There is premature fusion of cranial sutures which leads to restriction of intracranial and orbital space expansion giving characteristic dysmorphic facial appearance. We are reporting two cases of Apert syndrome of different age groups from ophthalmic point of view, and with different sets of ocular manifestation. Of the two cases, the one who presented early had a better visual prognosis. Apert Syndrome has a social stigma and the patients often suffers social and psychological disturbances. An 8 year old female presented to us with a swelling of left superolateral orbital region progressively increasing for past 3 years. CT scan was performed which showed a well-defined non enhancing cystic lesion with thin internal septations in extraconal space of left orbit, in superolateral compartment. Although definitive diagnosis was not clear but the CT findings were consistent with a dermoid cyst. Surgical excision of mass was done through a sub-brow approach and histopathological examination of sample was done which confirmed it to be a dermoid cyst. AS-OCT was done preoperatively to detect pre existing dehiscence of posterior capsule in patient of PPC. The images obtained by AS-OCT were divided into either intact or dehiscent. Integrity of posterior capsule was also observed during operation by surgeon.In 26 eyes, dehiscence of posterior capsule was detected in 4 eyes (15.4%) by AS-OCT pre operatively, of posterior capsule in 3 eyes (11.5%) and intact posterior capsule in 23 eyes (88.5%). Rupture of posterior capsule occurred in 4 eyes during phacoemulsification. 3 out of 4 eyes who had rupture, had dehiscence of posterior capsule that was noted by AS-OCT and also intraoperatively while 1 eye was detected to have intact posterior capsule. The sensitivity of AS-OCT was 75% for detecting posterior capsule dehiscence and specificity was 95.65%.: AS-OCT can be used preoperatively to identify posterior polar cataract that are at risk of posterior capsule rupture. The most crucial decision in the management of rhino-orbital mucormycosis is when to advise exenteration. This study aims at understanding the pattern of cases which progressed and had extensive involvement of the entire orbital cavity with mild intra cranial extension requiring exenteration as life salvage measure in the setting of epidemic of mucormycosis in a pandemic of COVID – 19. Patients suspected or diagnosed with invasive orbital mucormycosis visiting our hospital were admitted and included in the study. CT PNS with orbit were done for all patients and MRI brain with orbit were done in patients in whom intracranial spread and involvement of optic nerve were present. Patients were started on liposomal intravenous amphotericin B (5mg/kg/day) and intraorbital amphotericin B injection when indicated and closely monitored for clinical progression. All patients were taken up for FESS and if positive for mucormycosis on histopathological examination. In cases wherever exenteration was indicated, after confirming that there was no perception of light and obtaining informed consent with psychiatric counselling patient was taken up for orbital exenteration. Lid sparing exenteration was the method of choice. A total of 41 patients out of 696(5.89%) underwent exenteration out of which 21(51.2%) were right sided and 20(48.8%) were left sided. 12 patients (29.2%) of those who had undergone exenteration did not have a history of being tested positive/ been symptomatic for COVID 19. 1 patient (2.4%) was not diabetic. All patients (100%) had sinonasal involvement (Involvement of one or more paranasal sinuses) with involvement of orbital cavity to different degrees. 4 out of 41(9.75%) patients had deteriorated and required ICU care. 1 patient succumbed to death (2.4%) due to multiple comorbidities. On histopathological examination of exenteration specimen, 2(4.87%) patients turned out to be negative for mucormycosis. In 32(78.04%) of them the socket healed well while 5(12.1%) had slough who required 2 or more sittings of sloughectomy. As mucor is angio invasive rapidly progressive highly fatal infection close monitoring for invasion into structures like apex of orbit and intra cranial spread is essential for timely intervention and decision to perform exenteration. The weight gain data was collected for those who developed any grade of ROP-these babies were followed up for spontaneous regression or given laser treatment for Type 1 ROP.Type 1 ROP which required treatment was termed as Severe ROP.The weight gain parameters analysed were absolute weight gain at 2,4 and 6 weeks and weight gain proportionWe included 94 babies were included in the study. Out of 94, in 73 babies spontaneous ROP regressed was noted and 21 developed severe ROP. Mean birth weight in treated babies and non-treated babies were 909.29 ± 210.88 grams and 1107 ± 375.00 grams respectively. Absolute weight at 2,4 and 6 weeks were significantly low in severe ROP. Weight gain proportion at completed 2,4,6 weeks of life showed that at 4 weeks it was significantly (p 0.025) low in severe ROP. In this retrospective observational study the case records of 243 infants who visited our ROP clinic between January 2020 and august 2021 were reviewed. The mean GA was 33.4 weeks and the mean birth weight was 1.3 kg. A total of 55 (22.82%) patients developed ROP. low birth weight (p value = <0.001), low gestational age (p value = <0.001), multiple pregnancy (p value 0.002), blood transfusion (p value = 0.033), sepsis (p value = 0.009, odds ratio = 2.29 at CI 95%), oxygen supplementation (p value = <0.001 odds ratio= 10.56 at CI 95%) and duration of oxygen (p value = <0.001, odds ratio =1.93 at CI 95%) were significant risk factor for development of ROP. 2 (3.63%) cases were zone I, 8 (14.54%) zone II, and 45 (81.81 %) in zone III. Out of which 8 had plus disease. This case report was done in a tertiary care eye hospital in Bangladesh. We evaluated the patient thoroughly. CT scan of the orbit and histopathology were helped to diagnose the case. Per operative video recording, preoperative and postoperative photograph was documented properly. : A young boy presented with gradual swelling in the right upper eyelid. Clinically a firm mass was found in the right upper eyelid. CT Scan of the orbit revealed a cystic lesion in the eyelid. A live worm was extracted from the right upper eyelid. Parasitic lesion may be occurred in the eyelid. Best-corrected visual acuity in Right eye 20/200 and Left eye 20/120. On examination, Right eye has a 6 x 6 mm fleshy pink oval pedunculated mass lesion arising from the temporal limbus and a 2mm hyperpigmented lesion in the nasal limbus. Left eye has a gelatinous white flat diffuse lesion with the pigmentation of 3mm both in the nasal and temporal limbus. Both eyes' Anterior segment, ocular motility, and fundus are normal. The patient had multiple hyperpigmented spots (freckles) and scaly skin on the face and trunk suggestive of Xeroderma pigmentosum for which he was on irregular treatment. Right eye tumor mass was excised with intraoperative control of surgical margins with adjunctive cryotherapy and sent for histopathology examination, which was confirmed as squamous neoplasia. Prospective, observational study in the time period. We included 37 eyes of 37 patients with treatment naïve diabetic macular edema who subsequently underwent treatment with injections of intravitreal Ranibizumab. Inclusion criteria was: Presenting vision < 0.3 logMaR (2) centre involving diabetic macular edema (CMT>250 microns). 3 injections were given monthly irrespective of treatment response followed by PRN dosing. Factors under evaluation were: 1) Submacular detachment (SMD) (2) cystoid changes in the inner and outer nuclear layer (4) Integrity of IS-OS junction. (5) Hyper Reflective Foci(HRF) and location (6) vitreomacular interface (7) Disruption of inner retinal layers(DRIL) We included 37 eyes in our study. For analysis, based on final visual acuity patients were divided in two groups: (1) Group A: Good visual acuity (>10 ETDRS letter gain) and (2) Group B: Poor visual outcome (< 10 letter gain). Presence of SMD was more likely in Group B as compared to Group A. There was no significant difference between two groups for other mentioned parameters.
