A standardized protocol for photographing HS is critically needed.

Objectives: To establish a standardized protocol for using photography to clinically document HS.

Methods: We brought together a panel of international experts and used a modified Delphi process to reach consensus on statements about standardized practices for using photography to document HS in clinical practice and research settings. A 75% agreement level was prespecified as the cutoff for consensus.

Results: Consensus for photographically documenting HS was developed within 6 key areas: patient preparation, background, lighting, size and reference scale, camera considerations, and patient positioning. The revised and optimized protocol includes 34 standardized reference illustrations. However, the proposed recommendations are for ideal scenarios, and their implementation may not always be practical in a clinical setting. Variations in resources may not have been captured.

Conclusions: The proposed consensus-based protocol provides a standard for consistent photographic documentation of HS. Off-label use of mammalian target of rapamycin inhibitors like sirolimus has shown promising potential in dermatology. We analyzed the literature on the efficacy and safety of topical sirolimus for a range of dermatological conditions, including inflammatory eruptions, genodermatoses, bullous disorders, neoplasms, and others. Publications on topical sirolimus for other vascular anomalies, such as lymphatic malformations, were excluded to maintain focus on other lesser-known dermatological uses, except for studies involving post-laser combination therapies for vascular lesions. Topical sirolimus demonstrated notable efficacy in several genetic and benign proliferative skin disorders, including pachyonychia congenita, trichilemmomas, trichoepitheliomas, and Kaposi sarcoma, with a generally favorable safety profile and minimal systemic absorption. However, variability in formulations, dosing regimens, and the predominance of small-scale studies limit definitive conclusions. Further research is warranted to standardize treatment protocols and validate these findings.
","PeriodicalId":15403,"journal":{"name":"Journal of Cutaneous Medicine and Surgery","volume":" ","pages":"12034754251355595"},"PeriodicalIF":3.1,"publicationDate":"2025-07-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144626424","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Studies involving patients with DA are difficult to conduct because patients typically move from one hospital to another. Moreover, very little has been written about its treatment, as most published series of cases have been presented without reporting the follow-up.

Objectives: To study the characteristics and therapeutic results in patients with dermatitis artefacta followed up at a university medical centre in Brazil.

Methods: We performed a retrospective study of patients with dermatitis artefacta at a dermatological university clinic. The medical records were reviewed to obtain personal data and treatment findings.

Results: Forty-two patients were included in this study. Twenty patients were treated with placebo pills plus topical treatments, 14 patients received no treatment, and four patients used only topical treatment. The follow-up period for these patients ranged from a single visit to 7 years. Psychiatric interviews were conducted without the presence of a dermatologist in only a few cases.

Conclusions: A dermatologist is a healthcare professional who should be prepared to diagnose and manage cases of DA, and referral to a psychiatrist is usually unsuccessful. The success of the treatment depends on how the doctor approaches the patient. The use of placebo pills may help some patients adhere to treatment and strengthen their therapeutic bonds.
","PeriodicalId":15403,"journal":{"name":"Journal of Cutaneous Medicine and Surgery","volume":" ","pages":"12034754251355589"},"PeriodicalIF":3.1,"publicationDate":"2025-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144618128","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Demographic and clinical data were recorded within the database, which included over 45 patient variables.

Results: Throughout the active phase of the trial (duration: 27 months), 123,124 injectable treatments were conducted. One hundred and eleven patients, experiencing a total of 235 AEs, were entered into the portal. This equated to an AE incidence rate of 0.19%, per treatment. Thirty unique products were associated with AEs. In total, there were 112/235 (47.66%) mild, 88/235 (37.45%) moderate, and 35/235 (14.90%) severe AEs. The most common complication (n = 48/235; 20.43%) was swelling, with a prevalence of 0.04%. Of the documented AEs, only 5 were reported to other sources, including 1 case being reported to Health Canada and 4 cases to the respective product manufacturer.

Conclusions: The initial feasibility of a registry assessing safety outcomes following injectable treatment has been demonstrated.
