The inclusion criteria involved studies published between 2000 and 2023 that analyzed the relationship between pesticides and child health, including systematic reviews, cohort studies, case-control studies and clinical trials. The descriptors used were \"pesticides,\" \"child health,\" \"prenatal exposure,\" \"environmental health,\" and \"developmental toxicity.\"</p><p><strong>Data synthesis: </strong>In this review, an association was observed between exposure to pesticides and the development of neurological and endocrinological diseases, childhood cancer and immunological diseases.</p><p><strong>Conclusions: </strong>Pesticides represent a significant risk to children's health, with impacts ranging from neurological alterations to chronic diseases. Medical charts were retrospectively reviewed for demographics, clinical manifestation, laboratory findings at diagnosis, outcome, and diagnosis of BCG vaccine-associated complications.</div></div><div><h3>Results</h3><div>Eleven patients diagnosed with SCID were enrolled. Ten were male. Seven (64 %) were considered probable SCID, while four (36 %) were considered definite SCID (genetically confirmed). The median age at the onset of symptoms was one month; the median age at SCID diagnosis was four months. Respiratory symptoms were the most frequent. Eight patients were vaccinated within seven days of life. Seven (87 %) of these patients experienced BCG vaccine-associated complications (86 % disseminated reactions; 14 % localized reactions). BCG vaccine-associated complications were the first clinical manifestation in 75 % of the vaccinated patients. Less than half of the patients (36 %) underwent hematopoietic stem cell transplantation. The overall death rate was elevated (73 %); the death rate related to BCG vaccination was 25 %.</div></div><div><h3>Conclusions</h3><div>Patients with SCID can present a high rate of BCG vaccine-associated complications, which negatively impact the clinical outcome and mortality. Pediatricians must be aware that BCG vaccine-associated complications can be the first presentation and a warning sign of SCID. This narrative review aims to provide an overview of wildfire smoke (WFS) characteristics and associated impacts on adults' and children's health.</p><p><strong>Data source: </strong>Literature review based on a bibliographic survey in PubMed (National Library of Medicine, United States), SciELO (Scientific Electronic Library Online), and Google Scholar databases. Observational, cross-sectional, longitudinal, and review studies were considered, prioritizing peer-reviewed articles published in the last 10 years (2014-2024).</p><p><strong>Data synthesis: </strong>Wildfire smoke (WFS) contributes to the deterioration of air quality, resulting in increased exposure to air pollution especially in wildland-urban interfaces. WFS contains particulate matter (PM) in a range of sizes and chemical compositions, as well as multiple toxic gasses. The health impacts of WFS are systemic, affecting the respiratory, cardiovascular, and neurological systems. Exposure to WFS is associated with inflammatory and oxidative stress, DNA damage, epigenetic modulations, and stress-disorders in adults and children. Children may be at an increased risk of WFS respiratory impacts, due to their smaller airways and developing lungs.</p><p><strong>Conclusion: </strong>Wildfires are increasing in frequency and intensity, resulting in thousands of premature deaths and hospitalizations worldwide, each year. Preventive measures against wildfire spread must be reinforced, considering the increasing trends of global warming and extreme weather events. {"title":"Climate change and its impact on children and adolescents sleep.","authors":"Maria Cecilia Lopes","doi":"10.1016/j.jped.2024.10.009","DOIUrl":"10.1016/j.jped.2024.10.009","url":null,"abstract":"<p><strong>Objective: </strong>This review discusses the impact of climate change on sleep, anxiety, and eating in the pediatric population.</p><p><strong>Data source: </strong>This is a nonsystematic literature review based on a search using PubMed and MeSH terms in titles and abstracts with these keywords: climate change, sleep, greenhouse effect, children, and adolescents.</p><p><strong>Data synthesis: </strong>Climate change events are associated with human intervention in the ecosystem, having a strong impact on cognitive functions, physical and mental health, as well as subjective well-being, particularly in youth. Climate change is caused by human activity with changes in the composition of the global atmosphere caused by emissions of gases, such as carbon dioxide, which increase the greenhouse effect. This review discusses the impact of climate change on sleep, anxiety, and feeding in the pediatric population.</p><p><strong>Conclusions: </strong>Early detection of vulnerability conditions, along with adaptation strategies is necessary to address climate stressors with a focus on healthy sleep and eco-anxiety. Carvalho","doi":"10.1016/j.jped.2024.10.008","DOIUrl":"10.1016/j.jped.2024.10.008","url":null,"abstract":"<div><h3>Objective</h3><div>To describe features and habits of diaper area care and compare the frequency of diaper dermatitis in infants using cloth diapers with those using disposable diapers.</div></div><div><h3>Methods</h3><div>Questionnaires were administered to families with infants who had not started potty training, to assess the frequency of diaper rash in two groups: babies who use exclusively cloth diapers (CD), and others with exclusively disposable diapers (DD). The hygiene methods of the perineal region and the skin lesions frequency were evaluated. The study was approved by the Ethics Committee.</div></div><div><h3>Results</h3><div>1389 participants were included, 53 % male, with a median age of 16 (7–24) months, 1269 (91.4 %) in DD and 120 (8.6 %) in CD. Mild diaper rash occurred a few times a year in 47.0 % and 47.5 % in the DD and CD groups, respectively (<em>p</em> = 0.47). Severe diaper rash occurred a few times a year in 13 % and 10.7 % in the DD and CD groups, respectively (<em>p</em> = 0.66). The authors aim to estimate the prevalence of NAFPD and its association with anthropometric parameters in a cohort of Chilean adolescents.</p><p><strong>Method: </strong>The authors conducted a cross-sectional analysis of the \"Growth and Obesity Chilean Cohort Study\" (GOCS), a longitudinal study involving nearly 1000 children, followed yearly since 2006. All participants underwent anthropometric measurements and abdominal ultrasonography.</p><p><strong>Results: </strong>A total of 741 adolescents were included; 30 exhibited ultrasonography findings compatible with fatty pancreas (4 %). Adolescents with NAFPD had higher BMI z-score (2.33 (1.52-2.69) vs 0.67 (-0.2-1.4), p < 0.001), waist circumference (WC) (90.9 (81.53-98.58) vs 72.2 (67.55-79.83), p < 0.001), waist-to-height ratio (0.55 (0.48-0.6) vs 0.44 (0.41-0.49), p < 0.001), triponderal index (17.35 (15.14-19.25) vs 13.62 (12.07-15.54), p < 0.001), subcutaneous fat (32.4 (21.77-44.95) vs 16.2 (9.3 - 25.3), p < 0.001), visceral fat (45.15 (36.92-62.08) vs 35.5 (28.55-44.25), p < 0.001), systolic blood pressure (p = 0.009), and diastolic blood pressure but only in boys (p = 0.004) compared with controls. The prevalence of liver steatosis was significantly higher in the NAFPD group (63.3% vs 5.2 %, p < 0.001). After adjusting for sex and BMI, only the association with waist circumference and liver steatosis remains statistically significant.</p><p><strong>Conclusion: </strong>In adolescents, NAFPD has a prevalence of 4 % and is associated with a higher BMI z-score, WC, superficial fat, and blood pressure levels. Due to the fact that the primary medical concern for children presenting with headache complaints is the possibility of intracranial pathology, nowadays, imaging methods are frequently used in those patients.</p><p><strong>Methods: </strong>Retrospective data analysis was performed on the records of children who attended the Pediatric Neurology Outpatient Clinic between June 01, 2018, and December 01, 2018, complaining of headaches. Children who had a headache for longer than four weeks and had brain magnetic resonance imaging were included in the study. Brain MRI findings were classified as (1) headache-related and requiring definitive intervention, (2) possibly headache-related abnormalities, (3) headache-related abnormalities that did not require intervention, and (4) normal.</p><p><strong>Results: </strong>The 387 patients included in the study were between the ages of 2 and 17, with a median age of 10.5 years. Of the patients, 234 were female and 153 were male. The duration of the headache was a median of 12 months. According to brain MRI findings, 253 patients (65%) were in group 4, 79 patients (20%) were in group 2, 54 patients (14%) were in group 3, and 1 patient (0.3%) was in group 1.</p><p><strong>Conclusion: </strong>The probability of detecting significant abnormalities with brain MRI in children with chronic headaches with normal neurological examination is found to be low.
