While less common in the age < 35 population, the occurrence of SCD in the young raises concern, with multiple possible etiologies and often unanswered questions. While coronary artery disease is the leading cause in those > 35 years of age, the younger population faces a different subset of pathologies associated with SCD, including arrhythmias and cardiomyopathies. The tragic nature of SCD in the young entails that we explore and implement available screening methods for this population, and perform the necessary investigations such as electrocardiography (ECG) and echocardiography. In this review, we not only explore the vast etiology associated with SCD in those age < 35, but emphasize evaluation methods, who is at risk, and delve into screening of SCD in potential victims and their family members, in an attempt to prevent this traumatic event. These patients often experience long wait times in the emergency department before receiving their first dose of analgesia. This delay results from the complexities of the emergency care system. Using the intranasal or sublingual approach to administering analgesia to SCD patients with VOC offers a fast, safe, noninvasive, atraumatic, and easily accessible route of administration which could reduce the time to first dose of analgesia. With the evolving advances in the development and delivery of analgesic medications, providers should be conversant with the nuances of intranasal and sublingual analgesia in the management of acute vaso-occlusive pain crisis. Anticoagulation reversal is required in some acute cases. This is usually accomplished by plasma transfusion or four-factor prothrombin complex concentrate (4F-PCC). The aim of this study was to gain insight into the clinical course of patients with gastrointestinal (GI) bleeding who require VKA reversal.</p><p><strong>Methods: </strong>Medical records were collected from two centers from patients who presented to the emergency department (ED) for GI bleeding and received 4F-PCC or plasma for VKA reversal between January 2015 and December 2020. ED, hospital, intensive care unit (ICU) length of stay (LOS) as well as time from admission to GI procedure were determined.</p><p><strong>Results: </strong>4F-PCC patients (n = 49) as compared to plasma (n = 63) patients were found to have a greater number of comorbidities (average of 4.2 vs. 2.7 comorbidities/patient) and more ICU admissions (47% vs. 21%). Time to GI procedure was significantly decreased in the 4F-PCC group (median (interquartile range (IQR)) 19.47 (9.23 - 30.25) vs. 27.88 (21.38 - 45.00) h; P = 0.01). When adjusting for comorbidities, differences in time to GI procedures were also significant in favor of 4F-PCC regardless of any comorbidities (P = 0.014), in atrial fibrillation (P = 0.045) and in hypertension (P = 0.02). The 4F-PCC patients had shorter LOS in the ED and ICU.</p><p><strong>Conclusions: </strong>Our study demonstrated that compared to plasma, 4F-PCC was utilized in more acutely ill patients with higher rates of comorbidities and ICU admission. Due to the scarcity of such evidence, no consensus guidelines regarding the diagnostic approach and management of patients with splenic infarction exist. Most of published articles on splenic infarction are case reports and there was no systematic review on splenic infarction.</p><p><strong>Methods: </strong>We conducted a retrospective analysis of all radiologically confirmed cases of splenic infarction patients with any history of admission at National Center for Global Health and Medicine Kohnodai Hospital, from 2014 to 2020. Further, to understand the pathophysiology that causes splenic infarction, we searched the literatures on splenic infarction.</p><p><strong>Results: </strong>We found 18 patients with splenic infarction. The average age was 78 years, and about half of patients had abdominal pain; however, the other half did not have abdominal pain. One-third of patients with splenic infarction died. Leukocytosis with neutrophilia, a decrease of lymphocytes, anemia, hypoalbuminemia, and liver dysfunction were observed. Fibrinogen was decreased and D-dimer was remarkably elevated. Lactate dehydrogenase (LDH) and C-reactive protein (CRP) were remarkably increased. Six patients (33.3%) had cancer, four patients (22.2%) had atrial fibrillation, and four patients (22.2%) had infection. We found 466 case reports on splenic infarction published from 1975 to 2021. Recently, the number of case reports on splenic infarction due to infection, especially, coronavirus disease 2019 (COVID-19), has been remarkably increasing. Furthermore, we found that leukocytosis, a decrease of lymphocytes, elongated activated partial thromboplastin time, decrease of fibrinogen, liver dysfunction, elevation of LDH and blood urea nitrogen can be the prognosis predicting factors for patients with splenic infarction.</p><p><strong>Conclusion: </strong>Our study elucidated clinical, hematological, biochemical and radiological characteristics for patients with splenic infarction. We newly found significant differences in blood cell counts, coagulation markers, transaminases, LDH and blood urea nitrogen between patients who died and those who survived, suggesting that these parameters can be the prognosis predicting factors for splenic infarction. Method: Acclaim 120 C18 chromatographic column (5 μm. 4.6 * 250mm), acetonitrile-0.1% formic acid aqueous solution was used as the mobile phase for gradient elution, and the contents of 5 components, 3'-methoxy puerarin, puerarin, daidzin, daidzein, and 3'-hydroxypuerarin, were determined in 11 commercial pueraria lobata from different habitats. Results: The contents of these five components in pueraria lobata from different habitats were different. Anhui No. 003 and No. 006 have higher chemical constituents. Conclusion: The method established in this experiment is accurate, with good separation effect, and can be used to detect the content of five different substances in Pueraria lobata. The control group only received conventional chemotherapy regimen.The observation group was added Huangqi Fuzheng decoction on the basis of the control group. The incidence of adverse reactions of chemotherapy was compared between the two groups,the changes of immune indexes, quality of life score and sleep quality score before and after treatment were compared between the two groups. Results: ① The incidence of adverse reactions in the observation group was lower than that in the control group (P < 0.05). ② The changes of CD4+, CD8+, CD4+/CD8+ in the observation group before and after treatment were higher than those in the control group (P < 0.05). ③ The changes of quality of life score and sleep quality score before and after treatment in the observation group were higher than those in the control group (P < 0.05). Methods: A total of 106 patients who received orbital repair treatment in the author's unit from 2019 to 2021 were selected as research objects and were divided retrospectively into observation group and control group according to different treatment methods, with 53 patients in each group. The control group was treated with botulinum toxin type A repair therapy, and the observation group was treated with phased radio frequency microneedle repair therapy on the basis of the control group. The periorbital skin texture, wrinkles and static wrinkles before and after treatment were compared between the two groups. Results: After 2 weeks of treatment, the periorbital wrinkle and texture based evaluation value and static wrinkle score of 2 groups were decreased to different degrees, and the observation group was lower than the control group, with statistical significance (P < 0.05). After 1 month and 3 months of treatment, the periorbital wrinkle and texture evaluation value and static wrinkle score of the observation group were lower than those of the control group, with statistical significance (P < 0.001). The analysis directions included (1) The trial types of civil litigation cases in each year; (2) Annual distribution of civil litigation cases in obstetrics and gynecology in an annual year basis; (3) Distribution of the amount of the subject matter and the amount of civil compensation; (4) The geographical distribution of litigation cases; (5) Characteristics of the medical institution as defendant; (6) Distribution of litigation departments in medical institutions; (7) Common causes of action in obstetrics and gynecology cases; (8) Results of common injuries in obstetrics and gynecology cases; (9) Court confirmation of disputes over medical records. Results: According to the data analysis results, the distribution of litigation trial types, annual distribution, compensation amount distribution, regional distribution and other basic data were obtained, and based on the analysis of the characteristics of medical institutions, department distribution, causes of litigation, damage structure and court recognition results were systematically discussed. Conclusion: Although the liability disputes for medical damage in obstetrics and gynecology have been declining in recent years, the problems in medical treatment and management can not be ignored. Results: According to the data summary and data discussion in the same year, the geographical distribution of cases, the characteristics of the defendant's medical structure, the distribution of litigation departments and other data were summarized.
