandage has been proposed in which the LASER is used in the lowest settings settings of low wattage no water and some air with fewer pulses per second to put a coating on the tissues for haemostasis to decrease pain from an oral lesion or to make small alterations to a soft tissue surgery. This article describes about different restorative techniques in placing composite to reduce polymerization shrinkage.Aims and objectives The purpose of this study was to compare and evaluate microleakage of Class V cavities restored with two different techniques using dye penetration method and to determine better technique for placing composites which can increase the longevity of restoration by minimizing polymerization shrinkage.Methods Thirty extracted human premolar teeth were selected and Class V cavities were prepared. They were then randomly divided into two groups Group A- Snow plow technique and Group B- Control group. Composite restorations were done using two different techniques. All the samples were thermocycled and subjected to cyclic loading. Nail polish varnish was applied were immersed in dye for 24 hours and were sectioned buccolingually into two halves. The sectioned samples were subjected to stereomicroscopic investigation and depth of dye penetration was analysed according to scoring system. Statistical analysis using Statistical Package for Social Science SPSS software was done.Results No statistical variance was found in the microleakage scores between Group A and Group B and between the two techniques followed for restoration. Two google forms were formulated. One for the parents to fill out and the other form for the child. Form 1 to be filled by the parent included four sections - Socio-demographic details Dental anxiety scale CORAHrsquoS revised DAS-R Parenting style and dimension questions PSDQ Childrenrsquos fear survey schedule CFSS-DS. Form 2 to be filled by the child included three sections - Dental anxiety scale CORAHrsquoS revised DAS-R Oral health behaviour perception scale and Childrenrsquos fear survey schedule CFSS-DS.Using the results of the two formsThe proportion of children with dental anxiety was assessed.Comparison of childrenrsquos dental anxiety to those of their parents was done.The correlation between parenting style to a childrsquos dental anxiety was assessed.Childrsquos oral health behaviour was correlated to their anxiety.Socioeconomic status with oral health behaviour was assessed.Childrsquos degree of anxiousness was determined.Results This study investigated the association between childrenrsquos dental fear and anxiety DFA and family-related factors. It showed significant correlation between dental anxiety of children and dental anxiety of parents oral health behaviour of parents fear in parents. Oral health behaviour perception of children was found to be significantly associated with dental anxiety of parents oral health behaviour perception of parents and fear in parents. Fear in children was found associated with dental anxiety of parents oral health behaviour perception of parents.Conclusion It can be concluded that parentsrsquo dental fear and anxiety are reflected in their children.
