We are reporting a unique presentation of bilateral ptosis with external ophthalmoplegia in a patient of a 17-year-old female patient who came with chief complaint of drooping of lids in both eyes for 3 years. Complete ophthalmic and systemic examination was done. Patient was diagnosed as MS according to the revised Mcdonald criteria and was given Methylprednisolone Pulse therapy. We concluded that acquired bilateral ptosis with ophthalmoplegia externa should be given special attention to rule out systemic associations. These symptoms are related to the alterations in the layers of tear film, insufficient production or excessive evaporation of tear film. The primary aim of this study is to diagnose and to find out the incidence of dry eye in patients presenting with symptoms of ocular irritation. A total number of 225 patients of both sexes aged between 20 and 80 years with complaints of ocular irritation were included in this study. Detailed history taking, slit lamp examination of the anterior segment, tear film breakup time (TBUT), Schirmer’s test, Fluorescein stain score and Rose Bengal stain score was done for all patients. Data was presented only for the worst eye of each patient. Dry eye was found in 46.67% of patients. Among symptomatic patients, 24% had TBUT value < 10 sec. Fluorescein score was ≥ 1 in 6.6% of symptomatic patients. RB score ≥3 was found in 28.4%. Both Schirmer test and TBUT were positive in 22.7% of the symptomatic patients. Only 4.9% of symptomatic patients were positive for all 3 tests including staining. Corneal, conjunctival margins and lamellar sclera was evaluated histopathologically. Twelve eyes of 11 patients with mean age of 59.2 ± 12.2 were studied. Hyper reflective epithelium, increased thickness, abrupt transition of corneal epithelium and increase back shadow were the features of OSSN noted. The mean functional epithelial thickness and epithelial thickness were 137.5± 60.6µm and 58.5±10.8 µm respectively. The histopathology confirmed as squamous cell carcinoma (SCC) (n=4, 33.3%), carcinoma in situ (CIN) (n=8, 66.6%). In CIN 2 eyes were severe dysplasia and 6 were moderate dysplasia. Corneal clearance was noted as 100 %. Of 36 conjunctival margins, the positive margin noted in 6 eyes (50%). Nasal margin (n=3) and temporal (n=3) were the common margins. There was no difference in clearance between CIN (50%) or SCC (50%) on conjunctival side. Two eyes with recurrence required and revision excision subsequent topical chemotherapyOptical biopsy by SD OCT though is an effective method for OSSN removal. A prospective, interventional study of 279 clinically diagnosed and or histo-pathologically confirmed eyelid mass lesions. At presentation, detailed demographic data, symptoms, duration since the mass noted, it’s location, size & physical appearance, progression/regression/recurrence of the lid mass if any and associated systemic ailments were noted. Then, a pre-treatment clinical diagnosis (PTCD) was made. The treatment given, its outcome and histo-pathological examination (HPE) report were noted. Results then analysed in terms of how many cases needed HPE and in how many did PTCD match with HPE reports. Commonest lesions were of infective type (64.5%) followed by cysts and neoplastic which together contributed for 26.9% cases and others like congenital, traumatic, depositive etc., contributed for 8.6%. Totally, 25.8% sent for HPE; 94.4% did match with PTCD. Accuracy in clinical diagnosis is good with a thorough understanding of physical characteristics of lid mass lesions. Using the Preferred Reporting Item for Systematic Review and Meta-Analyses (PRISMA) criteria, a total of 12 research papers were chosen for thorough evaluation after the exclusion criteria. Spherical equivalent refraction and axial length were outcomes of myopia progression. Time spent using digital devices and time spent exercising outside were examined as factors. Greater stress, increased use of electronic gadgets like laptops, tablets, and cell phones, as well as less time spent outside, have all been linked to an increase in the prevalence of myopia. Additionally, we discovered that the COVID-19 pandemic's commencement significantly changed children's behaviors since there was a switch from in-person instruction to online learning and a decline in outdoor activities because of home quarantine. : In children who participated in remote learning during the COVID-19 lockdown, increased reliance on digital devices for online classes has either caused or worsened visual disturbances like the rapid progression of myopia, symptoms of dry eye and visual fatigue, and vergence and accommodation disturbances. The best corrected visual acuity was counting fingers close to face (CF-CF). Slit lamp examination revealed ciliary tissue prolapse with gaping of incision scar along with a posterior synechia, iris pigments, vitreous in Anterior chamber, cells and flare. Patients presented aphakic and had the extruded rigid PMMA IOL with a broken haptic with him. Fundoscopic examination and B scan imaging were suggestive of vitreous haemorrhage. Pseudophacocele happens following blunt trauma leading to dehiscence of the corneo-scleral wound scar. Subconjunctival dislocation is the most common type, however sub-choroidal dislocation has also been seen. Childers with dermoid tumour persent with other congenital abnormality 13 year old girl presented to our OPD, with complains of swelling in her left eye upperlid since birth, patient has multiple facial anomalies. On ocular examination, her vision in both eyes were 6/6. A 3*3cm noted in the left upper lid near the lateral canthus. Clinical diagnosis of Dermoid cyst left eye upperlid made. A surgical excision biopsy was planned under General anaesthesia. Intraoperatively the cyst removed with the cyst wall intact. Cut section revealed cystic lesion with tangle of hair follicles. Specimen sent for histopathology. HPE confirmed the diagnosis of teratomatous dermoid cyst. Dermoid cysts account for 5% of all orbital lesions and are among the most common orbital tumors of childhood. This longitudinal study was reviewed all patients with culture-proven fungal keratitis over a year at a tertiary referral facility in Bangladesh. All of the patients who had corneal stromal infiltrates had their corneal scrapings subjected to the normal microbiologic examination process, as well as a smear and culture-guided antimicrobial medication. Epidemiological features of patients (n=100), the mean age of patients was 55.6±9.8 years. More in the age group 51 to 60 years (28%). Males were more predominant (67%), commonly come from rural areas (59%). More were agriculture in workers (57%). Predisposing factors in fungal keratitis were trauma with vegetative matter (58%), herpetic keratitis (16%), penetrating keratoplasty (10%), extended wear of contact lens(12%), LASIK(4%). Treatment taken by another ophthalmologist before the presentation (46%). Low vision (20/50 to 20/160) was found in 58% cases and legal blindness (20/200 to worse ) was 40% of cases on visual acuity test at presentation. Filamentous Aspergillus (68%), Fusarium (2%), Scedosporium species (6%) and Candida(20%) were found by doing culture. Surgical intervention was needed in 36% of cases with topical and systemic anti fungal medications for management. Visual acuity after treatment was 20/40 or better in 16% of cases, low vision (20/50 to 20/160) in 39% of cases and legal blindness (20/200 to worse) in 45% of cases. Fungal keratitis causes irreparable vision damage. Yeast infections were less common than infections with filamentous fungus, which were typically treated more aggressively through both medicinal and surgical means. Similar durations of infections and visual results were observed in filamentous and yeast keratitis. Accommodative and vergence facility measurements were taken with accommodative and vergence flippers.This study was done on 50 subjects, age group between 18-30 Years and it was approved by the IRB committee. This study has done to compare the values of accommodative facility and vergence facility between myopic emmetropic patients. The accommodative facility was measured using accommodative flippers. It was done both monocularly and binocularly for each subject at 3m and 40cm. Vergence facility was measured using vergence flippers. It was confirmed that Accommodative facility [Distance and Near] in right eye, left eye, and botheye was observed as significantly lesser in Myopic patients than Emmetropia. Also, the Vergence facility wasobserved as significantly lesser in Myopic patients than the Emmetropic patients.A reduced accommodative facility and vergence facility was found in myopes in comparison to emmetropia. Patients usually present with signs and symptoms derived from the involvement of structures within the orbital apex, the superior orbital fissure, and the cavernous sinus. Visual loss from optic neuropathy and ophthalmoplegia involving multiple cranial nerves are the hallmarks of orbital apex syndrome. Historically, superior orbital fissure, orbital apex, and cavernous sinus are being used to define the anatomical locations of a disease process. However, the diagnostic evaluation and management are similar for each of these entities.
