However, there are many treatment options that may help to improve daily functioning of a child suffering from CP. But these options failed to prevent early death of the patients. New curable treatment era has started for CP patients with bilateral cranioplasty and duraplasty. This was the second case of the case series of CP treated by bilateral cranioplasty and duraplasty with very good outcomes. During operation of this patient, after removing cranial bones by craniotomy, found no dural pulsation. It is indicative of raised intracranial pressure. After duraplasty, brain pulsation was found very nicely, which indicates that intracranial pressure became normal. In early childhood, brain grows with the dural growth simultaneously. But dura is adherent with the skull bones. Premature closure of sutures and with thickening of skull bones result in small intracranial volume which does not allow the brain to grow according to age. So, brain atrophy causes widened fissures and widened sulci with brain parenchymal tissue loss as evidenced in CT scan of brain or MRI. Peptic ulcer perforation followed by peritonitis remains as an important cause of mortality. In severe peritonitis, the use of exploratory laparotomy and intra-operative lavage with large amount of saline solution has been the standard of care to reduce risk of postoperative infections.\u0000Objective: To compare the outcome between povidone iodine mixed normal saline (N/S) lavage and conventional normal saline lavage in the management of peritonitis in a tertiary care public hospital in Bangladesh.\u0000Methodology: This was a randomized clinical trial conducted in the department of Surgery, Shaheed Ziaur Rahman Medical College Hospital, Bogura during July 2013 to December 2013. There were total 50 patients, selected by purposive sampling as a diagnosed case of peritonitis due to peptic ulcer perforation. All the patients were divided into 2 groups by using coin toss as group A (conventional N/S) and group B (povidone iodine mixed with N/S). The data were collected using the pretested data collection sheet. Results were tested by chi-square test to see their level of significance with a p-value at <0.05.\u0000Results: The maximum (17, 34.0%) patients presented as peritonitis due to peptic ulcer perforation was from 18-30 years age group. The mean age of the patient was 29.16±9.25 years (age range: 18-61 years). The female to male ratio were 5.25:1 and 3.16:1 in group A and B respectively. Some (22, 44.0%) of the patients showed 13-18 hours delay since first symptom appeared. Fever (88.0% vs 80.0%), surgical site infection (SSI) (32.0% vs 24.0%), burst abdomen (20.0% vs 12.0%), intra-abdominal sepsis (8.0% vs 4.0%) were evident as postoperative complications in the groups A and B respectively. Only 1 (4.0%) patient died among group A patients due to sudden cardiac arrest. A protocol helps to ensure that all survivors of GBV regardless of their age, background or ethnicity receive appropriate and effective care when they enter a health facility. Concern for patient welfare extends to ensuring that patients’ dignity is respected after an assault that may leave them feeling humiliated and degraded. Additionally, medical and forensic services should be provided in a manner that minimizes the number of invasive physical examinations and interviews of the patient. Rape management is generally considered as a medical emergency and the provision of medico-legal services thus takes secondary importance to general healthcare services [such as trauma treatment, evaluation and management of pregnancy and sexually transmitted infections (STIs)]. Performing a forensic examination may be considered as a negligent practice without meeting the primary healthcare needs of the patients. Health facilities are often the first entry points for GBV survivors – so, it is crucial for HCPs to be skilled and well-equipped to manage the care to the survivors. The ecotoilets are especially built to collect human excreta for using them as manure on the fields after appropriate treatment for 12 months.\u0000Objectives: This study was designed to see the physical properties appropriate for fields and pathogenic potential to ensure the treated human excreta are safe for cultivator’s handling.\u0000Methodology: The study was carried out in three phases during 2010 to 2014. A total of 210 composted faecal excreta were examined for physical properties like colour, odour, odour intensity, moisture, bacteriological examination for faecally transmitted potential bacteria and parasitological examination for ova / cysts of faecally - transmitted parasites to recommend whether these composted excreta were suitable for use as manure by the cultivators.\u0000Results: Majority of the specimens in all three phases contained remarkable moisture: 22.3-80.0% in all specimens in phase I, a considerable number (16, 53.3%) contained 25% moisture in phase II and high moisture content (66.4%-78.9%) in 6(50.0%) of specimens in phase III. Bacteriological examination revealed no pathogenic organism in any specimen of the phases- although, some specimens in all phases showed growth of non-coliform bacteria. Parasitological examinations in all phases revealed ova/larvae of helminths of Ascaris lumbricoides (AL)/ Trichuris trichiura (TT)/ Strongyloides stercoralis (SS). {"title":"Evaluation of the Clinical Features of Bronchiolitis in Paediatric Patients and Exploration of the Risk Factors","authors":"Rumy Tabrez Hyder, Shafi Ahmed, Atm Faruque Ahmed","doi":"10.3329/jmomc.v8i2.64440","DOIUrl":"https://doi.org/10.3329/jmomc.v8i2.64440","url":null,"abstract":"Background: In general, bronchiolitis presents as a progressive viral respiratory illness in children younger than 2 years of age, most commonly between 2-6 months.\u0000Objective: The study was conducted to assess the clinical presentations of acute bronchiolitis in infants and young children and to evaluate the individual characteristics and environmental factors which can increase the risk of severe disease and hospitalization.\u0000Methodology: This descriptive study was conducted in the department of Paediatrics, Khwaja Yunus Ali Medical College and Hospital, Enayetpur, Sirajganj. A total of 70 patients were included in the study, who were admitted to the hospital with a clinical and radiological diagnosis of acute bronchiolitis and were followed up in the Paediatric department during January, 2021 to December, 2021. Data collection was done by using a questionnaire form and by physical examination. The information for the questionnaire was provided by the childrens’ mothers. The SPSS software was used for data analysis.\u0000Results: According to the results of the study, 39(55.7%) children were infants, aged between 2-6 months, 21(30.0%) were between 7-12 months and 10(14.3%) children of more than 12 months of age. Boys (47, 67.1%) were affected more commonly than girls (23, 32.9%) in a ratio of 2:1. Some 60 (85.7%) of the children had siblings and 46 (65.7%) children lived in crowded environment at home. Many (25, 35.7%) parents of the children were cigarette smokers and 37(52.9%) children were given formula feeds. There was previous history of hospitalization among 14(20.0%) cases and 5(7.1%) children were pre-term. The most common clinical presentations of bronchiolitis were cough among 61(87.1%) cases, wheeze in 56(80.0%), respiratory distress in 49(70.0%) and feeding difficulty in 28(40.0%) children.\u0000Conclusion: The risk factors for bronchiolitis were young age (2-6 months), presence of a sibling, a history of hospital admission, crowding and paternal smoking at home and formula feeding. Sharma","doi":"10.3329/jmomc.v8i2.64438","DOIUrl":"https://doi.org/10.3329/jmomc.v8i2.64438","url":null,"abstract":"Background: Good dental appearance is an increased concern during adolescence and as because malocclusion comprises the health of oral tissues, it can lead to bad dental appearance with social and psychological problems.\u0000Objective: The purpose of the present study was to find out the clinico-demographic status of malocclusion among school children.\u0000Methodology: This was a cross-sectional study carried out in four high schools at Dhaka city, Bangladesh. The study was conducted from May, 2015 to November, 2015 for a period of 6 (six) months on 384 secondary school children aged between 11 to 16 years by systematic random sampling. The participants were included among those who had no preventive and interceptive orthodontic treatment previously and participants having late mixed or early permanent dentition. Data were collected using a data collection sheet after taking informed consent. The relevant socio-demographic data of the patients were also collected and recorded.\u0000Results: The frequency of malocclusion was more common in males (218, 60.1%).
