We aimed to investigate the risk factors affecting the progno-sis of patients with decompensated hepatitis B cirrhosis and their predictive values.The clinical data of 149 patients with decompensated hepatitis B cir-rhosis, treated from August 2015 to June 2019, were analyzed retrospectively. They were divided into death and survival groups during a one-year follow-up. Their baseline data were compared, and the risk factors affecting death, corre-lations among risk factors, and predictive values of these factors for death were analyzed. Survival analysis was conducted. During a one-year follow-up, 103 pa-tients survived, and 46 died. High neutrophil-lymphocyte ratio (NLR), red cell distribution width (RDW), the model for end-stage liver disease (MELD) score, the Child-Turcotte-Pugh (CTP) score, and low serum sodium were independent risk factors for death in patients with decompensated hepatitis B cirrhosis. NLR correlated positively with CTP and MELD scores (r=0.346, p=0.0001, r=0.243, p=0.0003, respectively). Likewise, the RDW had positive correlations with CTP and MELD scores (r=0.417, p=0.0001, r=0.413, p=0.0003, respectively). Se-rum sodium was negatively correlated with CTP and MELD scores (r=-0.484, p=0.0001, r=-0.476, p=0.0001, respectively). The survival rate was high in patients with NLR<7.38, RDW<16.15%, serum sodium>146.31 mmol/L, CTP score<10.26 points, and MELD score<11.31 points (p=0.0001). NLR, RDW, serum sodium, MELD, and CTP scores had high death predictive values.NLR, RDW, serum sodium, CTP score, and MELD score can be considered as critical indices for evaluating and predicting the prognosis of patients with decompen-sated hepatitis B cirrhosis.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42008628","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Numerous studies indicate the influence of matrix metallopro-teinase-2 (MMP-2) overexpression in osteosarcoma (OS) outcomes. A previous study has systematically analyzed the correlation between MMP-2 expression and the prognosis of OS. However, the results of subsequent studies remain in-consistent. Therefore, a meta-analysis in terms of the prognostic value of MMP-2 expression in OS was conducted. We employed the Newcastle-Ottawa scale (NOS) to evaluate the quality of the studies. Five studies involving 284 patients were included. The relative risk (RR) with a corresponding 95% confidence in-terval (95%CI) was calculated to appraise the predictive value of MMP-2 positive expression for OS recurrence and metastasis, and lower disease-free survival.It was indicated by the results that MMP-2 positive individuals with OS had higher recurrence and metastasis rates than negative individuals (RR=1.85, 95%CI:1.16-2.93, p<0.01). Sensitivity analysis showed that the combined RR was stable. There was no significant change, independently of whichever article was excluded.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46909848","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Studies have shown that regular physical activity (PA) may re-duce the risk of stroke. Nevertheless, the protective effect of pre-stroke PA on the post-stroke functional status in patients treated with endovascular throm-bectomy (EVT) is still uncertain. For this study, hospitalized patients with acute ischemic stroke were selected, all of whom underwent EVT after occlusion of the large vessels in the anterior circulation. The PA levels before the stroke were assessed by the International Physical Activity Questionnaire, and the outcome indicators (the modified Rankin scale score and the Barthel index) were evalu-ated. A total of 144 patients were included in this study. Among the patients with high PA levels before the stroke, men were more active than women, and the proportion of patients with smoking and alcohol abuse was high before admission. Also, the patients with a high PA level were more likely to have no history of hypertension and atrial fibrillation, and had lower triglyceride levels at admission. We determined that a high pre-stroke PA level is independently related to a favorable functional outcome at three months. In addition, there was no correlation between a high pre-stroke PA and a good functional outcome at two weeks. A high pre-stroke PA level is independently associated with sat-isfactory activities of daily living at three months. Regular PA before a stroke is an independent predictor of a favorable functional outcome in patients with anterior circulation strokes who receive EVT, and it has a protective effect on the functional prognosis.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46716096","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

One was the withdrawal of journals by the Web of Science for not carrying out adequate peer review processes and publishing articles lacking in scientific rigor in exchange for high publication costs. In fact, it can be verified that it is common to find works that appear accepted just two or three days after the date of receipt, making it doubtful of an adequate review. Many advocate “Open Science” as the freedom to commu-nicate results without the consent of referees or the sometimes very rigorous requirements of scientific journals. Another example case was that of a researcher sanctioned by his Uni-versity for considering the excessive authorship of works published by the said individual in a very short time, which seems to be doubtful. It is common to find cases of researchers advising working groups from various countries outside their country of origin, perhaps only as statistical analysts or offering their recognized names to stimulate the citation of an in-stitution or country. It would then be appropriate to ask the question: What would be the number of publications that an author may have so that it is not suspected of gratifying or convenient authorship, for himself or for the institution to which he belongs? On the other hand, it is known that authorship of papers may be obtained in high-impact journals through payments or citations that do not correspond to the topic dealt with in the work. Indeed, we have discovered some contributions with out-of-context citations in Investigación Clínicathrough a rigorous analysis of the references. Worse still, it is the communication, carried out by any individual who is not an expert in a field, through the most popular social net-works, of concepts that deeply permeate public opinion in general, being transmitted as if they were proven facts and therein the danger, in the specific case of Medicine, damage to health. For all of the above, the Editors of scientific journals must be very vigilant and ready to discuss the advantages or inconveniences of modern initiatives, the influence of social media in disseminating information, and knowing how to interpret their impact on public health. On the other hand, Journals have to balance the role of citations and impact indexes in the projection of Science individuals, as some academic institutions in European (Spain) or Asian (China) countries are doing.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46102634","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An analytical, cross-sectional observational study was carried out on a group of 30 patients with T1DM between 8 and 16 years of age and on a control group of 30 individuals matched by age, gender, education, and socioeconomic status. Interrogation and review of medi-cal records to obtain data on the clinical characteristics and treatment of T1DM were conducted. The WISC IV test was then applied to evaluate cognition and intellectual coefficient (IQ). The average age of the diabetic patients was 13.27±2.31 years, and half of them were male. Lower scores were found in the different domains of the WISC IV in the group with T1DM (p<0.01). The IQ was found to be lower in children with T1DM than in con-trols (75.47±13.87 vs. 88.57±11.06; p=0.0001). Likewise, a higher fre-quency of IQ scores below the 10th percentile was observed in the diabetic children (63.3% vs. 33.3%; p=0.02; Odds ratio: 3.45; 95%CI: 1.19-9.99). It was concluded that T1DM negatively impacts the cognitive performance of children and adolescents. Cognitive evaluation of these patients is recom-mended, as it could affect their daily life.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46536122","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The search was conducted using Medline/PubMed, Scholar Google, Clinical Trails, ProQuest, Scopus, and Web of Science databases. During the search, 7,926 records were found, of which 105 full-text articles were analyzed, and 23 studies included analysis in patients with the use of POP, and their control groups without the use of POP prior to the same type of surgery. The devices most used for POP in the management of LPH were: the passive palate (Hotz palate), the nasolaveolar moulding (NAM), the McNeil device, and finally, the T-traction. The primary outcomes evaluated were: facial aesthetics and nasal appearance; the evaluation of cephalometric measurements, upper airways, nasal fissure width, as well as anatomical references such as inter-canine and inter-tuberosity distances. In addition, studies that evaluated occlusion and phonation were found. The evidence from this literature suggests that the use of active appliances had a better effect than passive appliances in terms of facial aesthetics and approximation of the maxillary seg-ments for the closure of the fissure. However, the heterogeneity, the risk of bias, and the low quality of the studies do not allow to state firm conclusions.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"45992260","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Their inflammatory factors, stress response, intestinal mucosal bar-rier, hemorheological indices, recovery time, length of stay, clinical efficacy, and adverse reactions were compared. The levels of serum interleukin-10 (IL -10), catalase and glutathione peroxidase rose in the three groups after ten days of treatment, compared with values before treatment, being the highest rise in group C. The levels of IL -18, tumor necrosis factor-α, soluble intercellular adhe-sion molecule-1, procalcitonin, high mobility group protein B1, lipid hydrogen peroxide, advanced oxidation protein products, epinephrine, cortisol, D-lactic acid, diamine oxidase, and endotoxin decreased after ten days of treatment compared with those before treatment, which were lowest in group C (P<0.05). After ten days of treatment, the levels of hemorheological indices were signifi-cantly lower than those before treatment (P<0.05). Compared with groups A and B, group C had a shorter recovery time of urine amylase, bowel sound and passing gas, remission time of abdominal pain, length of stay, and a higher to-tal response rate (P<0.05).During AP treatment, somatostatin combined with early hemoperfusion effectively relieved inflammatory and stress responses, protected the intestinal mucosal barrier function and improved the hemorheol-ogy, thereby promoting the recovery and benefiting the prognosis of patients.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42470794","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The main ob-jective of this study was to evaluate the amount and species of microorganisms isolated from necrotic pulps, establish a correlation between these and the size of periapical lesions, and how the amount and species of microorganisms decreased with non-surgical root canal treatment. Twenty-seven patients with a clinical diag-nosis of dental pulp necrosis and chronic periapical lesions were selected; a Cone Beam Computed Tomography (CBCT) and microbial samples of the root canal system were taken previous to a disinfection protocol, a post-instrumentation/disinfection protocol, and a post-medication placement. Samples were processed for colony-forming unit (CFU) counting, Gram staining technique, and bacterial identification by the API-20 Strep/API-20A system. The API system identified 21 species of microorganisms in the pre-instrumentation samples, 11 species in the post-instrumentation samples, and 11 in the post-medication samples. There was a correlation coefficient of 0.598% between the initial size of the lesion and the number of bacteria, with a coefficient of determination up to 35.7%, a correla-tion coefficient of 0.486% and a determination coefficient of 23.6% between the size of the periapical lesion and the number of CFUs. This study contributes to the knowledge of the amount and species of microorganisms isolated and identi-fied from necrotic pulps, establishes a correlation between the amount and spe-cies of microorganisms and the size of the periapical lesions, and shows how the decrease of microorganisms contributes to the healing of PL, corroborating the importance of an adequate disinfection protocol.","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46174346","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In order to analyze its projection in the current scientific field, we evaluated on its official “website”, the period of time between January 2011 and December 2020, in a general way and sepa-rately in two five-year periods. Investigación Clínica has been visited by 18,513 individuals from 104 countries on five continents. As expected, the country that made the most visits was Venezuela, followed by Mexico, the United States, Spain and Ecuador (5,081, 4,265, 1,389, 1,262 and 1,134, respectively). In the first five-year period (2011 to 2015), very similar results were found to those of the global analysis; there were visits from users from 93 countries, and in the distribution, Venezuela again ranked first, followed by Mexico, the United States, Spain and Colombia (3,366, 3,103, 746, 671 and 368 respectively). In the second five-year period (2016 to 2020), the network was visited by users from 86 countries in the following order of frequency: Venezuela, followed by Mexico, Ecuador, the United States and Spain (1,715, 1,162, 1,024, 643 and 591 respectively). The experience with the use of the “Network” or the “Web” has been very positive, in making the Journal known in distant countries, as can be seen with the appearance of authors and papers from countries such as Mexico, Ecuador and Colombia, in America and others who come from distant countries such as Iraq, Syria, Iran, and in recent times, China","PeriodicalId":14514,"journal":{"name":"Investigacion clinica","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44084093","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
