The present study aimed to compare the efficacy of topical calcipotriol with IL triamcinolone acetonide versus IL triamcinolone acetonide alone in AA.</p><p><strong>Methods: </strong>This randomized single-blinded clinical trial was conducted in a dermatology outpatient department. Both groups A and B received IL triamcinolone acetonide injections every 4 weeks for 12 weeks. Group A also received twice-daily topical calcipotriol. Follow-up was done every 4 weeks till 12 weeks. The primary outcome parameter was hair density assessment using a hair regrowth scale (RGS). Other parameters were the presence or absence of poor clinical response (less than 50% hair density), regrowth patterns, dermoscopy features, and adverse effects.</p><p><strong>Results: </strong>Seventy-three (179 patches) patients completed the study. RGS and patches with more than 50% terminal hair regrowth significantly improved in both the groups beginning in the fourth week and going forward (<i>P</i>-value: .001, .001). The two treatment groups exhibited no statistically significant difference in terminal hair density (<i>P</i> values: .930, .616, .178). However, subgroup analysis showed a statistically significant difference in scalp patches of group A compared to group B in the 12th week of the study (<i>P</i> = .018) but not in beard patches (<i>P</i>-value: .527). Group A exhibited more irritation with a reduced incidence of atrophy.</p><p><strong>Conclusion: </strong>The addition of topical calcipotriol to IL steroid showed increased efficacy in scalp patches of AA but not in beard patches. {"title":"Clinical Outcomes and Complications in Complex Closures Under Local Anaesthesia Post-Mohs Surgery.","authors":"Somaira Nowsheen, Vy X Pham, Shaundra Eichstadt, Shang I Brian Jiang","doi":"10.1177/12034754251316297","DOIUrl":"https://doi.org/10.1177/12034754251316297","url":null,"abstract":"<p><strong>Background: </strong>Mohs micrographic surgery offers high cure rates of cutaneous malignancies, but surgeons are often faced with large and complicated defects after tumour removal.</p><p><strong>Objectives: </strong>To assess the safety and complication rates of large flaps and grafts (measuring ≥30 cm<sup>2</sup>) and larger complex linear closures (CLC, ≥12.5 cm, as defined by the American Medical Association Current Procedural Terminology code set), when performed under local anaesthesia.</p><p><strong>Methods: </strong>A retrospective analysis was conducted on a cohort of patients who underwent skin cancer treatment by Mohs at an academic institution (UCSD) from January 1, 2010, to December 31, 2022, and whose defects were repaired with CLC, flap, or skin graft. Demographic, surgical, and complication data were collected and analyzed.</p><p><strong>Results: </strong>Of the 436 patients who met inclusion criteria, 39 underwent CLC, 344 underwent flap reconstruction, and 53 underwent grafting. Adverse effects to local anaesthesia were notably absent in all treatment methods. Overall complication rate was noted to be 16.1% (70/436). To increase reporting, investigators have recently developed a novel Electronic Data Capture system: The Global Registry of Adverse Clinical Events (GRACE<sup>©</sup>).</p><p><strong>Objective: </strong>To identify the incidence of AEs associated with aesthetic injectable treatments.</p><p><strong>Methods: </strong>Aesthetic clinicians from 10 Canadian sites were recruited. Demographic and clinical data were recorded within the database, which included over 45 patient variables.</p><p><strong>Results: </strong>Throughout the active phase of the trial (duration: 27 months), 123,124 injectable treatments were conducted. One hundred and eleven patients, experiencing a total of 235 AEs, were entered into the portal. This equated to an AE incidence rate of 0.19%, per treatment. Thirty unique products were associated with AEs. In total, there were 112/235 (47.66%) mild, 88/235 (37.45%) moderate, and 35/235 (14.90%) severe AEs. The most common complication (n = 48/235; 20.43%) was swelling, with a prevalence of 0.04%. Of the documented AEs, only 5 were reported to other sources, including 1 case being reported to Health Canada and 4 cases to the respective product manufacturer.</p><p><strong>Conclusions: </strong>The initial feasibility of a registry assessing safety outcomes following injectable treatment has been demonstrated. Findings support that the implementation of the GRACE Portal is an effective outreach strategy for increasing AE reporting by health care professionals. Burden of Vitiligo in Canada: Retrospective Analysis of a Canadian Public Claims Database.

Treatment options are limited.</p><p><strong>Objectives: </strong>To examine disease burden and healthcare resource utilization (HCRU) among patients with vitiligo in Québec, Canada.</p><p><strong>Methods: </strong>In this retrospective study, data were obtained from the Régie de l'Assurance Maladie du Québec (RAMQ) databases for 125,000 random individuals from January 2010 to December 2019. The <i>International Classification of Diseases, Ninth Revision</i> (<i>ICD-9</i>) diagnostic code [709.x (other skin disorders)] with vitiligo-related treatment was used to identify patients with vitiligo. Patient characteristics and treatments, including treatment type, episodes (treatments used without discontinuation), and sequences (treatment episodes ≥30 days), were assessed. Annualized HCRU and costs (2021 adjusted) included all-cause hospitalization, emergency department visits, outpatient visits, and medications among patients with vitiligo (n = 113) and age- and sex-matched non-vitiligo controls (n = 339).</p><p><strong>Results: </strong>Of patients with vitiligo (mean age, 50.0 years; 68.1% female) identified using <i>ICD-9</i> code 709.x with vitiligo-related treatment, 36.3% received ≥4 treatment episodes. Treatment patterns were heterogeneous, with 43 different sequences reported. Annualized mean outpatient visits (16.1 vs 5.5) and all-cause outpatient service costs per patient were significantly higher in the vitiligo versus the control group (CAN$1037 vs CAN$523; <i>P</i> < .01). Total all-cause services costs were higher for patients with vitiligo in the year after versus before diagnosis (CAN$3679 vs CAN$2085; <i>P</i> = .04).</p><p><strong>Conclusions: </strong>Vitiligo is associated with significant burden and HCRU among patients in Québec, Canada, who were identified by <i>ICD-9</i> code 709.x plus vitiligo-related treatment.
