Ultrasound measuring of NT (Nuchal Translucency) or B level scanning depicts many aspects of fetal lesions. Anencephaly consists one severe and lethal congenital fetal lesion. Ultrasound imaging findings reveal absence of cranial structures, cornerstone of this lesion. The most predisposition factor concerning the diagnosis of such cases represents the level of experience of the fetal medicine specialist. Aim of our study consists presentation of a case of dichorionic, diamnionic twin pregnancy discordant for anencephaly. {"title":"Live Delivery Despite Abnormal Serial Rise in Serum Beta Human Chorionic Gonadotropin Levels and Sac Size Crown-Rump Length Discrepancy","authors":"J. Check, A. Diantonio","doi":"10.33425/2639-9342.1180","DOIUrl":"https://doi.org/10.33425/2639-9342.1180","url":null,"abstract":"There are several parameters that when inadequate, e.g., low serum progesterone (P), low serum estradiol (E2), or a sac size crown-rump length discrepancy with a sac size lagging behind by more than one week, are poor prognostic parameters for the delivery of a healthy baby. Perhaps the most important abnormal prognostic parameter is an inappropriate rise in serial beta human chorionic gonadotropin (hCG) levels. A case is described who had abnormalities in all these parameters including, in addition, issues with both increased and decreased fetal heart rates. She did however deliver a healthy baby just three weeks before her due date. Her treatment consisted of aggressive P supplementation, plus estrogen, antibiotics, and dextroamphetamine sulfate. One cannot say for certainty whether any or all of these treatments were responsible for her good outcome. {"title":"Comparison of Amniotic Fluid Oxidative Stress Index in IVF/ICSI Pregnancies and Spontaneous Natural Conception","authors":"Murat Önal, M. Ağar","doi":"10.33425/2639-9342.1185","DOIUrl":"https://doi.org/10.33425/2639-9342.1185","url":null,"abstract":"Comparison of Amniotic Fluid Oxidative Stress in IVF/ICSI and ABSTRACT Objective: To compare amniotic fluid total oxidant status (TOS), total antioxidant status (TAS), and oxidative stress index (OSI) values of spontaneous pregnancies and IVF/ICSI pregnancies. Materials and Methods : A total of 50 patients who became pregnant spontaneously or by IVF/ICSI were included in the study. Twenty of the fifty participants consisted of patients who became pregnant by IVF/ICSI (n=20). The remaining 30 patients consisted of patients who became pregnant spontaneously (n= 30). Following the classical lower segment incision during C/S delivery, amniotic fluid sample was taken with a 5 cc syringe before the fetal membranes were cut. TAS and TOS levels were studied in an autoanalyzer by using TAS or TOS kits. OSI value was obtained by dividing TOS by TAS (TOS/TAS=OSI). OSI results are presented as percentages. Results: The amniotic fluid TOS levels of the patients in the ICSI group were found to be significantly higher than the pregnant women in the control group (23.11 ± 4.55 vs. 20.30 ± 6.05, p<0.01). On the other hand, amniotic fluid TAS levels of the pregnant women in the ICSI group were found to be significantly lower than the patients with spontaneous pregnancy (0.84 ± 0.10 vs. 0.96 ± 0.32, p<0.03). The OSI values (TOS/TAS) of the patients who became pregnant with ICSI were found to be significantly higher than the patients with spontaneous pregnancy (27.51 ± 6.33 vs. 21.1 ± 8.20, p<0.02). There was no significant correlation between OSI values and obstetric and neonatal parameters. There was no significant correlation between OSI and age, gravidity, parity, birth weight and gestational age at birth. Conclusions: The amniotic fluid oxidant/antioxidant balance of IVF/ICSI pregnancies was pathologically increased compared to natural conceptions. Adequate scientific based knowledge about menstruation and its relationship to fertility is crucial in attaining optimal reproductive health. Aim: We evaluated the perception and knowledge of African Nigerian women towards the relationship between menstruation and fertility. Materials and Methods: It was a prospective cross-sectional household survey with multi-staged cluster sampling design in which 230 randomly selected, consenting women in September and October 2012 were assessed about their perception of menopause using a structured questionnaire. Findings: We found that 59% of women thought that if a woman does not menstruate, she definitely cannot have a baby, 24% claimed that women with menstrual pain are likely to have miscarriages, while 21% believed that women with menstrual pain will have delay/difficulty getting pregnant. About 3/4 was of the opinion that it is morally wrong to have sex while menstruating and 76 (33.6%) of the women were of the opinion that women who take medications to stop their period are likely to develop cancer later in life. One-fifth of the women think there is no link between menstruation and family planning. Conclusion: Perceptions and knowledge about menstruation is varied among many Nigerian women, with widespread misconceptions and inadequate knowledge about its relationship with fertility. Jackson","doi":"10.33425/2639-9342.1174","DOIUrl":"https://doi.org/10.33425/2639-9342.1174","url":null,"abstract":"Objective: To determine if ABO blood group, age, body mass index (BMI), or symptomatic COVID-19 infection are associated with COVID-19 antibody response in unvaccinated COVID-19 antibody positive pregnant women at time of delivery. Methods: At the time of delivery, 2,499 consecutive pregnant women were tested for ABO blood group and antibodies to both the spike protein and nucleocapsid protein of the COVID-19 virus. The DiaSorin assay was used for antibody to spike protein analysis and the Roche assay for antibody to nucleocapsid protein. Gamma regression models with a log link were used to compare antibody signals, with blood group, age, and BMI as the predictors. Results: 260 (10.4%) of 2,499 women who had not been vaccinated for COVID-19, were positive for both spike and nucleocapsid protein antibodies to COVID-19. The mean signal for COVID-19 nucleocapsid antibody was significantly lower for blood group AB (p=0.028) compared with blood group O. A relationship between blood group and presence of symptomatic COVID-19 was detected (p = 0.028), with asymptomatic individuals having blood group B at a higher rate than the symptomatic individuals do. No other significant pairwise differences between blood groups were detected. There was no significant difference in signal level of antibodies to COVID-19 spike protein between any of the blood groups. Mean signals for antibodies to spike and nucleocapsid proteins were significantly higher in older women (p=0.001 for spike protein antibody and p=0.002 for nucleocapsid antibody). Significantly higher signal levels of antibody to spike and nucleocapsid proteins were found in women with class 2/3 obesity (p=0.022 and p=0.003, respectively). Conclusions: Pregnant women of AB blood group had lower antibody signal to nucleocapsid protein compared to the O blood group, and women of older age and greater BMI had higher antibody signal to COVID-19 spike and nucleocapsid proteins.
