The most common malignancy of the biliary tract is gall bladder carcinoma and the main subtype according to the histological classification is Adenocarcinoma. Pure squamous cell carcinoma of the gall bladder is very rare entity accounting for only 1.1-3.7% of the gall bladder carcinomas. It is highly malignant with poor prognosis due to high proliferative rate and local invasiveness to the adjacent organs. The patients are usually diagnosed at an advanced stage with a bulky tumor owing to its aggressive behavior. Aim. In this paper, we describe a female patient with primary pure squamous cell carcinoma of the gall bladder. Description of the case. A 42-year old female patient presented with chief complaints of pain in abdomen associated with nausea and vomiting and gradually progressive jaundice since 02 months. Contrast Enhancing Computed Tomography (CECT) abdomen showed an enhancing mass lesion in gall bladder involving adjacent organs for which she underwent extended cholecystectomy with pancreaticoduodenectomy. Conclusion. Diagnosis as well as the management of this exceptionally rare type of tumour is undoubtedly challenging because of non-specific clinical as well as imaging findings. This case report is an attempt to add to the literary evidence for better pathological as well as clinical understanding of this rare and aggressive entity thereby providing additional material for the early diagnosis as well as the development of effective targeted therapies which will certainly help in increasing the lifespan of these patients.","PeriodicalId":15378,"journal":{"name":"临床和实验医学杂志","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85236981","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Structural glycans have great biological significance and are involved in signaling and cell communication of the immune system. They are attached to proteins and lipids in an enzymatic process called glycosylation where glycosyltransferase and glycosidases bind sugar residues and lead to the formation of bioconjugates. Aim. In this paper we describe the importance of glycosylation in the immune system and its changes in diseases. Material and methods. This review was performed according to systematic literature search of major bibliographic databases. Results. Proper glycosylation ensures the functioning of the organism, however, defects in structural glycans of immune system changes their properties and can lead to disorders and further to autoimmune diseases. It has been also proven that glycosylation of autoimmune system is changed during cancer. In this paper we described types of structural glycans, significance of glycosylation of selected components of the immune system and its modifications in disorders. Conclusions. Knowledge about changes in the glycosylation in diseases is the key to understanding the processes of autoimmune diseases and may allow the development of new treatments in the future.","PeriodicalId":15378,"journal":{"name":"临床和实验医学杂志","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80561660","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Thyroglossal duct cyst (TDC) is the most common cause of congenital neck mass. These can present anywhere from foramen caecum to mediastinum. Usually presents as a solitary cyst, the double thyroglossal cyst is very rare and a triple thyroglossal cyst has never been reported. Aim. Herein, we report an atypical case of triple thyroglossal cyst, at levels of hyoid, thyrohyoid membrane and thyroid isthmus managed surgically without any complication. Description of the case. We are presenting case of a 48-year-old female who presented to us with the complaint of anterior neck swelling since birth. On work up it was diagnosed as a case of the thyroglossal duct cyst and was intraoperatively found to have 3 distinct cystic swellings connected to a common stalk lying beneath the hyoid. It was successfully treated by modified Sistrunk’s procedure. There was no evidence of recurrence on follow up for 6 months. Considering atypical presentations, there are 9 cases reported with the double thyroglossal duct cyst, TDC within the thyroid gland and sublingual TDC. Such presentations make the diagnosis more challenging, leading to improper treatment. Conclusion. We are presenting this case as there is no case reported in English literature with a triple thyroglossal duct cyst. An awareness that thyroglossal cyst can present as multiple cysts is important for clinician in order to perform correct surgical management and to avoid the most feared complication of recurrence.","PeriodicalId":15378,"journal":{"name":"临床和实验医学杂志","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89663388","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The 2019-novel-coronavirus (2019-nCoV) outbreak has become a common problem for the whole world. Aim. To investigate the impact of the 2019-nCoV pandemic period in our country on emergency room admissions. Materials and methods. The study was designed as a retrospective cohort. The first case of pneumonia infected with nCoV in our country was diagnosed on March 11, 2020. Considering the date of March 11, 2020, emergency room admissions for two periods of 7 days were investigated, pre-COVID-19 and COVID19. Demographic data, admission diagnosis, vital findings, stay times in emergency room, terminations and emergency department mortality examined. A group of ‘geriatric’ patient populations were created to examine the admission characteristics of patients aged-65 and over. Results. The 3466 patients included in the study. The average number of daily admissions was significantly higher in the pre-COVID-19 period (350.4 ± 54.5), compared to the COVID-19 period (144.7 ± 20.2, p=0001). While the proportion of cardiac-caused admissions increased during the COVID period, the proportion of gastrointestinal-induced admissions decreased (p<0001). Hospitalization rates for both adult and geriatric patients increased during the COVID period (p<0001). Conclusion. Despite all the negativity caused by the outbreak, this period has been one in which the public is aware of unnecessary emergency room which has been subsequently lessened.","PeriodicalId":15378,"journal":{"name":"临床和实验医学杂志","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"72417562","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
