Many factors contribute to the risk of medication discrepancies. Despite medication reconciliation service being practiced in the hospital setting, there is limited knowledge on its effectiveness in the primary care setting. \u0000Objective: To evaluate the effectiveness of a pre-consultation medication reconciliation service in reducing unintentional medication discrepancies in patients who are transitioning from hospital to primary care. \u0000Methods: A multi-centre, randomised controlled trial in 3 Singapore public sector primary care clinics was conducted.189 patients aged ≥ 21 years, with ≥ 5 chronic medications and on first follow-up visit to primary care clinics for chronic disease management after recent hospital discharge were randomly assigned to receive either a pharmacist-led MRS prior to physician consultation or usual care. Post-consultation medication reconciliation was subsequently conducted for both groups. \u0000Results: Post-consultation unintentional medication discrepancies were significantly lower in the intervention compared to the control group (15.8% versus 57.4% respectively, p<0.001). Pre-consultation medication reconciliation is effective in reducing unintentional medication discrepancies (OR 7.74 (95% CI 3.72 to 16.13). Omission of drug was the most common type of medication discrepancy (35.8%). The 30-day rehospitalisation rate was 5.4%. 30- day medication adherence was significantly higher compared to baseline (mean MMAS-8 score of 7.25 versus 6, p<0.001). \u0000Conclusion: Medication discrepancies posed as potential risks to patient safety. Mi","doi":"10.21065/1920-4159.1000254","DOIUrl":"https://doi.org/10.21065/1920-4159.1000254","url":null,"abstract":"Background: Patient satisfaction is a worldwide goal of health care organizations and is considered as an indicator for the quality of healthcare services provided in any health care sector. \u0000Purpose: To assess patients’ perceptions and satisfaction in the outpatient pharmacies at the National Centre for Cancer Care and Research (NCCCR) and the Heart Hospital (HH) at Hamad Medical Corporation (HMC) in Qatar. \u0000Methods: A cross-sectional, descriptive study was conducted at the HH and the NCCCR from February to March 2013 using validated and piloted questionnaire. The self-administered questionnaire consisted of 5 sections: patients socio-demographic characteristics, five perception statements regarding pharmacy layout and waiting area, six statements regarding patients interaction with pharmacists, four perception questions concerning pharmacists’ skills and two statements regarding the overall satisfaction. All statements were assessed with 5 points Likert scale. The survey was distributed to patients visited HH and NCCCR during the study period. Data was analysed descriptively using SPSS version 18. \u0000Results: A total of 198 participants completed the survey. Assessment of patients` satisfaction with the general pharmacy layout revealed that 93% of respondents were satisfied with pharmacy physical layout and 99% with dispensing area. With regard to patient's perception about interaction with pharmacists, the majority of respondents expressed high satisfaction with pharmacists' professionalism (99%), time spent consulting them (98%) and pharmacists` wellness to answer their inquires (99%). Top rated pharmacists’ skill by patients was pharmacists' explanation to treatment (97%). Ethno medicinal plant studies have become of particular interest and have become increasingly more valuable in the development of health care and conservation programs in different parts of the world. In developing countries, medicinal plants continue to be a main source of medication. It has been estimated that approximately 88% of the inhabitants of underdeveloped countries rely mainly on traditional medicine for their primary form of medicinal health care. This review investigates the available studies on the pharmacological effects of some medicinal plants (Aloysia triphlla, Citrus aurantium, Echium amoenum, Lavandula stoechas, Melissa officinalis, Valeriana officinalis, Viola odorata, Salix aegyptica and Cinnamomum zelanicum) on digestive diseases. Designated as ACV-oxalic acid, methods for the preparation thereof and its use in pharmaceutical applications are described. The co-amorphous ACV-oxalic acid was characterized by powder X-ray diffraction, differential scanning calorimetry and thermogravimetric analysis. Stability with respect to relative humidity (RH) for the co-amorphous ACV-oxalic acid was evaluated and compared with the parent ACV. Instrumentation of the method was very simple and used the mixture of acetonitrile: water (60:40, v/v) as the mobile phase. Separation was carried out on a BDS. Hypersil C8 (5 um, 250 × 4.6 mm) column. Effects of composition of mobile phase in addition to flow rate and detection wave length were studied. Calibration was obeyed in the range of 20-100 μg/ml of glibenclamide. Abukhadijah","doi":"10.21065/1920-4159.1000249","DOIUrl":"https://doi.org/10.21065/1920-4159.1000249","url":null,"abstract":"Background: Quality health personnel are an important asset to the healthcare institution which will benefit the patients and the society. \u0000 \u0000Objective: To study pharmacists’ and pharmacy technicians’ perceptions and satisfaction towards the structured pharmacy orientation program at two specialized hospitals. \u0000 \u0000Methods: A cross-sectional, observational study was conducted from January 2010 till April 2013 using an online validated and piloted questionnaire. The questionnaire consisted of 3 parts: Pharmacy staff socio-demographic and practice characteristics, perceptions and satisfaction. There were 8 perception statements related to the orientation’s site, 11 perception statements related to the mentors, and an overall satisfaction question. A five points Likert type scale was used in the questionnaire. The self- administered survey was distributed to 64 newly pharmacy staff upon the completion of their orientation program using Survey Monkey®. Data was analysed using descriptive statistics. \u0000 \u0000Results: A total of 64 pharmacy staff completed the survey; 42 (66%) were pharmacists and 22 (34%) were pharmacy technicians. The overall pharmacy staff satisfaction towards the orientation program was 94%. Fifty six (88%) of respondents agreed that the orientation at the site was adequate, 60 (94%) of pharmacy staff agreed that the mentor provided constructive criticism on interpersonal skills with staff and patients whenever needed, 59 (92%) of pharmacy staff agreed that the mentor displayed professional attitude and motivation. \u0000 \u0000Conclusion: The newly developed orientation system was successful in achieving high satisfaction level among pharmacy staffs in different aspects. This study helped in assessing and improving the orientation system in our institution and could improve work quality at workplace. No established prophylaxis exists against multikinase inhibitor-associated HFSR and further improvement of prophylaxis for HFSR is needed. Therefore, we offer multiagent therapy to prevent Regorafenib-associated HFSR comprising topical steroid (0.05% difluprednate) ointment and 20% urea-based cream. \u0000Methods: Subjects were unresectable or recurrent colorectal cancer patients who started Regorafenib therapy between May 2013 and March 2014 at the Shizuoka Cancer Center. Electronic medical records were retrospectively examined for HFSR incidence, CTCAE v3.0 grade of severest HFSR, time of HFSR onset, rate of therapy termination, delay and dose reduction. \u0000Results: Subjects were 55 patients and median treatment time 7.1 weeks. Overall and grade 3 HFSR incidence rate in this study (73 and 22%, respectively) was lower than in the CORRECT study Japanese subpopulation (80 and 28%, respectively). HFSR (grade ≥ 2) occurred in the first cycle or later in 42 and 11% of patients, respectively. HFSR accounted for 33 and 61% of first-cycle Regorafenib delay and dose reductions, respectively, and HFSR accounted for 40 and 53% in any cycle, respectively. \u0000Conclusion: Effectiveness of prophylactic topical steroids against Regorafenib-associated HFSR was shown in this study.
