Septic Pulmonary Embolism: A Rare and Cataclysmic Complication of Infective Endocarditis 感染性心内膜炎的一种罕见且灾难性的并发症:脓毒性肺栓塞

Case: We report a 21-year-old male, former methamphetamine user, complaining of intermittent fever, cough and dyspnea for 6 months. He had a Graham-Steell murmur, grade 3/6 pansystolic murmur at the left lower sternal border, bipedal edema and petechiae on the trunk and extremities. Ceftriaxone and Gentamicin were given empirically for possible infective endocarditis. 2D echocardiography revealed heavy echogenic densities attached to the pulmonic valve cusps (2.0 × 0.79 cm), fluttering echogenic densities at the tricuspid valve leaflet tips and spontaneous echo contrast. Anticoagulation with Enoxaparin was started. Streptococcus agalactiae grew on blood culture (3 sites). On the 10th hospital day, he had an episode of hemoptysis (~600 ml) requiring endotracheal intubation. CT angiogram of the pulmonary arteries revealed several filling defects adherent to the pulmonary valve (endocarditic lesion), pulmonary embolism/septic emboli involving the left lateral wall of the main pulmonary artery, anterior segmental arteries of both upper lobes, and bilateral lower lung pulmonary arteries. Multiple small nodules, some exhibiting cavitation and the feeding vessel sign, were seen in both mid to lower lungs. Antibiotics were continued. Repeat blood cultures after 2 weeks no longer showed any growth. He was successfully extubated. Valve surgery was advised but no consent was given. Medical management was maximized. He was discharged improved after 2 months. Conclusion: Patients with RSIE are at risk to develop SPE which is an uncommon and potentially fatal complication of IE which may go unrecognized without a high index of suspicion. Methods and results: Sprague-Dawley rats underwent iron chloride3 (FeCl3)-induced carotid artery thrombosis model surgery were treated with vehicle (M group), low- (PL group), middle- (PM group) or high-dose propofol (PH group). Rats were received sham operation served as control (C group). As compared with the M group, propofol treatment significantly inhibited thrombus weights and improved the degree of embolization. Propofol also markedly improved blood CD62P intensity. Moreover, as compared with C group rats, the levels of plasma TXA2, PGI2, t-PA, t-PA/PAI-1 ratio, and P-selectin were harmful altered in M group rats in response to FeCl3 induction; these effects were improved in the PL and/or PM and PH groups. This study assessed the level of CVD risk among senior staff members of a Nigerian university.Methods: This cross-sectional study involved 221 academic and non-academic staff members of the Obafemi Awolowo University, Ile–Ife, Nigeria. Purposive sampling technique was used to recruit the participants. CVD risk was assessed using the Framingham Heart study questionnaire. Risk factors recorded were age, sex, personality type, sedentary lifestyle, smoking, weight and blood pressure. Risk scores were classified as low (0-19), moderate (20-29) and high (40+). Data were analyzed using descriptive and inferential statistics. Alpha level was set at p 0.05). Participants in high risk category were almost twice more likely to develop CVD compared to those at low risk (OR=1.933, CI=0.457-8.184). Similarly, males and females in high risk category were almost twice at risk of developing CVD compared to those at moderate risk (OR=1.882, CI=0.434-8.167). Furthermore, they were twice more likely to develop CVD compared to those in low risk category (OR=2.056, CI=0.495-8.533).Conclusion: Cardiovascular disease risk of participants in this study appears to be low and CVD risk was not associated with occupation type. The clinical presentations of CAD include silent ischemia, stable angina pectoris, unstable angina, myocardial infarction (MI), heart failure, and sudden death. Objective: This study was designed to define the frequency of hemorrhagic complications and to identify clinical variables associated with increased risk of bleeding complications in diabetic versus non-diabetic patients presented with acute coronary syndrome whom received aspirin, clopidogrel and heparin only or in combination with GPIIb/IIIa receptors blockade (Tirofiban) and to detect any bleeding complications in all patients during the period of admission in the hospital. Patients and Methods: 150 patients with ACS were divided into two groups, 82 diabetic patients and 68 nondiabetic patients. 40 patients out of total sample received tirofiban. Assessment of in hospital TIMI bleeding, GRACE and CRUSADE risk scores was estimated for all of them. Results: We observed that, there is no statistically significant difference in TIMI bleeding in both heparin and tirofiban group in diabetic versus non-diabetic patients. Cardiac catheterization access site was the most frequent location of bleeding most likely secondary to the high rate of coronary angiography performed in the study. There is a lack of data on the role of the local arterial stiffness, assessed by radio-frequency based system and its relationship with the molecular profile of plaques. Subjects: In this study we enrolled 18 consecutive patients undergoing carotid endoarterectomy, with homogeneus or heterogeneous plaques, as established by Doppler-ultrasound and local pulse-wave velocity was assessed before surgery. Methods: In carotid plaque specimens, we evaluated inflammasome (NLRP3), Receptor Activator of Nuclear Factor κB (RANK) and its natural ligand (RANK-L), Osteoprotegerin (OPG), and other inflammatory and apoptotic molecules by Western Blotting and qPCR analysis. In addition, lipid peroxidation of arterial specimens was assessed by TBARS assay. Results: In heterogeneus plaques we observed increased OPG expression (p=0.04), positively correlated with lipid peroxidation values (r=0.511, p=0.03); increased levels of RANK (p=0.02), and other inflammatory and apoptotic molecules. RANK-L protein was augmented in homogeneus plaques (p=0.01) and correlated to s-index (r=0.514, p=0.03) and PWV (r=0.525, p=0.03) values. The patients were followed up to 24 months. Results: We found significant relation between Summed stress score (SSS) and sudden cardiac death, MI and HF. Also there were statistically significant relation between atypical pain and HF. Adaptive servo-ventilation (ASV) is one of the treatments for SDB. Recently, ASV has been applied to the treatment of patients with heart failure and SDB. The aim of this study was to estimate whether ASV therapy is effective and safe for patients with heart failure with reduced ejection fraction (HFrEF) compared with standard medical therapy in the real world, using three-year follow-up data. Methods: The patients with HFrEF (n=186) were treated with standard medical therapy plus ASV therapy (ASV group; n=57) or standard medical therapy alone (control group; n=129). We followed up the patients for 3 years. Results: The ASV group had significantly lower baseline left ventricular ejection fraction than the control group (27.9 ± 8.6% vs. 34.1 ± 8.8%; P<0.0001). Baseline brain natriuretic peptide (BNP) did not differ significantly between the groups. On follow-up, the changes in BNP level indicated that the ASV group showed significantly better improvement than the control group. There was no significant difference between the groups in the rate of fatal cardiovascular events (P=0.190, log-rank test). Their association, although exceptional, is burdened with a heavy mortality in the absence of an early and adapted surgery. We report a case of Standford acute type A aortic dissection associated with fatal pulmonary embolism. This is a 66-years old man with a notion of a long trip received for prolonged chest pain since 3 days, of sudden onset, maximal intensity, tearing, transfixing, migratory, radiating towards the back and the loins, exacerbated by breathing. The examination noted 120 bpm tachycardia, 70% SaO2 desaturation, asymetric blood pressure and asymetric pulse, aortic insufficiency murmur and bilateral crackling rettles at lungs. The chest X-ray showed an enlargement of the upper mediastinum with double-contoured image. The electrocardiogram recorded a regular sinus tachycardia at 132 cycles/min, a lateral sub epicardial ischemia in lateral leads. CT scan revealed total thrombosis of the right branch of the pulmonary artery extending to part of the trunk and an intimal flap from the aortic arch to the abdominal aorta. The evolution was brutally lethal two days after hospitalization.
