This heart condition occurs following rheumatic fever caused by streptococcal infection. It affects the functionality of heart valves. When untreated it may lead to medical complications and even death. The disease mainly targeting children and young adults is responsible for cardiovascular morbidity and mortality in people. Prevention and treatment of acute rheumatic fever plays an important role in controlling the disease. Diagnosis of the disease is critically important because misdiagnosis of acute rheumatic fever can lead to worsening of damage caused to heart valves and may also lead to premature death. This being a preventable problem, it can be managed through surgeries to repair functions of cardiac valves. The disease is marked by severe thickened and scaly skin on the entire body. It is a lethal disease, but patients can very rarely survive for several months or years. We present here cardiac abnormalities in a full term baby with Harlequin ichthyosis. To our knowledge, this is first case study to report associated cardiac abnormality in such patients. The optimal approach to such patients in complex cases is not known. They pose special challenge to coronary revascularisation due to short stature, angle of their elbows, kyphoscoliosis, frequent obesity, and lack of specific equipments and experience. We present an interesting case of 67 years old man with achondroplasia who presented with significant angina symptoms and past history of IWMI. Transradial angiography showed chronic total occlusion (CTO) of right coronary artery. Coronary angioplasty was done via right radial approach using available hardware with good results. To our knowledge, this is the first case of radial angioplasty in an achondroplastic patient with CTO. {"title":"Endovascular Approach to Combined Ipsilateral Supra-inguinal and Infra-inguinal Artery Stenosis through a Single Common Puncture in a Single Sitting: Technique and Follow-Up","authors":"S. When both are diseased, crossover technique from opposite femoral arteries or upper limb approach is the preferred technique. However, lot of anatomical factors can affect reaching the area from opposite femoral artery or upper limb. The purpose of this study is to determine the safety, efficacy, mid-term clinical and radiological outcome of the endovascular treatment using retrograde and antegrade femoral artery approach in those patients who have significant stenosis of both supra and infra inguinal arteries, from a single ipsilateral common femoral artery puncture. Materials and Methods: Between 2015 and 2017, we performed angioplasty to lower limb arteries for 73 patients at our centre. Among them 17 patients had significant stenosis of ipsilateral supra and infra-inguinal arteries. 12 patients underwent ipsilateral retrograde femoral artery puncture to access iliac arteries under fluoroscopy guidance, followed by converting the same puncture into antegrade approach to tackle superficial femoral artery or popliteal artery. Rest of the 5 patients, had alternative approaches (left upper limb or contralateral femoral artery) because of the involvement of ipsilateral femoral artery at the puncture site. Results: Acute success rate was 100%. There were no significant peri-procedure complications. At the latest clinical follow-up (mean of 21.25 months), a restenosis rate of 16.67% in infra-inguinal arteries and none in supra-inguinal arteries. Some trace elements play role in the development of ACS. Aim: This study aimed to evaluate the linkage between serum zinc, copper, iron level and cardiac markers in acute coronary syndromes. Methods: Eighty patients were divided into four groups. Group I (patients with unstable angina), Group II (acute myocardial infarction early 6 h), group III (acute myocardial infarction late 6 h) and group IV (patients with reperfusion therapy). Fifteen apparently healthy individuals served as a control group. Different risk factors as age, sex, diabetes and dyslipideamia were addressed in different groups. The correlation between Fe, Zn and Cu versus cardiac enzymes were demonstrated. The receiver operating characteristic (ROC) analysis was conducted. Results: Male and diabetic patients were significantly higher. The CK and Tn were significantly increased in groups I, II and III. The CK-MB levels were significantly increased in group II and group III. The AST levels were significantly increased in group III. The serum iron level was significantly lower in group I, II and III. Serum zinc show slight decrease in groups II and III. No significant correlation was obtained concerning serum Cu. In group II, there were significant positive correlation between Fe versus Tn, CK-MB and CK. The (ROC) analysis was identified the optimal Fe, Zn and Cu plasma level for potential prediction of development of ACS. Conclusion: Fe and Zn values were lower in ACS patients. The aim of the present study was to investigate the association between components of cardio-metabolic risk and health-related quality of life in young adults. Methodology: Two hundred and sixteen (216) male and female young adults (18-25 years) were evaluated by using harmonized criteria for MetS (H_MS); HQRL was evaluated by using the SF-36 questionnaire. Results: The subjects in the MetS group reported lower scores in almost all subscales of quality of life. In the sex-specific analysis of both young male and female subjects showed significant differences in the mean scores of physical functioning (PF) and general health (GH) between subjects with and without MetS. Compared with men, women with MetS showed significantly lower values in role emotional (RE) subscale. PF correlated significantly with both systolic and diastolic blood pressure in young males with MetS whereas young females showed no significant correlation with PF. Similarly, GH significantly correlated with BMI and TG level in male subjects but females showed no significant correlation. Communitybased programmes with the help of community health workers (CHWs) have been tried to offer culturally appropriate care leading to positive impact on glycemic control. Methods: We planned this open-label randomised controlled trial to assess the possibility of CHWs interventions in a rural community in central India. We designed this study to compare glycaemic control, lipid profile, blood pressure and anthropometric measurements between patients in the CHW intervention group and those in the standard therapy group. The intervention group was offered CHW interventions in the form of home visits, health education, patient diary, telephonic reminders etc as compared to standard care group that received usual care. Results: We included a total of 299 patients in our study (standard care group n= 146 and intervention group n= 153) and followed them for a period of 6 months (96.98% follow up data) and recorded outcome variables (fasting blood sugar, post-prandial blood sugar, glycosylated haemoglobin, lipid profile, blood pressure) at the start and end of the study. Both the standard care group and intervention group showed improved in their glycemic indices at the end of the study. We established no statistical difference between the intervention and the standard care group at the end of the study. The mean reduction of HbA1c and fasting blood sugar was more in the intervention group as compared to the standard care group. The macrovascular complications of diabetes cause morbidity and mortality. Ankle brachial pressure index (ABPI) is a simple non-invasive tool which may indicate atherosclerotic changes in the blood vessels. It is useful in diagnosis of peripheral artery disease (PAD), and is also an indicator of increased cardiovascular morbidity and mortality. Previous studies have reported a correlation between ABPI and coronary artery disease (CAD). Despite the well-established association between diabetes and coronary artery disease, the role of ABPI in predicting the presence and severity of CAD in diabetics has not been proven. The study was done to find whether there was an association between ABPI and coronary angiogram in diabetics. Materials and Methods: This was a cross sectional study that included 100 patients of Diabetes Mellitus who had undergone a coronary angiogram during the current admission. All coronary angiograms were evaluated by a cardiologist. The ABPI was measured according to the standardized protocol before the patients underwent the coronary angiogram procedure. Results: Out of the patients, 4 patients (4.3%) had ABPI < 0.9 out of which 2 (50%) had double vessel disease and 2 (50%) had triple vessel disease. Conclusion: Though all patients with abnormal ABPI had CAD as assessed by CAG, not all patients with abnormal CAG had abnormal ABPI.
